Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Vessel's waterlight lanyard was less than 3 feet. 46 CFR 180.70(d)(2)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection. The shutoff valve is located approximately 12 inches from the tank, connected via a rubber hose. 46 CFR 182.455(b)(4)

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request; You must keep records of negative and canceled drug test results and alcohol test results with a concentration of less than 0.02 for one year. Operator was unable to show any records of testing done within the past year. 46 CFR 16.260(a) & 49 CFR 40.333(a)(4)

Employers must maintain records of chemical tests, records shall be sufficient to Satisfy the requirements of §§ 16.210(b). No marine employer shall engage or employ any individual to serve as a crew member unless the individual passes a chemical test for dangerous drugs for that employer. Operator was unable to provide pre-employment test records for their employees. 46 CFR 16.260(b)(1) & 46 CFR 16.210(a)

At least one ring life buoy must be fitted with a lifeline that is buoyant, 60ft in length, be non kinking, has a diameter of 5/16th inch, has a breaking strength of 1,124 lbs, and dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. The lifeline attached to the ring life buoy was deteriorating and fraying when it was twisted or handled. 46 CFR 180.70(c)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The vessels port side navigation light housing is cracked. 46 CFR 183.420

An adult life jacket must be provided for each person carried on board a vessel. 02 adult life jackets were unserviceable. Replace unserviceable jackets. Passenger count is reduced to 18 and total persons to 20. - 46CFR180.71(a)

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of 46 CFR 4.06. Vessel owner does not have alcohol test strips available for testing within 2 hours of a serious marine incident. Provide test strips. - 46CFR16.240

The owner or operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crewmember is expected to perform in an emergency including, but not limited to, the emergency instruction listed on the emergency instruction placard required by § 185.510 of this part. Crew training shall be logged for review by the Coast Guard upon request. The training entry shall include the following information: Date of the training and general description of the training topics. Initial training has never been logged. Provide updated training record to USCG. - 46CFR185.420

Employers must maintain records of chemical tests, including pre-employment and random testing, and must make these records available to Coast Guard officials upon request. Owner had no record of pre-employment or random testing. Provide record of testing from APCA to USCG. - 46CFR16.260(b)(1)

A vessel must carry a first aid kit approved in accordance with 160.041 in subchapter Q of this chapter, or other standard specified by the Commandant, or a kit with equivalent contents and instructions. Vessel's first aid kit has expired contents. Replace eye dressing, antiseptic, and 2x burn gel. - 46CFR184.710
