No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including in paint lockers, unless the Administration is satisfied that such equipment is appropriate for the space concerned. The vessel’s paint locker has an intrinsic light apparatus installed that is rated for Class 1, Div 2 (Zone 2) locations, and requires one that is rated for Class 1, Div 1 (Zone 0 or 1). In all ships general arrangement plans shall be permanently exhibited for the guidance of ship’s officers, showing clearly for each deck the various fire sections enclosed by “A” & “B” class divisions. The vessel’s fire control plan indicates that the foreword bulkhead in the emergency generator fuel tank space is an A class boundary. A area of the boundary/insulation was missing. SOLAS 74 (1997 cons.) Ch. II-2 Reg. 20.1

The owner or operator of an OSV shall make the vessel available for inspection by a Marine Inspector whenever any important repairs or renewals are made. Vessel has reported a hydraulic leak from its swing down thruster. Prior to use of the thruster or DP operations, effect repairs to the satisfaction of the attending/reviewing Marine Inspector.

The vessel had a third party company conduct the fire-fighting servicing on June 17, 2021 and June 21, 2021. While conducting the inspection MI's identified 6 fire extinguishers that were not fully charged. The pull station to the paint locker was not serviced or tested and was incorrectly labeled as a pneumatic station on the servicing report. the report also stated that the manual pull stations were tested as well as the fusible link line in the galley. There is not a fusible link over the deep fat fryer. The weights listed for the fixed fire suppression system are all listed as 101 pounds on the report for the suppression agent. The accuracy of the third-party report is in question by the CG MI's.
