The operator of a vessel with a capacity of 250 or more barrels of oil, hazardous materials shall provide transfer procedures that meet requirement of this part and part 156 of this chapter. Transfer procedures were could not be provided to the MI. Provide transfer procedures that meet the above prior to transfer of cargo.

The following locations in the weather are class 1, division 1 locations and may have only approved intrinsically safe, explosion proof if the location is within 10 feet of a cargo pipe flange. The prime mover of the vessel has a battery installed for starting. The location is within 10 feet of a cargo pipe flange. Provide objective evidence the battery has been allowed to be installed, or remove the battery prior to using the prime mover for cargo operations.

The means for securing and maintaining weathertightness shall ensure that the tightness can be maintained in any sea condition. 04 void hatch covers were wasted around circumference near the gasket seat. Ensure hatch covers/ gaskets can maintain weathertight integrity.

Cargo tanks shall be constructed and tested as required in accordance with ABS standards. Observed oily residue extending approximately 20'x3' within #4 port void space, with associated petroleum smell. Prove integrity of cargo tank bulkhead structure.

Cargo pump relief valves shall be tested at least once each year to determine that they function satisfactorily at the pressure at which they are set to open. Relief valve test out of date. Test relief valve and provide certification to Marine Inspector prior to cargo discharge operations.

Each hose must be marked with one of the following: For oil products, the words “OIL SERVICE”. New oil hoses have no markings. Mark hoses as required and provide documentation to MI prior to cargo operations.

Nonmetallic flexible hose may be used in lube oil, fuel oil and fluid power systems only where flexibility is required and in lengths not exceeding 30 inches. Vessel was found to have a flexible hose supplying Fuel Oil to the cargo pump prime mover in excess of 30 inches. Vessel to reroute fuel oil supply to insure that the flexible hose is not greater than 30 inches.

Every portable fire extinguisher must be supplied with a suitable bracket which will hold he extinguisher securely in its stowage location. The bracket for the portable extinguisher on the stern near the prime mover was found to have wasted away, and no longer be able to support the full weight on the extinguisher. A new bracket needs to be installed for the portable extinguisher to the satisfaction of the attending MI.

Draft marks must be painted in contrasting color to the hull. Vessel did not have draft marks painted in contrasting color on the stem or stern post.

Each vessel of at least five net tons which engages in coastwise trade must have a Certificate of Documentation. Previous COD expired in April. Barge rep was unable to produce the current COD.

When repairs are made, they are required to be done under the direction of the cognizant Officer in Charge, Marine Inspection. Upon inspection, Marine Inspectors discovered several repairs were complete without Coast Guard notification or approval. Areas of concern are as follows: 1) Side shell longitudinal framing tacked, not properly stitched in place. 2) Missing relief cuts on side shell insert plates. 3) Angle iron and channel iron in wing voids not original. Submit repair plan detailing repairs made / to be made. When repairs are made, general rule is to repair as original. Provide surveyor's reports. All repairs and testing of repairs should be made to the satisfaction of the attending marine inspector.

#3 Starboard void has excess pressure build up causing concern that the cargo space is not divided into tight compartments as required. #3 Void is adjacent to the #2 cargo tank, loading the #2 cargo tank is prohibited until the CG can externally examine the #2 cargo tank by means of entering the void space. 46 CFR 32.60-5

A portable non-intrinsically safe electrically controlled diesel engine pump wwas found in the hazard zone. As per CFR, there shall be no source of spark in a hazard zones. Pump needs to be removed or replaced with approved equipment.

Non SAE J1942 fuel hose was installed on the cargo pump engine. Replace with approved marine type hose, to the satisfaction of the attending marine inspector.

Vessel found to have a large unapproved shed on the tank top. Vessel instructed to remove shed priod operations.

PRIOR TO LOADING CARGO PROVIDE PROOF OF FINANCIAL RESPONSIBILITY I.A.W. 33 CFR PART 130.

MAKE ALL DRAFT MARKS LEGIBLE.

PROVIDE PROOF OF CURRENT HYDROSTATIC TEST OF CARGO PIPING.

PROVIDE PROOF OF PROPER OPERATION OF THE CARGO PIPING RELIEF VALVE.

MAKE PERMANENT REPAIRS TO THE CARGO PUMP ENGINE EMERGENCY SHUTDOWN PRIOR TO NEXT CARGO DISCHARGE.
