All ships 500 Gross Tonnage and upwards shall be fitted with a gyro-compass bearing repeater, or other means, to take bearings, over an arc of the horizon of 360 degrees. Vessel had a non-operable starboard bridge wing gyro-compass repeater, thus preventing them from taking readings over an arc of the horizon of 360 degrees. 74 SOLAS (Cons. 2020) V/19.2.5.3

All lifesaving appliances shall be fitted with retro-reflective material where it will assist in detection and in accordance with the recommendations of the IMO. Vessel did not have retro-reflective material on the underside of the recuse boat to assist in detection in the event of a capsize. LSA Code 1.2

An International Pollution Prevention Certificate for the Carriage of Noxious Liquid Substances in Bulk shall be issued to any ship intended to carry noxious liquid substances in bulk. Vessel is restricted from carrying NLS cargo in bulk.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. CO2 bottles were found loose at connection.

Ships shall be provided with at least one international shore connection. The international shore connection was found missing bolts. SOLAS 74 Cons (2020) II-2/10.2.1.7.1

A general emergency alarm system shall be provided and used to summon passengers and crew to muster stations. General emergency alarm did not operate. SOLAS 74 Cons (2020) III/6.4.2

Connections within fuel supply and spill lines shall be constructed having regard to their ability to prevent pressurized oil fuel leaks. Anti-splash tape was found missing from starboard generator. SOLAS 74 Cons (2020) II-2/4.2.2.5.4

Ready means of escape shall be provided from all spaces. Aft steering room emergency escape was seized. SOLAS 74 Cons (2020) II-2/13.2.1

The owner or operator of an OSV shall make the vessel available for inspectiom by a marine inspector each time an accident occurs. The inspection is to ensure that necessary repairs or renawals have been effectively made; the material and workmanship used to accomoplish the repairs or renawals are satisfactory; and the OSV complies with the regulations. A wire spool was dropped on the aft deck resulting in an approximate 30 X 40 ft buckle between the vessels midship and aft deck, above liquid mud tanks and dry bulk tanks. Vessel also sustained damage below deck. Vessel is to proceed to North American Shipyard in LaRose, LA to conduct repairs, and will clear upom satisfactory completion of all repairs and receiving a class report.

Electrical equipment on a vessel must be designed, installed, maintained to protect crew members and vessel from electrical hazards, including fire. The following electrical equipment needs repair/maintenance; missing non-conductive mat for e-generator, latches missing on electrical box on port generator, missing plate on outlet for port main engine, and open wires in communications room. 46 CFR 129.200(b)

Every other ship of 400 gross tonnage and above shall be subject to an initial survey before ship is put into service under this annex. vessel was missing IOPP. May not go on international voyage until IOPP is received from class society. MARPOL Annex I Regulation 6

Adjust all quick acting watertight doors.

Renew gasket in lower Engine Room aft watertight door.

Renew gasket and free-up Steering Room escape hatch.

Replace 3 flexible hoses for the Engine Room fixed CO2 bottles.

Renew floatlight for stbd aft lifering.

Provide one lantern for the Laundry Room fireman's outfit.

Replace the fuel return flex hoses on the stbd main engine, port ship's service generator, and the drop down thruster in accordance with SAE J-1942.

Provide maintenance and make operable the hinged stack covers.

Remove or service Type V semi-portable extinguisher stbd fwd main deck.

Reconfigure ventilation system located in the A/C space containing batteries IAW 46 CFR 127.260. LEGACY COMPONENT DESC: Batteries

Provide updated Fire Control Plan for CG approval.
