The company should ensure that each ship is: manned with qualified, certificated and medically-fit seafarers in accordance with national and international requirements; and appropriately manned in order to encompass all aspects of maintaining safe operations on board. During annual inspection, it was found that the 1st Assistant Engineer, did not possess the necessary STCW III/2 endorsement, a mandatory minimum requirement for all Chief and 2nd Engineering Officers. Per STCW and the Safe Manning certificate, 2 members were required to possess the III/2 endorsement, but only 1, the Chief Engineer, had the required endorsement. A similar occurrence was documented on the USNS PATHFINDER during a USCG inspection 14 Aug, 2018, in Iceland, where an engineer did not possess any STCW endorsements. The Company shall conduct a root cause analysis, and provide to the RO (ABS) responsible for issuance of the SMC. ABS is to attest, based on the root cause analysis, to the conformance of ISM Code 6.2.1&2.
