A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made and that the material and the workmanship of such repairs or renewals are in all respects effectively made; vessel was involved in an allision with a Navy RHIB vessel while docked at NAS Pensacola pier sustaining a 1.5 inch deep over 1 ft span deformation above the waterline on the stern side shell in way of the stbd #08 ballast tank. No other damage was reported. Vessel is to undergo ABS survey and damage assessment and provide report to Sector Mobile prior to vessel movement.

Vessel underwent ABS survey and damage assessment and provided report to Sector Mobile. Vessel permitted to make permanent repairs no later than the next drydocking survey. Notify CG inspection office as CG inspector will need to oversee repairs.

The system must be arranged and installed that the presence of a fire may be reported from any of the protected spaces and be automatically registered visibly and audibly in the pilothouse or fire control station. There must also be an audible alarm in the engine room. The Engine Room did not have an audible alarm for the fire detection system. 46 CFR 95.05-1 (a) 46 CFR 76.35.15 (a)

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work. Fire control: general arrangement plans showing for each deck the control stations, the various fire sections enclosed by the fire resisting bulkheads, the arrangement of the alarm and extinguishing systems, ventilation systems, details of the alarm system, and details of the extinguishing system. The approved General Arrangement Plan dated 26MAY2015 only shows a stove top. It does not reflect the Deep Fat Fryer which is on-board. A new ventilation hood was install which services the stove and the Deep Fat Fryer. Submit plan reviews to MSC for approval to address the new equipment on-board, electrical installation, and any needed fire fighting equipment. Provide an updated approved Fire Safety Plan to the CG which addresses the alternation of the galley equipment. 46 CFR 91.55.10(a)

Signaling unit shall be energized from a self contained storage battery. Vessel's day light signal lamp battery is inoperable. 46 CFR 111.75-15(h).

Each cargo vessel must carry an immersion suit approved under 160.171 of an appropriate size. Master's jumbo sized was not Coast Guard approved. 46 CFR 199.273.

Each liferaft must be arranged for float free launching. Vessel's weak link on the stbd side #3 liferaft was broken. 46 CFR 199.130(c)(7).

Each class 1 door on a vessel in ocean service must be designed to withstand a head of water. The starboard aft engineroom watertight door is not watertight along bottom. Repair or replace to the satisfaction of attending marine inspector. 46CRFR170.270(b)

Bilge and ballast piping must be so arranged as to prevent oil or water from the sea or ballast system from passing into machinery spaces. Ballast pipe in port side Z drive alley has a temporary patch. Repair to the satisfaction of attending Marine Inspector. 46CFR56.50-50(h)

Means shall be provided for closing all ventilators. The port forward engine room vent closure was not operable. Repair to the satisfaction of attending Marine Inspector. 46CFR92.15-10

Vsl is broadcasting: Greater/smaller than expected vessel length.

Gangway Load Test post visual inspection, found multiple cracks in the structural welds. Repair or replace Gangway and conduct a load test to the satisfaction of the attending Marine Inspector.

Port chain locker was found to have several clad weld repairs and severe pitting throughout an insert on the side shell. The insert is approximately 5' x 12' spanning from aft of frame 2 to aft of frame 6. Crop and renew this insert to also include severe pitting found just aft of the existing weld seam on this insert, during the next scheduled dry dock availability to the satisfaction of the CGMI

Vessel has reported a hole in the anchor chain locker, accompanied by seawater ingress. Contact CG MI to inspect damage extent. Submit a repair proposal for approval & completion to the satisfaction of the attending CG MI.

The air stop solenoid on the #1 SSDG does not actuate when overspeed condition sensed.

The number 1 ssdg circuit breaker does not open when the "open breaker" switch pushbutton on the swbd is pressed.
