Piping conveying vapors, gases or liquids whose temperature exceeds 150 deg. Shall be suitably insulated to preclude injury. Exhaust piping on the upper and lower jet pumps missing insulation.

Each generator arranged for parallel operation must have reverse power relay. No procedures available for testing reverse power.

An inspection shall be made whenever any repairs are made. Pipe between the OWS and the overboard discharge wasted, and OWS pump housing leaking during operation. Make repairs to the pipe and the pump to the satisfaction of attending CG marine inspector.

The Garbage Record Book along with receipts obtained from reception facilities shall be kept on board the ship and to be readily available for inspection at all times. This document shall be preserved for a period of at least two years from the date of the last entry made in it. No receipts from reception facilities were kept on board the ship for the past 2 years. Company created receipt log procedure and will immediately implement.

If installed, fire detection systems must meet the requirements of 46 CFR part 76. The fire detection and alarm system must be capable of immediate operation at all times that the vessel is in service. Vessel was missing smoke detectors in the accommodation space. Replace the missing smoke detectors. 46 CFR 95.05-1 46 CFR 76.27-5

The inspection for certification shall be such as to ensure that the vessel, as regards the structure is in satisfactory condition and fit for the service for which it is intended. Vessel has a potential leak in the helium tank storage room. Vessel to pump the area dry and identify the source of the water intrusion. If necessary, vessel may proceed directly to drydock to conduct repairs. 46 CFR 91.25-10

Any discharge of oil or oily mixtures into the sea from a ship other than oil tanker or from machinery space bilges of an oil tanker is prohibited except: 4) ship has in operation oily-water separating equipment, a bilge monitor, bilge alarm, or combination thereof as required by part 115 subpart b of this chapter and 5) oily-water separating equipment is equipped with a 15 ppm bilge alarm; for US inspected ships. Vessel had oily-water in bilge area with an electric bilge pump with float switch that is directly connected to an overboard discharge. Plastic pipe installations must be in accordance with IMO Resolution A.753(18) and IMO Resolution MSC.313(88) and plastic pipe and associated fittings must be approved to approval series 164.141 as well as piping in accommodation, service and control spaces must be approved for use in those spaces. Vessel had plastic pipe connected to the aforementioned bilge pump which did not have a clear USCG approval. Remove unapproved pipe and replace with an appropriate and approved type or provide evidence of installed pipe’s approval. 33 CFR 151.10 46 CFR 56.60-25

Bulkheads of galleys, paint and lamp lockers, and emergency generator rooms shall be of “A” Class construction. Additionally, the boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces, galleys, main pantries and storerooms, other than small service lockers, shall be of “A” Class construction. Vessel had numerous locations where structural fire protection separating spaces was damaged or otherwise compromised. Vessel to conduct thorough repairs and replacement of all impacted structural fire protection. 46 CFR 92.07-10

The inspection for certification shall be such as to insure that the vessel’s main and auxiliary machinery, and other equipment, is in satisfactory condition and fit for the service for which it is intended. Suitable hand covers, guards, or rails must be installed on each exposed and dangerous place, such as gears of rotating machinery, and hot surfaces. Starboard exhaust piping leading to the stack was missing appropriate lagging to prevent accidental contact by crew with a hot surface. 46 CFR 91.25-10

Each crew berthing area and communal head shall be equipped with information on the procedures and resources to report crimes, including sexual assault and sexual harassment. Reporting procedures and resources for sexual assault and sexual harassment were not posted in all required spaces. 46 USC 11101(5)(B)

Control system for auxiliary machinery shall meet the requirements for fluid power transmission and control systems. Hydraulic fluid was leaking from the bow thruster controls. Vessel identified and rectified the problem to the satisfaction of the marine inspector. 46 CFR 58.30-1

The inspection for certification shall be such as to ensure that the vessel’s main and auxiliary machinery, electrical installations, and other equipment, is in satisfactory condition and fit for the service for which it is intended. Upon testing a power failure of steering, neither audible or visual alarms were working. 46 CFR 91.25-10

A vessel of 12 meters or more in length shall be provided with a whistle. The ship’s whistle did not work adequately. The vessel rectified the deficiency to the satisfaction of the marine inspector. 33 CFR 83.33

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. One of the circuit breakers in the breaker panel was missing, and a blank was not installed. The vessel rectified the deficiency to the satisfaction of the marine inspector. 46 CFR 111.01-1

All vessels shall have efficient guard rails or bulwarks on decks and bridges. The handrail was wasted in numerous locations on the crane platform. The handrails were repaired to the satisfaction of the marine inspector. 46 CFR 92.25-5

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. There was wiring under the bridge panel that was exposed, posing a shock hazard. Dead end wires were properly terminated. 46 CFR 111.01-1

Vessels must be equipped with independently driven fire pumps in accordance with Table 95.10-5(a). Vessels fire pump is inoperable. Repair/replace fire pump/motor to satisfaction of USCG. Due to presence of divers in saturation chamber, immediately arrange shore-side fire fighting capabilities to service the vessel. In addition, provide 24 hour fire watch within sat chamber room with communications to notify watch stander on bridge of any emergency in the saturation chamber space.

All vessels except unmanned barges shall be provided with a satisfactory bilge pumping plant. MI observed ruptured pipe on discharge side of the ballast/bilge pump. Affect permanent repairs to piping to satisfaction of USCG.

Fire pumps may be used for other purposes provided at least one of the required pumps is kept available for use on the fire system at all times. MI observed a branch line off bow thruster fire station servicing a heat exchanger. Remove branch line from bow thruster hose station to satisfaction of USCG.

At each inspection for certification and periodic inspection, the inspector shall examine the tank tops and bilges in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. MI observed excessive accumulation of oil in the engine room bilge and underneath the bow thruster. Vessel is to remove all oil accumulation in these bilge areas.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. The vessels bow thruster as originally designed is keel cooled. The vessels crew is routinely having to add jacket water due to loss of water through a suspected leak in the keel cooler. Company is required to submit a repair proposal to USCG outlining a repair plan to repair leak and restore the engines cooling water system along with a timeline of repair. 20JUN2024: Company submitted repair proposal utilizing a temporary patch. Repair proposal is approved. Vessel is required to affect permanent repairs prior to the completion of the next dry dock.

Nothing limits the marine inspector from conducting tests or inspections necessary to be assured of the vessels seaworthiness. Jet pump #1 discharge line has a temporary repair. Repair or replace prior to using the jet pump. 46 CFR 91.27-1(a)(4)

Each hatch exposed to the weather must be watertight. Aft engine room escape hatch does not seal. Hole in outboard dog seat.

Flexible cord must meet requirements in NFPA 70, Section 400. Permanent extension cords were installed in the tender shack (fan), crane (exterior light). Cords removed.

Oceangoing ship of 400 gross tons and above must be fitted with approved 15 ppm oily-water separating equipment. OWS is installed but crew could not successfully test the system. Tech was called and confirmed OWS works, provided training to crew on testing and operation. Updated operating instructions were posted.

Each vessel must have a means of communication between the bridge and the alternative steering station. The bridge sound powered phone was difficult to hear and understand from the engine room; the engine room sound powered phone headset had wires exposed.

Actions and observations shall be entered into the official logbook IAW subchapter "W", Pre-Underway checklist, Drafts and Load Lines, compliance with Stability requirements, E-Lights and Power Systems IAW 97.15-30, fuel oil data, cargo gear inspection, etc. The last entry was made on 16MAY2022; Vessel had not logged weekly lifesaving inspection. 705 - Vessel shall have till next inspection to prove vessel is properly maintaining log book.

Nonmetallic flexable hoses shall not exceed 30 inches. The Jet Pump #1 & #2 and the Port Dive Gen Fuel hoses exceeded 30 inches. 705 - Vessel shall have to next drydock to rectify.

All safety and relief valves for use on pressure vessels shall not exceed the maximum allowable working pressure (MAWP) of the pressure vessel. The pressure vessel's MAWP is 585psi and the relief valve was set to 645psi.

Vessel shall maintain watertight integrity. Engine Room Escape hatch was not able to close without adjustment.

Vessel shall maintain watertight integrity in any sea conditions. The engine room escape hatch did not seal properly with daylight being seen in the engine room with the hatch fully secured.

Emergency Lighting shall be sufficient to allow passengers and crew to find their ways to open decks and to survival crafts. Vessel did not illuminate the corridor to watertight door that leads from the saturation room to the stairs to the main deck.

Vessel shall conduct fire and boat drills at least once in every week. Vessel not currently manned and not conducting drills. Vessel shall prove crew proficiency in drills in presence of marine inspector prior to departure. 46 CFR 97.15-35 (OLD I)

MMCs and TWICs must be produced to verify qualifications and identity when requested by an authorized official. Vessels may not be navigated unless properly manned and it has in its service and onboard the crew complement required by the COI. Vessel not currently manned. Marine inspector shall verify crew credentials and TWICs prior to departure. 46 CFR 10.203(c)&(d) 46 CFR 15.515 & 15.801 46 CFR 97.53-1 (Old I)

Vessel shall have onboard 12 red rocket parachute flares. Observed only 6 of such flares. 46 CFR 94.90-5(a) (OLD I)

Rescue boat wire rope shall be replaced every 4 years OR replaced every 5 years while also being end-for-ended every 2.5 years. Rescue boat wire rope has 2015 date. 46 CFR 199.190(j)

Vessel shall have no accumulation of oil in machinery space bilges which might create a fire hazard. Observed excessive accumulation of oil in machinery space bilges. 46 CFR 91.25-45(a) (OLD I)

Oceangoing ships > 400 gross tons but < 10,000 gross tons shall be fitted with an approved 15 ppm oily water separator. Vessel could not ensure proper operation nor provide approval letter or calibration certificate for OWS onboard. 33 CFR 155.360(a) 33 CFR 155.380

Vessels > 400 gross tons shall maintain onboard an Oil Record Book Part 1 (Machinery Space Operations). ORB could not be found on vessel. 33 CFR 151.25

Vessel shall ensure that the rules for the design, construction, use of equipment, operation, and safety standards for commercial diving operations are maintained. Vessel saturation system is inoperable, and vessel does not have all proper diving equipment on-board to conduct diving operations. Rectify prior to conducting diving operations. 46 CFR 197.200

Vessel crane shall be inspected and tested in accordance with American Petroleum Institute (A.P.I.) standards. Vessel crane cylinders are missing and crane is inoperable. Rectify prior to usage of crane. 46 CFR 91.25-25 (d)(3) (OLD I)

Battery deck boxes exposed to the weather should be watertight to prevent entrance of spray or rain. Battery deck box on O-2 deck was corroded severely near lid seal. Rectify by end of next drydock. 46 CFR 111.15-10(d) (OLD J)

Vessel shall carry up-to-date, charts, notice to mariners, tide tables, and current tables. Vessel did not have and up-to-date chart with chart corrections and had tide tables and current tables from 2019. 46 CFR 97.05-5 (OLD I)

The inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that is in a satisfactory condition. Overhead deck insulation on O-3 deck was falling and hanging from ceiling outside an emergency exit. 46 CFR 91.27-5(a) (OLD I)

Vessel liferafts shall be properly secured to the vessel through its appropriate painter. Vessel liferafts on port side were not secured with painter properly. 46 CFR 199.130(c)(6)

VESSEL IS REQUIRED TO MAINTAIN A CRANE RECORD BOOK IN ACCORDANCE WITH API RP-2D. VESSEL COULD NOT PROVIDE SATISFACTORY RECORDS OF PRE-USE, MONTHLY, QUARTERLY, OR ANNUAL INSPECTIONS, OR RECORDS OF ANY REPAIRS. PROVIDE SATISFACTORY RECORDS TO THE SATISFACTION OF ATTENDING COAST GUARD MARINE INSPECTOR.

THE MASTER OF THE VESSEL IS REQUIRED TO DESIGNATE A CRANE OPERATOR IN WRITING. VESSEL COULD NOT PROVIDE A CRANE OPERATOR DESIGNATION LETTER. PROVIDE A CRANE OPERATOR DESIGNATION LETTER TO THE SATISFACTION OF ATTENDING COAST GUARD MARINE INSPECTOR.

Vessel had a crewmember evacuated for medical reasons due to pre-existing condition, and is now below manning requirements. Proceed directly to EPS dock in Cameron, LA during daylight hours only and do not sail again until fulfilling manning requirements to the satisfaction of a United States Coast Guard marine inspector.
