The vessel's pre-engineered fixed gas fire extinguishing system is required to be capable of manual actuation from outside the engine compartment. During the Annual inspection it was discovered that the “Fireboy” system was installed without a means to manual activate the system. The vessel rep must provide a means to manual activate the pre-engineered fixed gas fire extinguishing system from outside the space.

Installed fire detection system (manufactured by HILLER) must be approved by the Coast Guard and listed in the U.S. Coast Guard Maritime Information eXchange as a Type Approved System or be considered an equivalent by the Marine Safety Center. Fire detection system is required by 46 CFR 181.400(c) and required in the space containing propulsion machinery except when a fixed gas fire extinguishing system is installed that is capable of automatically discharging upon heat detection. Replace installed system with an approved heat detection system or install fixed fire extinguishing system that automatically discharges upon heat detection (Fireboy, Seafire, ect). Special note added to vessel, defecency remains outstanding.

Provide manufactures instruction/procedures for testing installed fire detection system and model/serial number (Hiller Systems Inc.). Conduct testing of system as per manufactures instructions in the presence of attending USCG Marine Inspector.

Replace flexible return fuel hose, outside of hose was deteriorating. Flexible hoses over 30" must meet SAE J-1942 with hose end fittings that meet SAE J-1475.

Ventilation closures to E/R shall be able to be secured completely to prevent loss of fire suppressant agent.

Connections to battery terminals shall be of the permanent type, wing nuts are not permitted.

Replace missing screw on main mast boom connection point for boom traveler.

Provide additional deck partners/mast shims or re-seat to properly support fore mast.

Electrical wires shall be supported with metal supports spaced not more than 24" apart. Wires powering cigarette lighters.

Secure diesel fuel leak by way of fuel transfer pump (STBD side). Corrected by tightening connection before completion of inspection.

Provide installation dates for all rigging components listed in Appendix (1) to Enclosure (1) of NVIC 02-16.

Make available a Rigging Preventative Maintenance Program that provides a level of detail commensurate with Appendix (2) to Enclosure (1) of NVIC 02-16 (Rigging Inventory/Preventative Maintenance Inspection Template (Sample)) or propose a mutually acceptable third party to survey the rigging and provide a report with a level of detailed commensurate with either Appendix (3) or (4) to Enclosure (1) of NVIC 02-16 (Third Party Rigging Survey Samples A&B).
