Provide actual weights from removed windows plexiglass, new plywood and fiberglass, and new forward installed seat benches. Weights shall be submitted to the USCG Marine Safety Center at [email withheld].

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. The portside and forward horizontal lower cross structural beam, and the vertical structural support beam on top of the same cross beam, both were found with wood rot. Submit a repair proposal for approval of USCG, that includes the following: 1. Remove the wood rot until good wood material is found. Once good wood material is found request an attending USCG Marine Inspector to check the extend of the damage. 2. Provide process and list of materials on how the repair will be carried out. 46 CFR 177.100 17C.

A vessel on a limited coastwise route must carry Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter. Two orange hand smoke distress signals were missing. 46 CFR 180.68 16C.

Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. Starboard side plastic ventilation porthole window in the toilet area was found broken and hanging. Until a replacement is found, provide means to close the opening and make it weathertight.

A hatch cover must have securing devices. Portside forward deck hatch cover has one of its two closing devices broken. 46CFR179.360(b)(1). 16 C.

The master of a vessel shall have examined and tested the steering gear. A hydraulic steering fluid was found in a hose fitting close to the steering piston ram, starboard side lazzaratte. 46 CFR 185.320 16C.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First aid kit missing 4m of adhesive elastic bandage.

Each vessel must be equipped with at least two operable portable battery lights. 01 out of 02 portable lights were inoperable.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. 09 Child life jackets required retroreflective tape.

A life preserver must be of such construction, material and workmanship that it can perform its intended function in all weathers and at all temperatures which may be expected in the normal usage of the life preserver. 04 Adult life jackets were found unsatisfactory in their condition due to cracked foam and ripped cloth.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Bilge high level alarm did not have a visual alarm.

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards. Floor in port forward compartment was found to have water damage determined to be non-structural. Provide repair proposal.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Marine inspector found cracks, under-cut,and not fully penetrated welds through out newly fabricated engine sled. Conduct NDT on entire sled welds, repair has needed. Have Certified Welding Inspector (level 2) provide report of repairs to Marine Inspector. 46 CFR 176.840 c 705 (prior to drydock completion)

Life jackets must be stored in convenient places distributed throughout accommodation spaces. Lifejackets are not stored according to the vessel plans, nor distributed throughout the accommodation spaces. 46 CFR 180.78

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. The flame screens or flame arresters must be of such size and design as to prevent reduction in the net cross sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements Found that the flame screens installed for the gasoline was made of 100 x 100 screen and deteriorated state. 46 CFR 182.450 (e)

A hatch exposed to the weather must be weathertight. (a) The hatch located on the starboard by the bow was found not to be weathertight. When the hatch is secured in the closed position daylight passes through the inboard side of the hatch. Additionally the aft dogging hatch is broken, and the center hinge is broken. (b) the plastic window on the port and starboard hatches on top of the passenger a accommodation space is crazed. 46 CFR 179.360 (a)

A hatch cover must:(1) Have securing devices; and (2) Be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Found that both hatch covers did not have securing devices and a captive device to prevent loss. 46 CFR 179.360 (b)

A port light must have an inside, hinged dead cover regardless of whether the port light is or is not capable of being opened. Found the following port light rubber seals were not installed properly preventing the port light from completely closing: a. Port glass bottom compartment, b. Starboard aft toilet, c. Starboard forward toilet. 46 CFR 179.5.350(b)

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Found that the bilge suction piping located in the port glass bottom viewing compartment had a leak in the threaded pipe connection at bottom of the valve body connection to the bilge strainer. 46 CFR 182.500(a)

Cable must be listed by Underwriters Laboratories (UL), as boat cable or UL marine cable. Found the electrical wiring to the starboard navigation side light had been replaced by two single wires, instead of approved electrical cable. The unapproved wiring runs from the forward starboard void, through the collision bulk head, through the main deck and inside the hand rail to the light fixture. In the starboard forward void the unapproved wiring was spliced into the old cable with a crimp connection that were not made inside a junction box. 46 CFR 183.340

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor;(2) A solder lug on each conductor;(3) A splice made with a pressure type connector to a flexible lead or conductor; or(4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. Found exposed terminal blocks or splices in the following compartments that were not in enclosures (i.e. Junctions Boxes): a. Starboard forward void, b. Port forward void, c. Port glass bottom viewing compartment (Light fixture and Fan). 46 CFR 183.340(g)

All cables and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support, except that on vessels of not more than 19.8 meters (65 feet) in length, installations in accordance with paragraph 14.h of ABYC E-8 and paragraph 15.h of ABYC E-9 (both incorporated by reference; see 46 CFR 175.600) are acceptable as meeting the requirements of this section. Found electrical cables did not have support to avoid chafing and other damage in the following compartments: a. Starboard forward void, b. Port forward void, c. Port glass bottom viewing compartment. 46 CFR 183.340(b)

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. Found that the fresh water pump and control mounting to the bulkhead had come loose and was attached to the bulkhead by only two of four screws. 46 CFR 183.200

Cable and wiring not to be installed with sharp bends. In the Port glass bottom viewing compartment, found an electrical cable under the inboard bench that was bunched up and zip tied causing several sharp bends. Shorten the cable to remove all sharp bends. 46 CFR 183.340(a)(5)

Through hull fittings and shutoff valves must be metallic. In the case of nonmetallic hulls, materials that will afford an equal degree of safety and heat resistivity as that afforded by the hull may be approved. Found PVC plastic thought hull fittings in the following compartments: a. Two fitting under ladder between the forward and aft toilets located on the starboard side of the vessel. b. Port forward void in way of the aft bulkhead, outboard side. c. Starboard forward void in way of the aft bulkhead, outboard side. 46 CFR 182.7209(d)(3)

The lights prescribed in these Rules (Subpart C) shall have an intensity as specified in Annex I to these Rules (33 CFR part 84). The port and starboard running lights had UV Damage that decreased their visibility below the minimum range required by 33 CFR 83.22(b)(ii) 46 CFR 182.720(d)(3)

Stern light means a white light placed as nearly as practicable at the stern showing an unbroken light over an arc of the horizon of 135 degrees and so fixed as to show the light 67.5 degrees from right aft on each side of the vessel. Stern light 135 degree arc of visibility is obstructed by water slide and mounted kayak. Vessel rep requested to move the stern light to the radar mounting station. The vessel owner/operate to make an application for a Certificate of Alternative Compliance per 33 CFR 81.5. 46 CFR 83.21(c)

A vessel proceeding under sail when also being propelled by machinery shall exhibit forward, where it can best be seen, a conical shape, apex downward. A vessel of less than 12 meters in length is not required to exhibit this shape, but may do so. There is no conical day shape onboard. 33 CFR 83.25

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. The lifeline was deteriorated by UV damage and was unserviceable.

Each ring life buoy with a floating water light must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the water light around the body of the ring life buoy. The lanyard was deteriorated by UV damage and was unserviceable. 46 CFR 180.70(d)

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers each hose must Be lined commercial fire hose that conforms to UL 19 (incorporated by reference, see 46 CFR 175.600) or hose that is listed and labeled by an independent laboratory recognized by the Commandant as being equivalent in performance. The fire hose had no marking showing approval and under pressure water sprayed from holes near the connection to the fire hydrant. 46 CFR 181.320

Lifejackets must be stored in convenient places distributed throughout accommodation spaces. Lifejacket relocation plan proposed by vessel management was conditionally approved. The following items shall be completed when the vessel goes to dry dock: 1. Move 50 lifejackets to main cabin area as prescribed in the proposal letter. 2. Update vessel plans with revised lifejacket stowage locations. 3. Crew shall demonstrate efficient retrieval of lifejackets stowed in the forward voids in the presence of an MI. 4. Vessel shall have written procedures outlining lifejacket retrieval and responsibilities.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Penetrations were observed open and not watertight in the following spaces: 03 electrical cable penetrations in the starboard forward void, port aft void on forward bulkhead, and starboard aft void gray and black water piping repairs.

A hatch exposed to the weather must be watertight. The starboard forward void space hatch securing device was broken and a portion of the gasket was missing.

A hatch exposed to the weather must be watertight. The gasket is wasted and missing portions on the port and starboard aft void covers.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection. The bilge float switch in the starboard void was not mounted.

On each container for life jackets, the number and identification of items stowed inside, and their sizes, must be marked in clearly legible letters and numbers. Letters and numbers must be at least 2 inches high. The stowage area for the the life jackets on the port and starboard side was not identified.

Electrical equipment used in the following locations must be dripproof: A location normally exposed to splashing, water washdown, or other wet conditions within a galley, or another space with a similar moisture level. The operating station battery compartment door and frame was wasted and needs repair. In addition, the starboard cross beam compartment electrical box was missing the cover and needs replaced.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. The plug on the life buoy was wasted allowing water to saturate the foam.

A portable hand bilge pump must be provided with a suitable suction hose capable of reaching the bilge of each watertight compartment and discharging overboard. No hose was connected to the portable hand pump.

The vessel must provide and maintain a preventative maintenance plan for their rigging.

Conduct a rigging survey and provide a copy to the cognizant OCMI.

Provide an inventory list of all rigging components.

The outer bottom three longitudinal frames in the starboard aft restroom had water damage and exhibited areas of soft wood.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Main sail could not be lowered during operations. OCMI requires inspection and report from vessel owner/operator as to the cause of the sail malfunction.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Drills not conducted, conduct prior to operation or test of propulsion.

The pump outlet must be fitted with a pressure gauge.Replace fire pump gauge.

All hatches and openings in the hull, except loading doors, of a vessel must be kept tightly closed except when being used. Port side weathertight window gasket needs to be replace.

Electrical equipment on a vessel must be installed and maintained to:Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; Electrical fixture expose on port side forward void.

Charts onboard were from 2007. As appropriate for the intended voyage the vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible.

Day light shapes were missing. The required for the route are: three balls, one diamond (or two cones).

An adult life jacket has to be provided for each person carried onboard a vessel. Vessel required to have 10% of life jackets for children. Vessel had 21 adult life and 2 children life jackets in bad shape.

Make available a Rigging Preventative Maintenance Program that provides a level of detail commensurate with Appendix (2) to Enclosure (1) of NVIC 02-16 or propose a mutually acceptable third party to survey the rigging and provide a report with a level of detail commensurate with either Appendix (3)or (4) to Enclosure (1) of NVIC 02-16. During 3rd party survey a CG marine inspector shall be present. 46 CFR 176.802 (a)(3)

Stress damaged rudder post

Only 51 life jackets were found in serviceable condition. Limited to carrying no more than 51 persons on board.

Fire pump was removed due to internal problems with pump.

Fixed Power Bilge/fire pump was removed due to problems discovered the day of the inspection and replaced with a fixed hand pump reducing the passenger count to 49 PAX.

Port fuel shut off valve unable to be closed completely due to STBD valve being in the way.

3 cracks to stanchions on the Port side.

2 wire runs coming through the bulkhead on the STBD side are not watertight.

PORTSIDE FORWARD LOCKER WAS FOUND TO HAVE HANGING UNSUPPORTED ELECTRICAL WIRING. PROPERLY SECURE WITH METAL SUPPORTS SPACED NOT MORE THAN 24 INCHES APART.

PROPERLY MOUNT BATTERIES IN SUITABLE LINED TRAYS RESISTANT TO ELECTROLYTES.
