COI expired 04FEB2025. Vessel is not to carry passengers until a satisfactory COI Renewal Inspection is complete.

All alcohol testing devices used to meet the requirements of this part must be currently listed on either the Conforming Products List (CPL) titled “Modal Specifications for Devices To Measure Breath Alcohol” or “Conforming Products List of Screening Devices To Measure Alcohol in Bodily Fluids,” which are published periodically in the Federal Register by National Highway Traffic Safety Administration (NHTSA). Device owner/ operator made available for inspection is not shown on this list. Either provide proof that this product conforms, or provide conforming product. Until vessel has adequate testing devices on-board,vessel must stay within 2 hours of a location to perform the required alcohol tests from the time of occurrence of the SMI.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a dry-dock examination and an internal structural examination at least once every two years. As per her COI, vessel is currently outside compliance and can no longer operate under the conditions of her COI. Vessel must undergo a satisfactory dry-docking in compliance with 46 CFR Sub-chapter T, in the presence of a USCG Marine Inspector, in order to clear this deficiency.

Employers must ensure that they and their crew-members meet the requirements of 46 CFR Part 16. Vessel could not provide proof of crew members enrollment in a drug and alcohol program required by 46 CFR Part 16. Provide proof of crew enrollment in required drug and alcohol program as required by 46 CFR Part 16 to attending Marine Inspector.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Items in kit were expired/ missing. Make kit in compliance with regulations.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot or similar publication; Coast Guard Light List; Tide tables; and Current tables. Vessel did not have electronic versions or hard copies of these publications. If vessel intends to use electronic versions of these publications they must be downloaded for immediate use.

Each ring life buoy on a vessel must be approved in accordance with § 160.050 in sub-chapter Q of 46 CFR, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. 01 life buoy grab-line was wasted. Make grab line satisfactory and to manufacturer specifications.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in §185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessel must update emergency procedures as addressed during vessel's annual inspection to address the particular equipment, arrangement, and operation of the vessel e.g. when to notify for assistance during a fire at sea, and to address generator shutdown for blower in engine room.

A vessel on an oceans route must carry six hand red flare distress signals and six hand orange smoke distress signals. Vessel was missing 2 red distress signals for night. Bring amount of flares to correct amount as required by regulation and to the satisfaction of attending Marine Inspector.

Single sideband/MF radio not provisioned with MMSI and GPS. Provide properly configured radio.

Bilge piping penetration through fuel tank / engine room bulkhead leaking. Provide vapor tight barrier.

Renew retro-reflective tape on 5 child life jackets.

Wiring for aft public address speaker connected with twist type connectors. Provide proper connections.

IBA servicing due in January 2020. Service IBA prior to departure.

Starboard running light not operable.

Provide thirty eight (38) serviceable adult life jackets. Eleven (11) lifejackets condemned during inspection.

The existing mattresses used in the passenger sleeping bunks were not flame retardant. Provide flame retardant treatment to the mattresses or provide new mattress's that meet the flame retardant requirements of new subchapter "T".

The required 2.5 gallon fire buckets were not onboard the vessel. Provide 03, 2.5 gallon fire buckets with lanyards to be kept ready for use aboard the vessel.

There was no public address speakers in the salon or below deck passenger accommodations. Provide public address speakers in the salon and below deck passenger accommodations.

The trash receptacles throughout the inside of the vessel were not constructed from non-combustible material. Provide trash receptacles that are constructed from non-combustible material.

Vessel has 11 bunks total, 02 bunks are designated for the required extra crew when on a voyage of more than 12 hours. Vessel must limit overnight passengers to 09.

Provide complete and current first aid kit.

Vessel unable to dewater lazarette space using installed bilge manifold system. Demonstrate proper operation of bilge system.

Connect GPS to VHF-DSC radio.
