All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel reported 03 inoperable main engines after striking an unknown object in the ship channel. Repair or replace prior to the carriage of passengers/cargo. 46 CFR 182.100

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A space with a through-hull fitting below the deepest load waterline, such as a lazarette, A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. Vessel's high level switch raised to a level that allows a significant amount of water to gather in every bilge space without alarm at the operating station. Lower bilge high level switch to the satisfaction of the attending USCG Marine Inspector (MI). 46 CFR 182.530

The bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. Vessel does not have any strainers on bilge suction lines. Install strainers to the satisfaction of USCG MI. 46 CFR 182.25-5 (b)

In addition to meeting § 180.130, each life float and buoyant apparatus must be stowed as required under this section, the float-free link required by § 180.130(a)(1) must be:(1) Certified to meet § 160.073 in subchapter Q of this chapter, or other standard specified by the Commandant; Vessel's float-free link missing identification tag certifying that it meets required standard. Replace float-free link to the satisfaction of the attending USCG MI.

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Vessel's floating waterlight had only 1 of 3 clips on the top of the light that were operable. Replace floating waterlight to the satisfaction of the attending USCG MI.

The alcohol testing of each individual must be conducted within 2 hours of when the SMI occurred, unless precluded by safety concerns directly related to the incident. Vessel did not have any way to conduct alcohol testing available. Order alcohol testing equipment for the vessel to the satisfaction of the attending MI.

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions. After completing sea trials and returning to the mooring area, vessel lost power. The vessel was on the #1 generator and then switched to the #2 generator to regain power to necessary services. Identify cause of power failure and repair or replace to the satisfaction of the attending Marine Inspector. **Vessel is restricted as follows: Prior to carrying Passengers and conducting commercial operations. - 46 CFR 183.200(a)

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Vessel's overboard sewage discharge valve is not fully secured nor fully closed. Sewage was slowly discharging on the side shell/skin of the vessel. The means of securing the valve is via flexible wire which still allowed the valve handle to turn. Repair or replace to the satisfaction of the attending USCG Marine Inspector 33 CFR 159.7(c)

No major repairs or altercations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge and approval of the Officer in Charge, Marine Inspection. Vessel was repairing the motor on the main diesel engine without making any notifications to the Coast Guard on the repair. Vessel will also need to conduct a sea trial. Repair to the satisfaction of the attending Coast Guard Marine Inspector. 46 CFR 176.20-1

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following test and inspections of the hull structure and its appurtenances: Examine all accessible parts of the exterior and interior of the hull. Vessel had multiple cracked welds on both the port and starboard railings around the parameter of the vessel and was missing railing on port and starboard stairs. Repair to the satisfaction of the attending Coast Guard Marine Inspector. 46 CFR 176.25-5(a)(1)

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Port and Starboard Engine room ventilation covers do not properly seal when installed. Correct ventilation covers to fit snugly and properly seal engine room to the satisfaction of the attending Marine Inspector. 46 CFR 176.25-50(a)Repair at Drydock

At each inspection for certification and reinspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. #4 MDE has accumulation of fuel oil in the flame impingement bowl. Remove fuel oil and repair leak causing fuel oil to accumulate to the satisfaction of the attending Marine Inspector. 46 CFR 176.25-50(b)

A vessel on an oceans route must carry six hand red flare distress signals and six hand orange smoke distress signals. Substitutions are allowed per 46 CFR 180.68(c). The 12 combination flares carried on board were 5 months expired. Replace to the satisfaction of the attending Marine Inspector.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Vessel’s First Aid kit was incomplete and contained expired products. Replace to the satisfaction of the attending Marine Inspector.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and General description of the drill scenario and training topics. Vessel did not have a record of Abandon Ship and man overboard drills and training in any vessel log.

Firefighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and General description of the drill scenario and training topics. Vessel did not have record of Firefighting drills and training in any vessel log.

The master of the vessel shall ensure that: The EPIRB required by 180.40-1 of this subchapter is tested monthly. The vessel has no recent records of EPIRB testing.

At each inspection for certification and reinspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Excessive oil in engine room, multiple leaks from main engines and standing oil in bilges. Clean oil from bilges and machinery and repair leaks to the satisfaction of the attending Marine Inspector. 46 CFR 176.25-50(b)

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following test and inspections of the hull structure and its appurtenances: Examine all accessible parts of the exterior and interior of the hull. Starboard midship railing has a cracked weld on the top and a broken weld on the course. Repair to the satisfaction of the attending Marine Inspector. 46 CFR 176.25-5(a)(1) Repair at drydock.

The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessels hailing port on the stern board does not match the hailing port listed on the Certificate of Documentation. Correct the hailing port to read “Corpus Christi, TX” to the satisfaction of the attending Marine Inspector.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Alcohol testing strips were expired, replace to the satisfaction of the attending Marine Inspector.

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Both watertight doors leading into the engine room have compromised seals. Replace seals to the satisfaction of the attending Marine Inspector. 46 CFR 176.25-50(a) Repair at Drydock

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel had 12 life jackets that were not marked with the vessels name. Correct to the satisfaction of the attending Marine Inspector.

Electrical enclosures shall be constructed so uninsulated parts cannot be inadvertently touched. The electrical panel mounted on the Starboard side forward bulkhead of the engine room was open and did not have the front panel attached. The electrical panel in the passenger accommodation space was missing blank fillers that would allow accidental contact to live electrical circuits. Replace the front panel and fill in the blank spaces of the electrical enclosures to the satisfaction of the attending Marine Inspector. 46 CFR 183.10-10(a)

All wiring and cables shall be suitable for marine service and for the particular installation used. The main cable coming out of the port side generator has compromised armor and possibly compromised sheathing from rubbing on the generator mount due to insufficient mounting and lack of chaffing gear. Repair or replace cable and mount properly as to not cause future failure due to chaffing on frame to the satisfaction of the attending Marine Inspector. 46 CFR 183.10-20(a)

All wiring and cables shall be suitable for marine service and for the particular installation used. Wiring inside the electrical enclosure mounted on the Starboard side forward bulkhead of the engine room has compromised insulation that could cause an electrical arc between wires. Repair to the satisfaction of the attending Marine Inspector. 46 CFR 183.10-20(a)

Electrical installations on vessels shall be such that services essential for safety will be maintained under various emergency conditions and the safety of passengers, crew and vessel from electrical hazards will be assured. Vessel’s electrical system has many loose and or exposed wires in the engine room, air conditioning space and in the crew’s mess that could cause electrocution or arcing of wires. Crew’s mess also contains a live light fixture with no bulbs or cover. Repair or replace wiring to the satisfaction of the attending Marine Inspector. 46 CFR 111.05-15(a)Repair at Drydock

At each initial and subsequent inspection for certification, the marine inspector shall examine and test items to the extent necessary to determine that they are in proper operating condition and fit for the service in which they are intended. The oil pressure gauges on both generators are illegible and the oil pressure gauge above the starboard generator is leaking oil onto the generator. Repair to the satisfaction of the attending Marine Inspector. 46 CFR 176.25-10(a)

At all inspections special attention shall be paid to insure that no fire hazards exist and that guards or protective devices are provided in all hazardous places. Lagging on all four main engines as well as the generators is severely degraded, missing or separated. Repair or replace to the satisfaction of the attending Marine Inspector. 46 CFR 176.25-10(b)Repair at Drydock

A cooking appliance must be installed to prevent movement in heavy seas. Sea rails, with suitable barriers to prevent accidental movement of cooking pots, must be installed on a cooking range. Sea rails on stove were partially installed and microwave was not secure as to prevent movement in heavy seas. Install sea rails on stove and secure microwave to the satisfaction of the attending Marine Inspector.

Each hatch exposed to the weather must be watertight, except that the following hatches may be weathertight: Each hatch on a watertight trunk that extends at least 12” above the weather deck. Weathertight hatch on stern of accommodation has degraded seals and has a notch cut into the bottom knife edge to allow an extension cord to transit rendering the hatch non-weathertight. Repair to the satisfaction of the attending Marine Inspector.Repair at Drydock

At each initial and subsequent inspection for certification, the marine inspector shall examine and test items to the extent necessary to determine that they are in proper operating condition and fit for the service in which they are intended. Water heater tank reservoir bracket has cracked / broken weld. Repair to the satisfaction of the attending Marine Inspector. 46 CFR 176.25-10(a)Repair at Drydock

Electrical installations on vessels shall be such that services essential for safety will be maintained under various emergency conditions and the safety of passengers, crew and vessel from electrical hazards will be assured. Outlet in lower deck head and two outlets in engine room had missing / loose covers and loose electrical boxes. Repair to the satisfaction of the attending Marine Inspector. 46 CFR 111.05-15(a)Repair at Drydock

Switchboards shall be fitted with driphoods and non-conducting handrails. Vessels main distribution panel switchboard is missing the non-conducting handrail. Replace to the satisfaction of the attending Marine Inspector. 46 CFR 183.10-15(b)

An interconnected detection and alarm system must:(1)Consist of multiple-station smoke detectors listed by an NRTL, or independent laboratory accepted by the Commandant according to 46 CFR subpart 159.010, as meeting UL 217; (2) Be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system; (3) Contain an independent power source; and (4) Alarm on low power. (b) A fire detection and alarm system of an approved type installed in accordance with 46 CFR part 76 would satisfy the requirements of this section. Vessel does not have an interconnected detection and alarm system. Provide plans for approval for the proposed system prior to installation. Installation must be verified onboard the vessel by the attending Marine Inspector.Repair at Drydock

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel currently has a <200nm route and does not have the proper installation of radios for the route. Vessel is missing a MF-DSC or SSB-DSC radio or approved Ship Earth Station (SES) with reserve source of power for 3 hours, and NAVTEX. Install required radio equipment and have station re-certified by FCC inspector to the satisfaction of the attending Marine Inspector. Vessel is restricted to <100nm route until installation is complete.Repair at Drydock

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. The step on the inside of the weathertight door leading into the accommodation space is broken and has a temporary wooden board installed. Repair or replace to the satisfaction of the attending Marine Inspector. 46 CFR 176.25-50(a)

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Alarm panel in operating station does not have labels indicating which alarms are going off. Label panel to indicate which alarms are which to the satisfaction of the attending Marine Inspector. 46 CFR 176.25-50(a)

Each EPIRB must be tested monthly to determine that it is operative and shall be logged or otherwise documented, as applicable. Vessel had no record of EPIRB testing logged. 46 CFR 185.728(c)

A vessel must carry one floating waterlight, unless it is limited to daytime operation. Vessel had inoperable waterlight. 46 CFR 180.70(d)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessel did not have up-to-date charts nor means of correcting charts. 46 CFR 184.420(a)(1)

As appropriate for the intended voyage, all vessels must carry adequate and up-to-date charts. The vessels charts were printed in 2019 and did not contain any chart corrections. Vessel shall ensure charts are kept corrected/reviewed for corrections upon LNM release, for intended area(s) of operation within 14 days.

The bridge-to-bridge radiotelephone station will be inspected on vessels subject to regular inspections pursuant to the requirements of parts II & III of Title III of the Communications Act… Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. The vessels FCC Safety Radio certificate expired in 2021. Vessel shall renew their FCC Safety Radio certificate within 14 days. 47 CFR 80.1005

Except as otherwise provided in this section, all vessels shall be provided with not less than two avenues of escape from all general areas. The second means of escape in the passenger seating area has seized dogs and was unable to be opened by the crew for at least 8 minutes. Lubricate and/or repair to the satisfaction of the attending CG Marine Inspector. 46 CFR 177.15-1(a)

Cables entering boxes or fittings shall conform with the following requirements: openings through which conductors enter shall be adequately closed. The outlet box for the clothes dryer located next to the exhaust has an opening in the side and is covered in lint dust creating a hazardous fire condition. Close and/or repair to the satisfaction of the attending CG Marine Inspector. 46 CFR 183.10-20(f)(1)

At least one ring life buoy must be fitted with a lifeline. Each lifeline on a ring life buoy must: be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. The starboard side bridge wing ring life buoy has deteriotating line. The line must be replaced with a suitable lifeline to the satisfaction of the attending CG Marine Inspector. 46 CFR 180.70(c)(6)

The discharge ends of the diesel fuel tank vent pipes shall be fitted with removable flame screens. The port side diesel fuel tank vent flame screen is severely wasted around the edges where flammable vapors can pass around the remaining flame screen. Repair to the satisfaction of the attending CG Marine Inspector. 46 CFR 182.20-35(d)

Fire hose if 1.5 inches in diameter is used after Janauary 1, 1980 each length of hose must be lined commercial fire hose that conforms to UL Stnadard 19 or Federal Spectification ZZ-H-451E. The port side aft fire hose is worn through the top and secondary layers of the hose. Replace the hose to the satisfaction of the attending CG Marine Inspector. 46 CFR 181.15-10(d)(1)

All supply ducts for ventilation purposes shall be provided with cowls or scoops. The crew could not unsecure the scoop from the bulkhead for 10 minutes in order to demonstrate closure of the portside ventilation duct of the engine room. Modify the storage location of the ventilation scoop to the satisfaciton of the attending CG Marine Inspector. 46 CFR 182.20-45(e)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. The stern light has a blown bulb and did not work. The crew replaced the bulb and the light tested satisfactorily to the attending CG Marine Inspector. 46 CFR 183.420

An adult life jacket must be provided for each person carried on board a vessel. One of the lifejackets had a hole in it exposing the inside buoyant material. The crew destroyed the damaged lifejacket and replaced it with an extra one they had one hand to the satisfaction of the attending CG Marine Inspector. 46 CFR 180.71(a)

An exposed hazard, such as gears or rotating machinery must be properly protected by a cover, guard or rail. The two inboard main engine shafts were not suitably protected or covered. Provide appropriate protection from this rotating machinery. 46 CFR 177.960

An exposed hazard, such as gears or rotating machinery must be properly protected by a cover, guard or rail. The starboard ship service generator windings cover was partially open. Provide adequate cover to protect the crew from this hazard. 46 CFR 177.960

Electrical equipment on a vessel must be installed and maintained to: (b) protect passenger, crew other persons and the vessel from electrical hazards including fire caused by or originating in electrical equipment and electrical shock. A fixed overhead light in the engine room was spliced into an extension cord which was plugged into an outlet. The overhead light shall be properly hard wired into the vessels existing lighting system. 46 CFR 183.200

Openings through which conductors enter shall be adequately closed. Two conductors entering a junction box located forward of the starboard inboard main engine were not suitably closed. Properly close the two entry points with the existing plugs. 46 CFR 183.10-20(f)(1)

Each generator shall be protected with an overcurrent device set at a value not exceeding 15 percent above the full load rating. The starboard ship service generator overcurrent protection could not be suitably tested. Prover proper operation of the the overcurrent protection. 46 CFR 183.10-35(g)

Auxiliary machinery of the internal combustion type shall comply with the provision of this part. 46 CFR 182.20-5(b) requires gauges to indicate water cooling discharge temperature shall be provided. The port generator jacket water temperature gauge was unreadable due to moisture behind the glass. Gauge must be replaced. 46 CFR 182.10-5(a)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. An overhead light protective cover located in a crew accommodation was severely shattered leaving sharp edges and exposed bulbs creating an unsafe condition for those berthing in the space. Provide adequate protection for the light fixture that does not create an unsafe condition. 46 CFR 176.830(a)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Marine inspector found in a crew bunk room that two surge protectors were "daisy chained" creating an unsafe condition. Ensure electrical cords and surge protectors are properly used. 46 CFR 176.830(a)

All wiring and cable shall be suitable for marine service and for the particular installation used. An electrical through hole penetration on the starboard side bridge is open and not closed for a weathertight condition. Repair to the satisfaction of the attending CG Marine Inspector. 46 CFR 183.10.20(a)

Wiring joints and splices shall be mechanically secured and made in junction boxes. Electrical wires were not properly spliced and terminated in the AC Compartment. Repair to the satisfaction of the attending CG Marine Inspector. 46 CFR 183.05-50(b)

Each life jacket carried on a vessel on oceans and coastwise must have a Coast Guard approved life jacket light. 17 of the 60 required life jacket lights are not working or working intermittently. Attach Coast Guard approved lights to the life jackets to the satisfaction of the attending Coast Guard Marine Inspector. Vessel is limited to 43 total persons on board until deficiency is cleared by attending Coast Guard Marine Inspector. 46CFR180.75(a) - 16c

At a minimum, each garbage placard must notify the reader of the operating requirements contained in 33 CFR 151.67 through 151.73 as they apply to the ship. The garbage placards on board the vessel are not-up-to-date to the current regulations. Post current garbage placards in prominent locations and in sufficient number to the satisfaction of the attending Coast Guard Marine Inspector. 33CFR151.59(c) - 16c

The items contained in first-aid kit shall be as listed in Table 160.041-4(b). Vessels first aid kit was not complete several items were expired. Ensure all the items required by the table are onboard and current. 46CFR160.041-4(b) - 16c

Instructions for the use of the contents of the first-aid kit shall be printed in legible type on a durable surface and shall be securely attached to the inside of the cover. Ensure the instructions are installed. 46CFR160.041-4(c) - 16c

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel hydrostatic release was not marked with the expiration date. Install new hydrostatic release marked with expiration date and proof of purchase. 46CFR180.64 - 16c

Portable fire extinguishers shall be installed and located to the satisfaction of the OCMI. Vessel has 2 portable fire extinguishers that were not installed in brackets, outside the engine room and next to washer and dryers. Mount brackets and install fire extinguishers. 46CFR181.30-10(b) - 16c

At each re-inspection and at every other inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Vessels washer, dryer, and stove not securely mounted. Mount appliances securely to deck or bulkhead to prevent shifting during list and heel. 46CFR176.25-50(a) - 16c

At each initial and subsequent inspection for certification, re-inspection and at every other inspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. The lubricating oil from the pneumatic control is being sprayed onto the electrical wires and connections inside the operating station. Reposition the air vent so the oil mist does not cause a fire hazard. 46CFR176.25-50(b) - 16c

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspections. The following unsatisfactory high voltage electrical hazards were observed: lower passenger compartment stbd side 110v outlet box unsecured, crew quarters passageway 110v outlet box cracked, main vessel entrance outside 110v outlet box broken with silicone holding wires, emergency fuel pump shut off switch box broken with exposed wires, galley port breaker box missing mounting hardware and unsecured. Repair electrical components to the satisfaction of Marine Inspector. - 46CFR183.01-15(a) - 16c

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Found the following low voltage electrical hazards: wiring under the pilothouse console has wire nuts, uninsulated connections, bare wires, oil covered wires, connections should be made on terminal strips, dead-end and unused connections. Remove all unused wires, clean off all oil, make all electrical connections on terminal strips, remove all wire nuts to the satisfaction of Marine Inspector. - 46CFR183.01-15(a) - 16c

Acid batteries shall be located in a tray of lead or other suitable material resistant to deteriorating action by electrolyte. Battery under pilot house console is in metal tray with no insulating material between battery and metal. Battery has corroded connections. Insulate battery tray. Make connections on terminal strip. 46CFR183.05-20(d) - 16c

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. Vessel Captain could not provide evidence of an inspection of the radiotelephone installation within the last five years. Provide evidence of a valid radiotelephone installation inspection to the satisfaction of the attending Coast Guard Marine Inspector. 47CFR80.903 - 16c

Vessels operating beyond 100NM from shore must be equipped with a single sideband radiotelephone meeting the requirements of 47 CFR 80.1101(c)(4). Vessels SSB radio could not be proven operational. Prove proper operation of vessels SSB radio prior to transiting beyond 100 NM from shore. 47CFR80.905(a)(3) - 16c

The bilge pipe may be fitted with a screw-down valve without a reach rod on the aft side of the collision bulkhead if it is readily accessible under service conditions. The reach rod for the screw-down valve is not connected, preventing it from being opened or closed. The valve is also not accessible during service conditions. Prove proper operation of the forepeak reach rod valve connection to the satisfaction of the attending Coast Guard Marine Inspector. 46CFR182.25-5(d) - 16c

To maintain a valid COI, you must complete your annual inspection within the period specified in 176.500(b)(1) and your COI must be endorsed. The vessel's annual inspection is past due and the vessel is not ready for CG inspection. Contact cognizant OCMI to schedule inspection when ready.

No life saving maintenance record or logs could be provided. Establish method for documenting life saving equipment maintenance and have available to CG marine inspector upon request. 46 CFR 185.702

Provide appropriate weak links for lifefloats. Currently 2 floats have the wrong size link and the third has not tag to identify it’s size. The 6 person float also needs to be attached to a week link. 46 CFR 180.137(b)(2)

Provide and prove proper operation of a public address system that is useable from the operating station. Vessel currently has an inoperable PA system that only works at the external aft facing operating station. 46 CFR 184.610(b) & (c)

Provide appropriate first aid kit. First aid kit currently on board does not contain required contents. 46 CFR 184.710

Stow distress signals in an appropriate waterproof container and mark distress signals. Current container is a plastic nonsealing container. 46 CFR 180.68(e) & 185.614

Prove proper operation of ship board fixed fire pump pressure to 60 psi. Currently the fire pump only reaches 50 psi. 46 CFR 181.10

Prove air receivers relief valve are of appropriate set point and replace broken gauge on port air receiver. Relief valve set points are not visible and the gauge is broken. 46 CFR 54.15-10

Repair or replace emergency light in engine room. Light facing escape ladder is not operating. 46 CFR 184.30-5

Ensure all empty breaker spaces in panels located in engine room are properly blanked. There are currently open breaker spaces exposing people to live electrical equipment when panel door is open. 46 CFR 183.10-10

Cross connect valve between port and starboard air compressors siezed and unable to close

Both the interior and exterior of the starboard air receiver wascorroded to the point that inspector was unable to visually verify the integrity of the pressure vessel, required hydrostatic testing.

GPS not hooked up to VHF radio rendering DSC inoperable.

Installed bilge manifold valves not clearly marked and no line diagram or instructions for operation posted at the manifold.

Engine bed mount on #3 MDE weld cracked.

Vsl cargo deck was holed. Make permanent repairs to the satisfaction of the attending marine inspector.

Significant crack to fore/aft girder used to mount the port main diesel engine exhaust. Permanent repair to be made.

Multiple soft spots in deck plating observed with a complete penetration on the starboard side amidships. Port aft soft patch for engine replacement/repair has cracked weld on the entire aft side. Port forward soft patch (bolted) leaks water through the hatch, which is rotting bolts keeping the hatch down. Permanent repairs to be made to ensure the watertight integrity of the vessel.

Captain stated that crewmembers onboard were new and were not able to perform a man overboard drill.

Nearly all lights attached to lifejackets flicker and turn off upon movement.

Stern lighting fixture loose and causing the light to be intermittent. Vessel restricted to daytime operations.

Vessel failed to post EAP hotline throughout vessel.

Pressure relief valve markings were indecipherable.

Battery Charger in engineroom was not mounted securely to the vessel and the charger was not of the approved variety.

Sewage piping at the connection to the holding tank was leaking.

Fuel vent on the port side was missing a flame screen.

Port aft main deck rail bracken broken.

03 cracked welds on the cross tie for the ribs in the forepeak.
