Vessel was found to have a flexible hose supplying Fuel Oil to the cargo pump prime mover in excess of 30 inches. Vessel to reroute fuel oil supply to insure that the flexible hose is not greater than 30 inches.

Every portable fire extinguisher must be supplied with a suitable bracket which will hold he extinguisher securely in its stowage location. The bracket for the portable extinguisher on the stern near the prime mover was found to have wasted away, and no longer be able to support the full weight on the extinguisher. A new bracket needs to be installed for the portable extinguisher to the satisfaction of the attending MI.

When repairs are made, they are required to be done under the direction of the cognizant Officer in Charge, Marine Inspection. Upon inspection, Marine Inspectors discovered several repairs were complete without Coast Guard notification or approval. Areas of concern are as follows: 1) Side shell longitudinal framing tacked, not properly stitched in place. 2) Missing relief cuts on side shell insert plates. 3) Angle iron and channel iron in wing voids not original. Submit repair plan detailing repairs made / to be made. When repairs are made, general rule is to repair as original. Provide surveyor's reports. All repairs and testing of repairs should be made to the satisfaction of the attending marine inspector.
