REPAIRS OR ALTERATIONS TO THE HULL, MACHINERY, OR EQUIPMENT AFFECTING THE SAFETY OF THE VESSEL MUST NOT BE MADE WITHOUT THE APPROVAL OF THE COGNIZANT [name withheld] IN AN EMERGENCY. THE VESSEL WAS FOUND TO HAVE DETERIORATED AND ROTTED WOOD IN THE STARBOARD AFT QUARTER AND PORT AFT QUARTER. ADDITIONALLY, DUE TO THE VESSEL REMAINING IN DRYDOCK FOR AN EXTENDED PERIOD, THE HULL PLANKING HAS DRIED AND SEPARATED AT VARIOUS LOCATIONS ON THE UNDERBODY. A REPAIR PROPOSAL WAS NOT SENT TO USCG BEFORE REPAIRS WERE COMMENCED, AND USCG WAS NOT KEPT INFORMED THROUGHOUT THE PROCESS TO ALLOW FOR INSPECTION OF AREAS THAT WOULD LATER BE HIDDEN FROM VIEW. THESE DEFICIENCIES MUST BE REPAIRED BEFORE THE VESSEL IS RETURNED TO SERVICE. A DETAILED REPAIR PLAN SHALL BE SUBMITTED TO THE COGNIZANT OCMI FOR REVIEW AND APPROVAL PRIOR TO COMMENCING ANY REPAIRS. THE REPAIR PROPOSAL SHOULD INCLUDE THE SCOPE OF WORK, REPAIR METHODS, MATERIALS TO BE USED, AND ANY PROCEDURES NECESSARY TO RESTORE THE VESSEL TO AN ACCEPTABLE CONDITION.
