Fired thermal fluid heaters must be designed,constructed, inspected, tested, and stamped in accordance with the applicable provisions in 46 CFR Subchapter F Part 52. Replace the holed flexible line on the the thermal fluid heater prior to conducting cargo operations that require the use of the thermal fluid heater but no later than 14 days which ever is first.

Operational tests and checks of all safety and limit controls, combustion controls, programing controls, and safety relief valves must be conducted by the owner, chief engineer, or person in charge at the inspection for certification, periodic inspection, and when directed by the Officer in Charge, Marine Inspections, to determine that the control components and safety devices are functioning properly and are in satisfactory operating condition. These tests and chescks must be conducted in the presence of a marine inspector and must include the following: proper prepurge, burner ignition sequence checks, operation of the combustion controls, limit controls, fluid fow controls, fluid level controls, high temperature control, proper prostpurge and controls, and verification of the flame guard. Marine inspector found the vessel representative unable to conduct all tests required by OCMI. Vessel has been instructed to consult with third part representative to conduct all testing prior to carriage of cargo.

Piping systems used where the fluid temperature exceeds 150 °F must be suitably insulated as necessary to preclude injury to personnel. Marine inspector found piping from the thermal fluid heater on vessel's bow tank to be missing insulation. Company is to enact repairs prior to heating cargo.

Nonmetallic flexible hose may be used in lube oil, fuel oil and fluid power systems only where flexibility is required and in lengths not exceeding 30 inches. Marine inspector found multiple hoses running to main generator to be over the 30 inch threshold. Company is to configure new hose rigging to meet the requirements of under 30 inches throughout the engineering space.

The respective piping systems installed on ships and barges shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. The mechanical seal on the line connected to the #1 circulating pump for the thermal fluid heater failed and caused heat transfer oil to leak on deck. Make repairs to the satisfaction of the Coast Guard prior to loading cargo. 46 CFR 56.01-1(b)

Piping, including valves, pipe fittings and flanges, conveying vapors, gases or liquids whose temperature exceeds 150 °F., shall be suitably insulated where necessary to preclude injury to personnel. Repair/renew steam pipe lagging in areas noted by attending MIs.

Piping exceeding 150 degrees F shall be suitably insulated to prevent injury. Outfit thermal fluid heater discharge line with insulation to the satisfaction of the attending Coast Guard Marine Inspector.

Each loading arm and transfer pipe system must not leak under 1.5 MAWP test. Provide documentation attesting to a satisfactory test of cargo piping.

Rusted out flame screens on fuel tank.

Guaging window wipers ALL missing with temp repair bolts only no wiper.

The #2 starboard void goose neck is wasted and need of repair in kind.

The stern void 10' x 4'of starboard corner radius, hull plating, and three of the corner gussets are damaged and needs replacement in kind. Fractures are marked by CG.

Insulation was found missing on Generator Exhaust

Repair/replace thermal fluid heater pump. One of the TFH pumps is leaking thermal fluid. A replacement pump has been ordered and should be installed within 2 weeks.

Cargo gauging glass is inconsistent with this type of cargo, Owner is to provide an alternate means of gauging the cargo consistent with type of cargo carried.

BARGE IS NOT TO CARRY CARGO UNTIL REPAIRS ARE COMPLETED. Cargo tank thermal probe tubes are broken off on the cargo tank tops. Barge is allowed to transit to Portsmouth Va. to complete repairs.

Repair thermal fluid heater faulty limit switch and prove proper operation.

Remove all deck insulation, clean and provide thickness readings of main deck, longitudinal's and transverse frames to the attending marine inspector. Areas of wastage to be cropped and renewed in accordance with the attending marine inspector's instructions.

REPLACE FAULTY ALARM ANNUNCIATOR AND INDICATOR LAMP FOR THERMAL FLUID HEATER.

REPLACE TWO B-II PORTABLE FIRE EXTINGUISHERS; ONE (1) IN GENERATOR FLAT AND ONE (1) IN THERMAL FLUID HEATER RM.

PROVIDE MISSING PLUGS FOR LIGHTS IN HEATER ROOM.

LABEL BREAKERS IN ELECTRIC BOX HEATER ROOM.

REPLACE BROKEN HATCH DOG FOR #2 PORT WING VOID MAN HOLE COVER.

PROVIDE TOP FOR BATTERY BOX FOR GENERATOR.

PAINT ALL HULL MARKING.

PROVIDE ON BOARD STABILITY LETTER DATED 10SEP99.
