Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding it designed limitations. Vessel representative reported port main engine fuel pump failure. Vessel is required to repair/replace fuel pump and demonstrate proper operation of the the port main engine to the satisfaction of Marine Inspector prior to the operations of the vessel with passenger or cargo. Passenger may remain on board while vessel is moored. 46 CFR 182.05-1 (a)

Emergency light in engine room had a broken lamp head and was dangling by the wires. 46 CFR 183.200

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Dead end wires with wire nuts was found next to the over head emergency light in the medical stairwell. 46 CFR 183.340(g)

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. Light in galley in front of reefers was missing the bulbs and cover which exposed the energized component of the fixture. 46 CFR 183.410(a)

The following issues were identified: -Electrical enclosure in crew RM101 not secured properly - Electrical enclosure in overhead of FWD 100's RM101 is missing the cover - Light fixture in room 107 is hanging from overhead with exposed wires. - Port Aft passenger room (Yakobi) vanity light is hanging off the bulkhead with exposed wires - Reefer room overhead light was not properly secured. Was found zip tied to the overhead frame. - Light in above engineering work bench is missing the protective cover. 46 CFR 183.200

All passenger accommodations must be arranged and equipped to provide for the safety of the passengers in consideration of the route, modes of operation, and speed of the vessel. Forward lounge space emergency escape scuttle did not open freely. 46 CFR 177.800

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant OCMI. 46 CFR 185.606

Vessels not equipped with a single source emergency lighting system shall have individual storage battery powered automatically operated lights in strategic locations. Engineering passageway emergency light was not properly secured to the bulkhead.

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Room 109 PA speaker was not properly secured and was found lying on the bed. 46 CFR 184.610(b)

Lighting fixtures shall be constructed in accordance with the requirements of Underwriters' Laboratories, Inc. Standard for Marine Type Electric Lighting Fixtures, Subject 595, or shall bear the proper Underwriters' Laboratories, Inc. "Marine Type" label, or shall be constructed in accordance with the requirements of other recognized testing laboratories. Several main deck exterior lights were missing the protective guard around the globe and were full of water. 46 CFR 183.10-20(I)

At each inspection for certification and at every other vessel inspection galleys may be examined to determine that they are serviceable and sanitary. Galley range hood fan and exhaust vent was found with excessive grease. 46 CFR 176.818

46 CFR 176.810(a)(1)On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. 46 CFR 181.300(b)

Combustibles and other foreign materials are not allowed within ventilation ducts. Galley griddle exhaust ventilation had excessive build up of grease. 46 CFR 176.830

An interconnected detection and alarm system must e installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system. New system was installed and initial testing did not activate all alarms as required. 46 CFR 181.450(2)

A space containing diesel machinery must be fitted with at least two ducts to furnish natural or powered supply and exhaust ventilation. Aft engine room powered ventilation was not functioning properly. 46 CFR 182.465(c)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. ENC charts were out of date. 46 CFR 184.420(a)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. After repairs are complete on the starboard side generator, complete operational test to the satisfaction of a Coast Guard Marine Inspector. 46 CFR 176.840

The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Monthly EPIRB testing logs could not be located. 46 CFR 185.728(c)

A weathertight door must be provided for each opening located in a deck house or companionway. Several companionway sliding doors on the 300 deck cabin level did not close properly and were not weathertight. 46 CFR 179.360(d)

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03–2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Alcohol testing device onboard was not calibrated this year and alcohol swabs were not located onboard. 46 CFR 185.212

A self priming, power driven fire pump must be installed on a vessel of more than 19.8 meters (65 feet) in length, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. Vessels fire pump was not able to achieve 60 psi at the pump outlet. 46 CFR 181.300(b)

HVAC electrical panel in the starboard side bridge crawl space was missing the panel cover. 46 CFR 183.200

Receptacle outlets and attachment plugs for portable lamps, tools, and similar apparatus operating at 100 volts or more, must have a grounding pole and a grounding conductor in the portable cord. Several portable fans and portable heaters that operated in excess of 100 volts that did not have a grounding pole were found throughout the vessel. 46 CFR 183.370(b)

A vessel of more than 19.8 meters (65 feet) in length must have at least one fire axe located in or adjacent to the primary operating station. Vessel had numerous fire axes throughout the vessel but none were located in or adjacent to the primary operating station. 46 CFR 181.35-1 (Old T)

The means provided for pumping bilges shall be tested by operation. All suctionj strainers shall be examined. Forward 100's bilge suction strainer was corroded and clogged. 46 CFR 176.25-1(7) - Old T

Found firedoor in forward 100's did not satisfactorily close.

Found dead end wire above air receiver in engine room. Wire needs to be corrected or removed.

Found grey water tank hatch cover removed. Secure hatch cover.

Found compressor cover contactor switch missing. Replace cover.

Found dead end wire above bow thruster engine in the overhead.

Properly inspect portable fire extinguisher in fwd lounge all the way fwd.

Prove proper operation of machinery shutdowns for fixed CO2 system.

Provide appropriate lanyard for water light for ring buoy, fwd port deck.

Grind out cracked weld on stbd aft deck railing base and re-weld.

Prove proper operation of ventilation shutdown adjacent to Chichagof cabin.

Prove proper operation of emergency lighting in FWD 100s, Engine Room, deck 1 hall and the flickering leds in fwd lounge.

Provide current FCC ships station license.

Provide proof of enrollment and preemployment drug testing for entire crew.

Remove trip hazard on top step of deck 3 fwd stairwell.

Replace deck 2 furnace bottom cover.

Remove or permanently secure blue fan in furnace space.

Correct fwd deck 1 fire doors into lounge from rubbing lighting fixture.

Replace missing structural fire protection in engine room overhead along aft bulkhead, above stbd gen and stbd MDE.

Provide appropriate gasket seal for fwd bow door.

Remove or provide means of keeping deadbolt locks from preventing fire doors from automatically closing.

Properly mount doors on E/R fixed extinguisher CO2 cabinets.

Provide international shore connection iaw. approved fire control plan.

Replace missing cover on 12v battery charger for emergency generator.

Repair or properly terminate dangling wires below 12v battery charger for emergency generator.

Replace missing exhaust pipe lagging on stbd generator.

Replace missing bolts on stbd generator cover.

Prove proper operation of safety relief valve on engine room air receiver.

Make lower 100s available for inspection.

Prove proper operation of automatic shut downs for port side generator and fixed fire extinguishing system.

Replace weather-tight seals on aft door and port side water tight doors. Item to be cleared prior to receiving credit for dry dock inspection.

While the vessel is engaged in towing the vessel must be controlled by a licensed Master or Mate of towing. Vessel has until April 1st 2022 to man vessel with licensed Master and Mate of towing.

Vessel must provide updated stability calculations to include towing to Coast Guard Marine Safety Center to update vessels stability letter by April 1st 2022

Vessel shall provide correct light and day shape configuration for the tow being conducted prior to towing.

Vessel shall start and maintain records for towline and terminal gear used for towing astern or alongside prior to towing

Make vessel ready and available for inspection. Vessel to be put into laid up status until completed.

Emergency escape exit hatch in the main deck lounge overhead does not open freely. Make permanent repairs to ensure the emergency exit opens easily.

Post operating instructions for installed fire fighting system on aft deck emergency generator.

Ensure all hot water heaters are properly installed and secured from rolling by straps or other devices.

Fire extinguishing equipment installed must be maintained in accordance with a recognized industry standard; the engine room siren supply piping is cut rendering the system inoperable.

Display an up-to-date SPV Decal.

Identified water heater located in fwd berthing that was not securely installed. Secure from rolling with straps or other device.

Renew broken electrical outlet cover identified in the bar area.

Provide missing fluoreescent light fixture cover in the fwd sewage space.

Replace starboard main engine coupling between shaft and reduction gear. Vessel is restricted from carrying passengers.
