No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including in paint lockers, unless the Administration is satisfied that such equipment is appropriate for the space concerned. The vessel’s paint locker has an intrinsic light apparatus installed that is rated for Class 1, Div 2 (Zone 2) locations, and requires one that is rated for Class 1, Div 1 (Zone 0 or 1). SOLAS 74 (2004 cons) Ch. II-1 Reg. 45.10 46 CFR 111.105-43

In all ships general arrangement plans shall be permanently exhibited for the guidance of ship’s officers, showing clearly for each deck the various fire sections enclosed by “A” & “B” class divisions. The vessel’s fire control plan indicates that the foreword bulkhead in the emergency generator fuel tank space is an A class boundary. A area of the boundary/insulation was missing. SOLAS 74 (1997 cons.) Ch. II-2 Reg. 20.1

The owner or operator of an OSV shall make the vessel available for inspection by a Marine Inspector whenever any important repairs or renewals are made. Vessel has reported a hydraulic leak from its swing down thruster. Prior to use of the thruster or DP operations, effect repairs to the satisfaction of the attending/reviewing Marine Inspector.

The vessel had a third party company conduct the fire-fighting servicing on June 17, 2021 and June 21, 2021. While conducting the inspection MI's identified 6 fire extinguishers that were not fully charged. The pull station to the paint locker was not serviced or tested and was incorrectly labeled as a pneumatic station on the servicing report. the report also stated that the manual pull stations were tested as well as the fusible link line in the galley. There is not a fusible link over the deep fat fryer. The weights listed for the fixed fire suppression system are all listed as 101 pounds on the report for the suppression agent. The accuracy of the third-party report is in question by the CG MI's.

Fire systems shall be maintained and ready for immediate use. During inspection, the emergency fire pump would not prime/operate as designed. SOLAS II-2/14.1.

Vessels shall have oil filtering equipment to ensure discharged oily mixtures do not exceed 15 ppm. During inspection, proper operation of OWS could not be verified. Vessel to retain all oily mixtures on board/discharge ashore until successful demonstration of OWS. MARPOL Annex 1/14.6.

Vessels shall have the necessary equipment to complete the Automation Test Procedure. During inspection, vessel did not have the necessary equipment to complete lube oil shutdowns or high jacket water temperature alarms (hand pump, hot pot, etc). 46 CFR 130.480 (c).

The angular position of the rudder should be recognizable on the bridge/steering gear room. During operational test of steering gear, it was noted that the starboard rudder angle indicators on the bridge and in the steering gear room was more than 40 degrees different from actual z drive position. Calibrate/adjust rudder angle indicator. SOLAS II-1/29.11.2

Machinery and associated controls shall be adequate for pupose intended. While attempting to demonstrate reverse power relay trip on #2 SSDG, the governor/frequency control on the switchboard was not responding. Repair/replace switch and demonstrate proper operation. SOLAS II-1/26.1.

Cables and wiring shall be installed and supported in such a manner as to avoid damage and batteries shall be properly secured. During exam, it was noted that the secondary start batteries for the EDG and associated cables were improperly installed/hanging from overhead. Properly support cables and fix battery installation. SOLAS II-1/45.5.1.

Lablel emergency fuel oil shutdown (EDG) in accordance with fire control plan. SOLAS II-2/4.2.2.3

Lost port anchor while at anchor in the Takoradi Anchorage, Ghana.

Appropriately mark release hook for rescue boat in accordance with NVIC 08-04 or replace with CG approved hook.

Provide Coast Guard approval letter for rescue boat release hook. Item reported cleared via E-mail dated 24Dec2008 and shall remain open until verified to be corrected by attending USCG Marine Inspector.

Prove proper operation of port anchor windlass brake.

Report proven repairs to and reliability of intermittent overspeed shutdown on Port Main Diesel Engine. Item reported cleared via E-mail dated 17Dec2008 and shall remain open until verified to be corrected by attending USCG Marine Inspector.
