3 separate cracks were identified on the starboard rudder post supports. Make satisfactory repairs to the cracks.

Leaks were observed of the bilge/fire main piping near the fire pump. Repair identified leaks.

At each initial and subsequent inspection for certification the Marine Inspection shall exam and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition and fit for the service in which they are intented; (6) fire protective devices. electrical apparatus which operates in conjunction with the fire protective device installed on-board the vessel shall be tested by operation the test shall be applied in the same manner as the manner intended to simulate the actual operation in case of fire. Found a trouble fault at the fire detection panel. Provide tech report attesting to the proper repair of fault in the fire detection panel.

For each 79,500 L (21,000 U.S. gallons) or part thereof of any Class 3 (flammable) or combustible liquid being transported on board a vessel in a portable tank here must be provided at least one B-V semiportable foam (152 L/40 gallon capacity) (see 46 CFR 95.50), dry chemical (45.4 kg (100 pounds) minimum capacity) or equivalent fire extinguisher, or a fire hose fitted with an approved portable mechanical foam nozzle with pick-up tube and two 19 L (5 gallon) cans of foam liquid concentrate. MI witnessed could not identify the lot or expiration numbers on the cans. Cans were also deteriorated and cracked. Replace cans of foam liquid prior to the carriage of any Portable tanks containing flammable or combustible cargo.

Auxiliary machinery shall be suitable in type and design for the purpose intended. MI found loose foundation and mounting hardware on the STBD Generator prime mover. Tighten the hardware to specification to avoid machinery from shifting.

The cable shall be examined as far as practicable without undue disturbance of the cable or electrical apparatus. MI identified Multiple open wires in the engine room. Make proper connections to the satisfaction of the MI.

Vertical exhaust piping must be water jacketed or suitable insulated between the engine manifold and the spark arrester. Found portion of exhaust piping with missing insulation on the PORT engine. Replace missing insulation on exhaust pipping.

Numerous life-jacket lights were not working properly and retro-reflective tape was worn. Replace defective lights and apply new retro-tape.

Rescue platform showed signs of a broken weld at the notch and tread. Permanently repair broken weld and secure chains to ladder.

Vessel's fire pump pressure gauge was not working and unable to verify pressure. Replace gauge Old T

Junction box missing in the engine room overhead and dead in wires were found. Install new junction box in overhead and remove non-working fixtures/dead end wires. Old T 46 CFR 183.10-20 (k)

Numerous buckets of oil and fuel were in the engine room and a large amount of oil residue was noticed in the bilge. General clean-up of engine room and removal of oil/fuel residue from bilge and buckets. Old T 46 CFR 176.25-50 (b)

Safety or relief valve discharges, when permitted to terminate in the machinery space, shall be led below the floorplates or to a remote position to minimize the hazardous effect of the escaping steam. Marine Inspector (MI) observed the hot water heater relief valve not piped in to bilge. Install piping to bilge to protect crew and equipment. Cite: 46 CFR 56.50-25 (c )

Vent pipes for diesel fuel tanks. (d) The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. The Marine Inspector (MI) observed there were not flame screen on two (2) fuel oil vents. Install flame screens. Cite: 46 CFR 182.20-35 (d) [Old T]

Penetrations and openings in watertight bulkheads in a vessel less than 100 gross tons must have a means to make them watertight. The MI observed the cable penetrating the bulkhead from the steering gear room to the machinery space is not of a watertight design. Install proper watertight design. Cite: 46 CFR 171.114 (a)(2) [Old T]

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other then small buoyancy compartments, under all practical conditions. The MI observed the bilge suction piping in the steering gear room was not secured from free movement and the strainer positioned such that it is able to chafe the hull bottom plating. Secure pipes and strainer from movement and chafing. Cite: 46 CFR 182.25-1 [Old T]

Exhaust on air cooled engines need not be water cooled but suitable insulation may be required. MI observed the lagging on portside generator needs repairing. Cite: 46 CFR 182.20-15 [Old T]

Machinery shall be suitable in type and design for the purpose intended. MI observed oil an leak from portside generator. Repair leak. Cite: 46 CFR 182.10-1 [Old T]

All electrical installations shall be to the satisfaction of the Officer in Charge of Marine Inspection. MI observed uninsulated wires, dead-end wires and wires in the bilge on main engines. Replace/repair wiring on main engines to ensure safe installation. Cite: 46 CFR 183.01-15 [Old T]

Machinery shall be suitable in type and design for the purpose intended. The Marine Inspector observed a hydraulic leak in steering at main operating station. Cite: 46 CFR 182.10-1 [Old T]

All electrical installations shall be to the satisfaction of the Officer in Charge of Marine Inspection. MI observed electrical panel in machinery space has mislabeled breaker. Cite: 46 CFR 183.01-15 [Old T]

Each paddle must be of at least 1,220 millimeters (4 feet) in length, lashed to the life float to which it belongs and buoyant. MI Observed the paddles were not lashed. Secure paddles to the life floats. Cite: 46 CFR 180.175 (f)(2)

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] observed the absence of markings for Emergency Egress leading from the galley and birthing spaces. Label emergency egresses. Cite: 46 CFR 185.606

All piping, valves, and fittings shall be in accordance with good marine practice and suitable for the purpose intended. The MI observed there is a temporary repair (soft patch) on the fire main piping located at the fire pump. Repair/Replace as original. Cite: 46 CFR 181.15-15 (c ) [Old T]

All electrical installations shall be to the satisfaction of the Officer in Charge of Marine Inspection. MI observed the washer and dryer need to be secured to prevent movement. Cite: 46 CFR 183.01-15 [Old T]

It shall be the duty of the master of any vessel subject to the regulations of this subchapter to prepare and post emergency checkoff lists. The MI observed the emergency Instructions need to be updated to include procedures for the deploying of the ladder/rescue platform combination by one person (minimum manning if master is at operating station). Update Emergency Instructions with current applicable procedures. Cite: 46 CFR 185.25-1 (a)

The light must be a floating water light approved under approval series 161.010 or other standard specified by the Commandant. MI observed the Life Float Battery was expired. Crew replaced battery. Cite: 46 CFR 180.175 (f)(4)

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificate will be issued for various periods of time based upon the endorsements the mariner holds. Master's Medical Certificate expired. Owner Rep. and Master are aware the Mariner will not be able to work in a position requiring a valid MMC/Medical certificate until mariner provides a valid medical certificate issued by the NMC. Manhole hatch cover on the aft starboard side was not watertight. Complete repairs to satisfaction of CG Marine Inspector.

Forward watertight door gasket failed chalk test. Conduct adjustments and or repairs in order to maintain watertight integrity.

Discovered multiple patches on the a/c condenser water cooling line. Complete permanent repairs to the condenser. Repair to the satisfaction of CG Marine Inspector.

Exhaust water flow sensor on port MDE was gagged with paper towel. Upon further inspection it was found to be broken. Repair or replace sensor to bring back system to normal operation. Repair to satisfaction of CG Marine Inspector.

Sea strainer caps were leaking on Main Diesel Engines. Complete repairs to prevent water accumulation on the bilge.

Water heater relief valve was plugged. Unplug relief valve.

Discovered an oil leak on port SSDG exhaust. Conduct troubleshooting and repairs on the SSDG.

Exhaust lagging was loose, missing, or not secure on all engines and evidence of leaks were present. Correct all leaks found and install lagging properly to cover all hot surfaces on the system.

Complete pick up welds on the areas marked by the Marine Inspector by the MDE exhaust outlet on the port side.

Float light on one of the Lifefloat was not operating properly. Conduct troubleshooting and repairs or replacement in order to bring back to normal operation.

Found 02 life rings with no vessel markings on Port and Stbd superstructure. Mark life rings with vessel's name to satisfaction of CG Marine Inspector.

U.S. Coast Pilot & CG Light List were old (2016) and need to be updated.

Stacked lifefloats were missing spacers, which need to be installed.

Forward accomodation space watertight escape hatch had an inoperable dog, which needs to be repaired.

Exhaust piping non-metallic joint is damaged and worn, causing the water to leak in the machinery space.

Remotely operated STBD side emergency fuel shutoff did not work and needs to be repaired.

New emergency lighting fixtures need to be marked with a red letter "E". That same marking must be removed from the old e-light fixtures, which are no longer used on the emergency lighing system.

Five old Kapok style lifejackets were flat and not serviceable.

During testing of fire detection panel, observed text display and panel were not operating properly. Require vsl troubleshoot and conduct servicing as required by manufacturer, as last servicing date was in 2011.

During testing of outer stbd engine, observed that ahead and astern commands were not working. Vsl plans to replace throttle control valve (101 valve). Replace valve and test to the satisfaction of attenging CG inspector.

Port and stbd air receivers have improper gauges. Also, the crossover valve for the port and stbd air receivers is leaking air, so MI was unable to satisfactorily test the relief valve on the port vessel. Replace gauges and retest to the satisfaction of attending CG Inspector.

Gasket for watertight door from galley to engine room has gap on top. Also, dogging wedges for door has cuts and is deformed. Replace gasket, adjust dogs, and repair dogging wedges to satisfaction of attending CG inspector.

Breakers in distribution panel on bridge are taped over. Replace tape with proper blanks to satisfaction of attending CG Inspector.

Improper size weak link installed on port side life floats. Vsl replaced weak link with correct size.

Clean oil clogged engine room exhaust blower motors.

Repair air leak on port air compressor unloader valve.

Repair #1 Main Engine exhaust leak IVO of upper elbow

Provide emergency lighting for forward engineroom water tight door.

Clean oil and oil soaked debris from both port and stbd generator windings.

Replace or complete hydrostatic testing by 3rd party IAW 46 CFR 61.10-5(b)(3) to 1.5 MAWP on both air receivers. Provide report to attending MI.

Provide visual and audible high bilge alarms for each space as listed in 46 CFR 182.530. Vessel is equipped with a common alarm that does not identify specific space affected.

Drill stop, gouge and weld fractures in welds, longitudinal frame and overhead deck identified by Marine Inspector in lazarette.

Crop and renew damaged area identified by Marine Inspector in overhead deck plating in lazarette.

The pipe hanger for the #4 MDE exhaust is severely corroded needs to be replaced.

Replace all hoses on both Generators with J1942 approved hoses.

Replace all L/O hoses on all 4 MDEs with J1942 approved hoses.

Make proper adjustment to engine room watertight door.

Re-Stencil Emergency Escape scuttle in Engine Room.

Replace the relief valve for the #1 Air Receiver.

Replace the 7 lifejackets that were deemed to no longer be in serviceable condition. Passenger count is limited to 45.

Replace the retroreflective material on 2 lifejackets. Passenger count is limited to 45.

The line on all four waterlights needs to be 18' in length.

The line for the port lifefloats is arranged improperly and is tanlged preventing the lifefloats from meeting the float-free arrangement.

Replace all the lifefloat paddles that were deemed to be in poor condition.

Place appropriate spacers between lifefloats to enable float free arrangement.

Prove proper operation of NAVTEX receiver.

Replace port SSDG jacket water expansion joint.

Provide a copy of the company drug policy onboard the vessel and post the EAP hotline.

Renew or repair 03 of 04 lifefloats. Passengers reduced to 11 and total persons to 15 until all life-saving 835s are cleared by marine inspector.

Conduct 06 drills (MOB - FIRE - ABANDON SHIP) and log results. Provide log entry to marine inspector as proof that drills had been conducted. Entry should be signed by [name withheld], Jr. LIC # 1160936.
