The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessel had emergency instructions that referenced equipment not installed on vessel and not tailored to vessel specifics. 46 CFR 185.510(b)

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. During Manoverboard drill deck hand onboard lacked authoritative communication calling distances during recovery phase of MOB with blindspots from captains POV at helm. 46 CFR 185.520 (a)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Fire Buckets not on board but ordered (receipt presented of the order) 46 CFR 181.610

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel missed annual inspection window, vessel cannot operate until annual inspection is completed.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel cannot operate until a DD/ISE inspection is completed. 46CFR176.600(c)(1)

Immediately after the addressing of resultant safety concerns, the owner, agent, master, or person in charge of a vessel involved in a marine casualty shall notify the nearest Sector Office of an occurrence materially and adversely affecting the vessel's seaworthiness or fitness for service or route, including but not limited to fire, flooding, failure of or damage to fixed fire extinguishing systems, lifesaving equipment, auxiliary power generating equipment, or bilge pumping systems. Provide proof that training of the Coast Guard casualty reporting requirements have been conducted with all managers, licensed captains and VSL crew. 46CFR185.202(a)(4)

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage. Except as allowed by paragraphs (b) and (c) of this section, before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the following: (1) The location of emergency exits , survival craft embarkation areas, and ring life buoys; (2) The stowage location of life jackets; (3) Either: (i) The proper method of donning and adjusting life jackets of the type(s) carried on the vessel including a demonstration of the proper donning if a life jacket. (4) The location of the instruction placard for life jackets and other life saving devcices; (5) That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master. The deckhand stated that the passenger safety orientation only communicated the location of the life jackets to passengers and no other information or instruction was provided.

All vessels must have navigation lights that are incompliance with the applicable sections of the International and Inland Navigation Rules. The stern light lens was incorrectly installed (turned 90 degrees).

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock Electrical cabling and wiring in way of operating station observed to have dead end and exposed electrical wiring, incorrect connectors, and no identifiable circuit information.

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage.Forward overhead light is inoperative.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. The first aid kit was missing items required by 160.041.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons in the water that have fallen overboard. Drill logbook showed a man overboard drill was conducted on 29 May 2019; however, the crewmember stated no drill was conducted. The only other drills logged were in 2015. Deckhand was unable to demonstrate or describe duties required for emergencies in accordance with this regulation.

The master shall conduct sufficient fire drills to make sure that each crewmember is familiar with his or her duties in case of a fire. Drill logbook documented dril dates, but did not contain a general description of the drill scenario and training topics.

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. U.S. Coast Guard inspector found that the displayed Certificate of Inspection was kept in a binder.

Each ring buoy on a vessel must: be approved in accordance with 160.050 in ubchapter Q of this chapter, ot other standard approved by the Commandant. The integeral lifeline manufactured in the ring life buoy was wasted, therefore negating its approval for its use as designed.

Additional personal flotation devices. The stoage locations of the personal flotation devices carried in addition to life jackets, and such as nto to be easily confused with that of the life jackets. Type II life jackets were stowed along with Type I life jackets.

Placards containing instructions for the donning and the use of the life jackets aboard the vessel mus be posted in conspicous places that are regularly accessible and visible to the crew and passengers. Posted instruction placard instructions were ont correct for life jacket type on board vessel.
