REPAIRS OR ALTERATIONS TO THE HULL, MACHINERY, OR EQUIPMENT AFFECTING THE SAFETY OF THE VESSEL MUST NOT BE MADE WITHOUT THE APPROVAL OF THE COGNIZANT [name withheld] IN AN EMERGENCY. THE VESSEL WAS FOUND TO HAVE DETERIORATED AND ROTTED WOOD IN THE STARBOARD AFT QUARTER AND PORT AFT QUARTER. ADDITIONALLY, DUE TO THE VESSEL REMAINING IN DRYDOCK FOR AN EXTENDED PERIOD, THE HULL PLANKING HAS DRIED AND SEPARATED AT VARIOUS LOCATIONS ON THE UNDERBODY. A REPAIR PROPOSAL WAS NOT SENT TO USCG BEFORE REPAIRS WERE COMMENCED, AND USCG WAS NOT KEPT INFORMED THROUGHOUT THE PROCESS TO ALLOW FOR INSPECTION OF AREAS THAT WOULD LATER BE HIDDEN FROM VIEW. THESE DEFICIENCIES MUST BE REPAIRED BEFORE THE VESSEL IS RETURNED TO SERVICE. A DETAILED REPAIR PLAN SHALL BE SUBMITTED TO THE COGNIZANT OCMI FOR REVIEW AND APPROVAL PRIOR TO COMMENCING ANY REPAIRS. THE REPAIR PROPOSAL SHOULD INCLUDE THE SCOPE OF WORK, REPAIR METHODS, MATERIALS TO BE USED, AND ANY PROCEDURES NECESSARY TO RESTORE THE VESSEL TO AN ACCEPTABLE CONDITION. UPON [name withheld], REPAIRS MAY COMMENCE. UPON COMPLETION OF ALL APPROVED REPAIRS, THE WORK SHALL BE MADE AVAILABLE FOR EXAMINATION BY A U.S. COAST GUARD MARINE INSPECTOR PRIOR TO THE VESSEL BEING REFLOATED.

(a) Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port and Starboard MDE are leaking oil. Port is more excessive than starboard. MDE's need to be serviced and reduce the amount of oil leaking.

(a) All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. Mounting bolts for rudder steering assembly on rudder posts in lazarette has sheered a bolt and remaining bolts (7) and nuts are extremely corroded. Replaced all bolts and nuts in kind.

Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. Exhaust hose in lazarette on port side aft as it leads overboard is deteriorated and needs replaced in kind. Both port and Starboard are missing a second hose clamp on flexible exhaust hose as it leads overboard.

(c) At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Both ring life buoys have a life line attached. One life line needs to be removed from the ring buoy.

(b) At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Oil soaked absorbents under both MDEs in bilge need to be removed. If absorbents become soaked in oil frequently, ensure they are replaced as needed to prevent fires.

(d) Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. A loose terminal for the battery was found in the overhead above port MDE batteries. Tighten connection.

(a) At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Deck light on port side midship, has a broken globe. Replace globe in kind.

Deadend wires in the #2 compartment.

Stbd main engine alternator had a build up of oil and dirt.

Starboard side frames 1-3 and the attached 5 wood planks in lazarette are wasted and require repair.

The #6 bottom frame, on top of the keelson, is wasted

Port stanchion in lazarette that supports the main deck, top of stanchion is wasted and requires repair.

Notify local OCMI once vessel is hauled out of water for winter lay-up, for MI to evaluate conditions of repairs

Contact Sector Buffalo MI upon haul out to arrange inspection of areas identified weeping. Provide written plan to address areas.

Replace raw water pump and prove proper operation of port mde.

Monitor weeping members and keel bolt with permanent repairs to be completed prior to the 2021 operating season.

Provide suitable metal marine type strainer meeting the requirements of the engine manufacture. Fuel strainer must meet the requirements of provided cite.

Void #1; de-water and maintain dry and ventilated. Prior to commercial operations in 2020 replace keel bolt within area grinding and glassing old bolt below surface. CG Marine Inspector to be notified when work is completed to allow out of water inspection.

Provide copy of annual fire extinguishing and alarm system attesting to the satisfactory operational testing of system. Requirement to be corrected within 7 days. Requirement will be a NO SAIL requirement if not corrected by requirement date.

Prove proper operations of engine cooling ball valves. Found handles either difficult to operate or unable to operate with interference of structural members.

Void #2: De-water bilge and maintain dry. Space to be inspected during follow up visit within 14 days.

Continue repairs to void #2 weeping seam ivo stbd chine between knees 3 & 4, PT 7 & 8. Lazarette PT aft transverse deck frame & PT aft knee weeping.

Make repairs to pilot house personnel hazards. Pilothouse removed to transit through canal, mating surface provides hazard - complete reattachment.

Mark quantity of adult life jackets stowed in forward space. Mark all life jackets with vsl name.

Remove dead ended wires ivo pilot house.

Provide covers for shafts - protection from rotating machinery.

Complete electrical bonding wires throughout vessel.

De-water all voids, clean all voids of debris ensuring lumber holes are open/provided to allow drainage to installed bilge pumps.

Provide electrical junction boxes for main deck overhead wires. (String lights installations)

Provide servicing report for fire fighting equipment and ensure appropriate mounts are provided.

Rig ring buoy with light/line.

Rig appropriate life line for upper deck w/sign crew only. Same sign/chain for main deck forward.

Prove proper ops of water pump for sanitary use.

Provide appropriate logs.

Make changes to emergency procedures to reflect actual method of mob recovery.

Provide on board required first-aid kit.

Provide required navigation lights.

Secure Lazarette bilge pump hose.

Post EAP hotline phone number.

Complete worklist issued 14 Apr 2016 (except items 5, 6, 7, 10, which are deferred to 01 Oct 2016).

Lifejacket placard required for jacket style lifejackets.

Replace bilge/fire main flexible hose in engine room (stbd side) with acceptable material.

Replace port engine raw water cooling line.

Install breakglass cover over fixed firefighting system activation point.

Remove hydraulic flexible hose penetrations of watertight bulkhead and install bulkhead fittings or change to metallic piping.

Install check valves in bilge lines connected to manifold.

Remove plastic cap on transducer through-hull fitting and replace with metallic cap

Repair lifefloat in accordance with manufacturer's instructions.

Vessel is restricted to a Lakes, Bays, and Sounds route.

Submit written repair proposal for all outstanding work list items for review and approval by CGMI.

Repair leaking three-inch pipe thru-hull fitting in second void space.

Conduct satisfactory underway check ride prior to carrying passengers for completion of credit dry dock.
