Every Oil fuel pipe, which, if damaged, would allow oil to escape from a storage, settling or daily service tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. Remote fuel shutoff for EDG could not be operated during inspection due to missing T-wrench. Make remote fuel shutoff easily accessible for quick operation in emergency.

Information shall be posted in crew berthing areas and crew washing places for sexual assault and sexual harassment prevention policies and resources. Master key control logs shall be utilized for all master keys and duplicate cabin keys to single occupancy cabins. Update company SMS, logs and posted information. See USCG CVC Policy letters 23-04 & 23-06 for additional guidance.

The Company should establish procedures, plans and instructions, including checklists as appropriate for key shipboard operations concerning the safety of the personnel, ship, and protection of the environment. The various tasks should be defined and assigned to qualified personnel. Two ABs in ISF Watchkeeper were not in compliance with work rest hours in March 2022. Each AB did not have comments assigned to the work/rest noncompliance. No training, procedures, plan(s), or instruction(s) identified how to make entries into ISF Watchkeeper to satisfy STCW/CFR requirements or OSHI's SMS. SOLAS IX/ REG 3 ISM Code Part A. 7

A training manual...shall contain instructions and information, in easily understood terms illustrated wherever possible, on the life-saving appliances provided in the ship and on the best methods of survival. The SOLAS training manual illustrated the donning of lifejackets that were different from the lifejackets on board the vessel. SOLAS (2014) III/Reg 35.3

Where davit arms are recovered by power, safety devices shall be fitted which will automatically cut off the power before the davit arms reach the stops in order to avoid overstressing the falls or davits, unless the motor is designed to prevent such overstressing. The company should ensure that each ship is: manned with qualified, certificated and medically-fit seafarers in accordance with national and international requirements; and appropriately manned in order to encompass all aspects of maintaining safe operations on board. During annual inspection, it was found that the 1st Assistant Engineer, did not possess the necessary STCW III/2 endorsement, a mandatory minimum requirement for all Chief and 2nd Engineering Officers. Per STCW and the Safe Manning certificate, 2 members were required to possess the III/2 endorsement, but only 1, the Chief Engineer, had the required endorsement. A similar occurrence was documented on the USNS PATHFINDER during a USCG inspection 14 Aug, 2018, in Iceland, where an engineer did not possess any STCW endorsements. The Company shall conduct a root cause analysis, and provide to the RO (ABS) responsible for issuance of the SMC. ABS is to attest, based on the root cause analysis, to the conformance of ISM Code 6.2.1&2.

Ground detected on the switchboard to the emergency generator. Deficiency must be corrected by next vessel drydock to the satisfaction of Class.

All CO2 cylinder flexible hoses for the fixed suppression systems were found loose.

Replace Wye gate valves in the fire mains branch line due to plastic ball valve and handle fittings. They have to meet 46 CFR 56.15-1 (Pipe joining fitting) and the materials must comply with table 56.60-1(a) and (b). The table does not list any materials other than ferrous and nonferrous materials to be used. Send picture of the installed Wye gate valve.

Fire doors not closing as designed. Maintain fire doors to operate properly. Provide attestation from C/E and Master that doors are working properly.

Clean oil/grease and replace filter in dryer vent opening on main deck starboard side. Provide picture once complete.

At least two burnt out light bulbs in the bridge alarm indicator panel. Provide picture once bulb is replaced.
