Electrical cable penetrating an enclosure is to be secured against strain. wiring in seat boxes are not secured against strain at the enclosures. Properly secure cable at the enclosures. 46 CFR 183.01-15(a)

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part and, when applicable, the duties listed in the station bill required by § 185.514 of this part. O/o is to ensure drills are conducted at a minimum of three month intervals and that adequate details of the trainings are recorded.

Each EPIRB, other than an EPIRB in an inflatable life raft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. O/o was unable to present logs proving the tests were being conducted or recorded. O/o is to start conducting and recording tests as required.

47CFR 80.903: Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Communications Act Radiotelephony Certificates may be obtained from the Commission's National Call Center—(888) 225–5322—or from its forms contractor. FCC Radio Telephony Certificate was expired.

46CFR 185.304(a)(8): The movement of vessel shall be under the direction and control of the master or a licensed mate at all times. The master shall operate the vessel keeping the safety of the passengers and crew foremost in mind by directing the vessel in order to prevent a casualty. Special attention should be paid to: Magnetic variation and deviation errors of the compass. Magnetic compass deviation chart was last updated in 1991. Observed physical and digital magnetic compass displaying different directions with a ten degree deviation. Deviation table is no longer valid. Provide updated deviation table.

46CFR 183.430: Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. VSL O/O was unable to locate two portable lights to be utilized near operating station and machinery space.

46CFR 179.360(b)(2): A hatch cover must: Be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Aft. Freeman Hatch was not secured to the deck with any means.

The required ring buoy water light was inoperable, not meeting the requirements of subchapter Q. Provide an approved floating water light. 46CFR180.70(d)

The required lifeline attached to the ring buoy must be dark in color and have a breaking strength of 1,124 pounds. Replace with a line meeting the requirements. 46CFR180.70(c)

Vessel over 26' must have an oil pollution placard. Provide a placard and mount in a location to be read by crew and passengers. 33CFR151.59(a)

Fuel tank shut off valve markings on the port and starboard fuel tanks were broken up and difficult to read. Replace with appropriate sized markings. 46CFR185.608

Provide a company corrective action plan to ensure compliance with records and logs. To-date, drill, training, and emergency equipment maintenance logs were not on board company vessels at the time of inspection.

Complete DAPI audit conducted by USCG

Repair worn gel coat at bow near waterline and stern in way of port side shell approx 2 feet above waterline. Repairs to be completed by 10May 2011, or if not completed by then, prior to carrying passengers. Repairs to be completed to the satisfaction of the attending marine inspector. Ensure initial inspection happens after sanding but before fiberglass is applied.

Perform vessel's emergency drills with season employees (new hires) NLT June 30 2005. Fire & abandon ship drills including general safety briefing by the crew.

REPAIR AFT RAILING. PASSENGERS RESTRICTED FROM AFTER DECK AREA WHILE UNDERWAY UNTIL RAILING IS REPAIRED.

ATTACH CHAIN OR KEEPER DEVICE FOR LAZARETTE HATCH OPENING.

PROVIDE ADEQUATE HOSE FOR HAND FIRE PUMP TO FIGHT FIRE ANYWHERE ON VESSEL.

PROVIDE DOCUMENTATION OF DRUG TESTING PROGRAM ONBOARD VESSEL.

PROVIDE 1" LETTER SIGNS MARKING EACH EMERGENCY FUEL CUT-OFF VALVE INDICATING OPEN AND CLOSED POSITIONS.

REPLACE WEAK LINKS ON BOTH BUOYANCY APPARATUS PAINTERS.

PROPERLY STOW ALL LIFE JACKETS SO THAT NUMBERS MARKED ON SEAT MATCH ACTUAL STOWAGE.

STENCIL THREE LIFE JACKETS WITH VESSEL'S NAME IN ACCORDANCE WITH 46 CFR 185.30-5(C).

PERMANENTLY REPAIR THE LEAKING HYDRAULIC RAM FOR THE VESSEL STEERING SYSTEM.

DEMONSTRATE PROPER OPERATIONS OF THE MAIN ENGINE PILOT HOUSE CONTROLS.

PROVIDE SIX HAND HELD ORANGE SMOKE FLARES

REPLACE THE WATER LIGHT BATTERIES FOR THE RING LIFE BUOYS AND THE AND THE BUOYANT APPARATUS. FOUR TOTAL.

PATCH AND PROPERLY STENCIL THE BUOYANT APPARATUS.

PROVIDE PROOF OF VALID FCC CERTIFICATE TO THIS OFFICE, TO THE SATISFACTION OF THE COGNIZANT OCMI.
