Terminations and joints in all conductors shall be so made as to retain the original electrical, mechanical, flame retarding and where necessary, fire resisting properties of the cable. Vessel's steering gear room is outfitted with a lamp over the steering gear which has been spliced into the existing ships power system. 04 splices observed in total. Resolution: Received class report and photos of corrected deficiency. The lamp with its cable and the splices were completely removed for getting back to her original wiring diagram.

Following installation, the master, owner, operator, agent, or person in charge of the vessel must properly maintain the BWMS in accordance with all manufacturer specifications. Vessel's ballast water management system is inoperable. Vessel shall not discharge ballast water into U.S. Waters.

In a ship in which fuel oil is used, the arrangements for storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. Accumulations of oil were observed in the engine room coming from drain pipes. Oily rags were also observed in way of baffle bulkhead in the vicinity of the OWS. SOLAS 74 (1997) CH II-2/Reg 15.2

Ventilator openings shall be provided with weathertight closing appliances of steel or other equivalent material. The closing appliances shall be permanently attached, where not so provided, they shall be conveniently stowed near the ventilators to which they are to be fitted. The vent neck to the Emergency Generator Space was wasted with two half-inch penetrations making the vent pipe not airtight when the closing device is closed. ICLL 88 Protocol Reg 19(4)
