Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. MI observed 01 missing smoke detector in the engine room and 01 missing detector in port crew berthing room. The vessel is restricted to a limited coastwise route, not exceeding 20 nautical miles from a harbor of safe refuge. This restriction remains in effect until the fire detection system is replaced and tested to the satisfaction of the attending Marine Inspector. 46 CFR 181.405(c)

If 1.5 inch diameter fire hose is used after January 1, 1980, each length of hose must: Be lined commercial fire hose that conforms to Underwriters' Laboratories, Inc., Standard 19. MI observed both fire hoses were not in conformance with UL19 requirements. 46 CFR 181.15-10(d)(1) (OLD T)

A mechanical, manually operated device to assist in launching a survival craft must be provided if: (1) The survival craft weights more than 90.7 kilograms (200 pounds); and (2) The survival craft requires lifting more than 300 vertical millimeters (one vertical foot) to be launched. MI observed one inflatable liferaft and one IBA located on the exterior of the second deck that weighed more than 200 pounds and required lifting in excess of one vertical foot for launching. Vessel shall submit plans and provide an arrangement satisfactory to the Coast Guard for mechanically assisted launching of the survival craft. 46 CFR 180.130(c)

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. MI observed 02 emergency lights in the engine room and 01 emergency light in the saloon did not automatically actuate during testing.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all crew accommodation spaces located below the main deck. No emergency lighting was observed in engine room. There was a red frame light globe between the main distribution panel and engine room emergency egress but did not energize during loss of power. Remaining below deck, passenger cabin and wheel house emergency lighting illuminated during loss of power test.

A work space must be of adequate construction with suitable equipment to provide for the safe operation of the vessel and the protection and accommodation of the crew in a manner practicable for the service, route, speed, and modes of operation of the vessel. A hatch cover must be attached to the hatch frame by hinges, captive chains, or other devices to prevent its loss. Lazarette hatch chain not secured to vessel.

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Lazarette bilge piping across port engine room bulkhead pulled away from two supports.

On a vessel of at least 26 ft in length, a visual and audible alarm must be provided at the operating station. MI observed no audible bilge alarm indication at the operating station.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel experienced loss of propulsion due to faulty spring on the port gear box. Provide proof of proper operation to the attending USCG marine inspector. 46 CFR 182.200

The frame or support of each semi-portable fire extinguisher permitted by paragraph (a) of this section must be welded or otherwise permanently attached to a bulkhead or deck. Fire extinguisher in the engine room does not have proper bracket. Install appropriate bracket to the satisfaction to the attending USCG marine inspector. 46 CFR 181.500

(a) Each vessel certificated to operate on an oceans route in cold water must either: (1) Be provided with inflatable buoyant apparatus of an aggregate capacity that will accommodate at least 100% of the total number of persons permitted on board. - MI discovered vessel equipped with two 25 man IBAs instead of one 50 man and one 25 man. - Vessels passenger count is reduced to 50 POB until deficiency is rectified.

All unfired pressure vessels must be installed to the satisfaction of the cognizant [name withheld] observed that 1) Compressed air system compressors do not power down when proper operating pressure is achieved. 2) Gauges on air receivers do not provide sufficient pressure range to verify 200psi relief valves. and 3) Air receiver pressure is only being regulated by unverified 200psi relief valves popping. Prove proper operation of the compressed air system prior to operations.

Each vessel that operates on the high seas or beyond 3 mile from the coastline of the great lakes must have on board a FCC type accepted category 1 406 MHz EPIRB. EPIRB battery had expired June 2019. Provide an operational EPIRB. Vessel is restricted in its rout to no more than 3 miles from the coastline.

Fire hose shall be 1 1/2 inches in diameter and 50 ft in length. Vessel port side fire hose had multiple pin hose right after nozzle. Replace port side fire hose.

All vessels must have nav light that are in compliance with the applicable sections of the international and inland nav rules. Vessels not under command lights were inop. Provide operable not under command lights (red over red).

Lazarette bilge suction is inoperable. Prove proper operation of bilge system to the satisfaction of an attending marine inspector prior to movement.

Need to conduct satisfactory man overboard drill prior to movement.

Air receivers located in engine room were not made available for inspection.

EPIRB is not tested monthly and is not logged properly.

There is no fire fighitng dril/training log on board. There is no evidence that fighting fire drills have been sufficiently conducted.

There is no training/drill log for abandon ship or man overboard drills on board. There is no evidence that drills have been conducting sufficiently.

Fluorescent light located in the engine room is missing a cover leaving the bulbs exposed.

There is no non conductive matting in front of a high voltage distribution panel in the engine room.

Electrical panel in the engine room has an opening in the panel for a breaker and is not properly blanked.

Steering pump/components are have a hydraulic fluid leak.

A vessel must be operated IAW applicable laws and regulations and in such a manner as to afford adequate precautions against hazards that might endanger the vessel and persons being transported. Vessel and crew are unprepared and failed annual COI inspection.

Aft ballast tank is being used as a fuel tank. Remove diesel fuel from tank getting underway (i.e. before vessel movement).

Fire extinguishers were found to be beyond their annual maintenance date.

Grind/reweld cracked welds and weld missed welds in steering gear compartement as marked by MI. 20 IWO deck insert from 2012, 01 crack on stern/stbd side at stiffener, 01 crack on port side stiffener just aft of fuel tank. Grind/reweld cracks on deck support at top of tank.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure,and personnel injury. Starboard generator displayed excessive oil leakage as well as oil soaked rags on deck/in bilge below the starboard generator; additionally oil staining was noticeable around the exhaust outboard vent (i.e. along the STBD hull).

The emergency lighting required by paragraph (a) of 46CFR183.432 must automatically actuate upon failure of the main lighting system. The emergency lighting failed to function as required.

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck. Reach-rod was disconnected and inoperable.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire explosion, machinery failure and personnel injury. Hydraulic leak found on hydraulic fluid line extending from the steering pump; area of leak located behind bulkhead near mid-ship.

Each large battery installation must be located in a locker, room or enclosed box solely dedicated to the storage of batteries. Back-up batteries installed on top of the pilot house within a battery box is missing the top (lid) and exposed to the elements particularly rain - lid needs to be replaced/installed.

Water for cooling the exhaust pipe must pass through the entire length of the exhaust pipe. Supply hose for cooling water on the Port side main diesel engine is kinked creating a reduction in water flow within the exhaust pipe.

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. The sea painter shackle was attached to the apparatus cradle rather than to the weak-link. The configuration would have negated the purpose of the weak and eliminated the ability of the survival craft to float free from the vessel.

Vessel limited to 25 passengers until second inflatable life raft is onboard to the satisfaction of attending coast guard marine inspector.

Provide satisfactory means to recover man overboard from water to deck of vessel.

Make repairs to bilge and fire pump systems overboard discharge line.

Provide smoke detectors for all overnight accomodation spaces.

Back gauge and re-weld all cracked welds in lazarette.

Replace grab lines on all life floats.

Provide placarding for all lifejacket storage locations.

Provide covers for all open panels on each breaker panel.

Provide latching mechanism for oven to ensure closure while underway.

Post each page of stability letter at pilot station.

Post certificate of inspection in conspicuous location.

Severe pitting was discovered on the port and starboard stern tubes during inspection. Prove to the attending Marine Inspector that both stern tubes have no more than %25 wastage. If replacement is necessary provide repair plan for approval prior to commencing work.

Renew 03 aluminum flanges for seachest tops (found severely wasted)

Emergency shut off vavle was leaking

A low flow sensor for the wet exhaust on the port and starboard engine needs to be installed with a audible or visual indicator located at the operating station.

Repair back-up electrical power for E/R and machinery space.

Repair fractures at exhaust penetrations at hull for MDE's and Gen.

Remove painter between two lifefloats.

Repair three cracked welds in aft port corner of lazarette.

REMOVE DEADEND WIRING UNDER STEERING STATIONS FORE AND AFT IN WHEELHOUSE.

REPAIR OR REPLACE WATERTIGHT THROUGH DECK FITTING FOR FOREPEAK BILGE VALVE REACH ROD.

REPLACE MISSING WATERTIGHT CLOSURE FOR FUEL COMPARTMENT GOOSE NECK VENT LOCATED ON STARBOARD SIDE OF DECK HOUSE.

REPAIR OR REPLACE WATERTIGHT THROUGH DECK FITTING FOR FUEL CUT-OFF REACH ROD PORT AND STARBOARD SIDES.

REMOVE ELECTRICAL FIXTURES, WIRING, AND MARKINGS RELATED TO DISCONTINUED FUEL TRANSFER PUMP SYSTEM.
