A space containing propulsion machinery must be equipped with a fixed gas fire extinguishing system specifically approved by the Commandant. Vessel did not have USCG approved paperwork attesting to engineering requirements of installed high pressure CO2 system.

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place. Fixed gas fire extinguishing system instructions did not provide a clear order of operation.

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Vessel did not have fixed fire fighting system servicing records available for inspection.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. MI Identified open venting gaskets on the engine room access hatches creating an improper seal to contain fixed fire extinguishing system gases.

Where subject to moisture, a storage cylinder must be installed to provide a space of at least 51 millimeters (2 inches) between the deck and the bottom of the storage cylinder. CO2 cylinder found stored on the deck.

Each life jacket must be marked with Type I retroreflective material. Found life jackets not fit for service with degraded missing retroreflective material.

Each ring life buoy must be marked with Type II retroreflective material. Found ring life buoy with degraded vessel name and retrorelective material.

Each floating waterlight on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. Found corroded lanyard clip.

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made. MI observed the Master demonstrate a lack of competency in regards to providing a passenger safety brief.

All cable and wire must: Not be installed with sharp bends. MI identified battery cables installed with sharp bends and twists.

Minimize accidental personnel contact with energized parts. MI identified exposed wiring on port battery cable.

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. MI identified water inside the battery box.

The design of all exhaust systems must ensure minimum risk of injury to personnel. Protection must be provided at such locations where persons or equipment might come in contact with an exhaust pipe. MI identified exhaust piping with degraded lagging.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. MI identified missing guards for rotating pulleys on both MDE's and both of the propulsion shafts.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Mi identified missing grease fittings on the shaft seal packing glands.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. MI Observed the machinery space portable light inoperable.

A vessel must carry a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent content. First Aid kit not present on vessel.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with 46CFR80.59 once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radio-Telephony Certificate. Each vessel must also carry a Bridge-to-Bridge Certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Inspector unable to verify certificate status because the vessel's Radio-Telephony and Bridge-to-Bridge Act certificates were faded and unreadable. Verify radio installation and post new certificates on the vessel. 46CFR80.903 46CFR80.1005

Throttle controller was not working. 46 CFR 182.05-1(a) (Old T)

Installation of diesel engine exhaust pipe shall be in accordance with 46 CFR 182.182.15-20. Exhaust piping shall be led to the point of escape with a minimum number of bends or elbows. It shall be gasket tight throughout its length and shall be properly supported by non-combustible hangers or blocks. MI observed exhaust leaking out of the exhaust manifold on both sides of the STBD MDE. 46 CFR 182.20-20(a) (Old T) 46 CFR 182.15-20(a) (Old T)

Vessels 26 ft up to 65 ft shall be provided with 1 fixed power pump (10 GPM) and 1 portable hand pump (10 GPM). Fixed power bilge pump was inoperable and portable hand pump was not available for inspection.

Employers must ensure that they and their crewmembers meet the requirements of 46 CFR 16 – Chemical testing. Vessel was unable to show proof of pre-employment testing or enrollment in random drug testing program.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it’s installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Marine inspector identified a lube oil leak on the starboard main engine.

At each initial and subsequent inspection for certification of a vessel, the owner shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, inspection of all accessible parts of the exterior and interior of the hull, the weathertight bulkheads, and weather decks. Main deck had a crack in between engine room hatches.

Remove fuel shutoff station shall be marked in at least 1-inch indicating purpose of the valve and direction of operation. Marine inspector identified the fuel shutoff valve wasn’t properly labeled.

Man over board and firefighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel was unable to provide log of any drills or training conducted.

Rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crew. Aft safety chain is no longer operable due to D ring broken off.

The electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. The vessel’s electrical system was found inoperable. The following systems were unable to be tested at the time of inspection: bilge pumps, high water alarms, radar, and navigation lights. Provide proper operation of the system.

Unable to get underway to conduct drills due to inoperable electrical equipment throughout the vessel. Conduct satisfactory man overboard and fire drill in the presence of a marine inspector.

Propulsion engines must have a gauge to indicate revolutions per minute. STBD engine tachometer does not accurately display the engine’s RPM. Conduct repairs.

Vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel has been out of service since being damaged during Hurricane Ida (AUG 2021) and failed to undergo second annual inspection within the 3 months before or after the anniversary date. Vessel's anniversary date is based off Certificate of Inspection issuance date of 14JAN2020. Vessel must complete satisfactory annual inspection prior to returning to service.

All fixed gas fire extinguishing systems aboard a vessel must be approved by the Commandant. The vessel representative was unable to provide the approval for the installed fixed firefighting system.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it’s installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port ME was inoperable. Vessel engine conducted repairs.

A space containing propulsion machinery must be equipped with fixed gas fire extinguishing. The fixed firefighting system was inoperable due to a tripped fuse.

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Vessel did not conduct man overboard drill during previous inspection for certification. Marine Inspectors observed vessel master unable to retrieve rescue mannequin, man overboard drill unsatisfactory. Vessel to complete satisfactory man overboard drill prior to carriage of passengers.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Marine Inspectors observed vessel fixed power bilge pump inoperable.

Each generator and motor must be designed for an ambient temperature of 50°C (122°F). Marine Inspectors unable to confirm vessel generator ambient temperature requirement, generator data plate partially illegible.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Marine Inspectors observed belts connecting STBD main engine to fixed power bilge pump not properly protected from hazard exposure.

vessels fixed fire-fighting system did not secure shipboard generator. Prior to movement fixed system shall be able to secure all internal combustion engines that share the engine space and proof of proper operation shall be provided to the cognizant OCMI.

EACH FIXED POWER BILGE PUMP MUST BE SELF PRIMING. BILGE PUMP DID NOT OPERATE DURING INSPECTION. BILGE PUMP IS ALSO USED AS FIRE PUMP. FIRE PUMP DID NOT OPERATE DURING INSPECTION. CORRECT TO SATISFACTION OF ATTENDING MARINE INSPECTOR. 46 CFR 182.520(c) 60c

VESSELS OPERATED SOLELY WITHIN 20 NAUTICAL MILES OF LAND MUST BE EQUIPPED WITH A VHF-DSC RADIO. VHF RADIO WAS NOT OPERATING DURING INSPECTION. CORRECT TO SATISFACTION OF ATTENDING MARINE INSPECTOR. 47 CFR 80.905(a)(1), 47 CFR 80.871 60c

VESSEL MUST CARRY UP-TO-DATE PUBLICATIONS AND CHARTS. PROVIDE UPDATED CHARTS AND PUBS TO SATISFACTION OF ATTENDING MARINE INSPECTOR. 46 CFR 184.420 16c

Port engine fuel lines leaking fuel; repair or replace to the satisfaction of the attending USCG MI.

STBD MDE water found draining from lube oil changing tube. Conduct repair and prove proper operation to attending Marine Inspector prior to moving vessel.

STBD MDE starter cable found chaffed through near entry point to battery box. Replace cable to satisfaction of attending marine inspector.

Steering space port rudder post found to have crack where frame and post meet. Conduct proper repair to the satisfaction of attending marine inspector.

Electrical distribution panel's generator ammeter lens missing cover. Correct or replace meter to satisfaction of attending marine inspector.

2" of water found inside battery box located forward in engine space. Remove water to the satisfaction of the attending marine inspector.

Installed CO2 system annual servicing not conducted, current/last service date Dec. 2014

Engine room ventilation closures inoperable, unable to secure ventilation to a CO2 protected space.

Forward engine room through bulkhead penetration for MDE J/W gauge is loose

Fuel fill electrical bonding is not connected.

Port MDE tachometer inoperable.

EAP contact numbers were inaccurate.

Battery installation was unsat, water found in battery box . Vessel utilizing wing nuts to secure battery cables to terminals.

Prove proper operation of fixed bilge pump/fire pump system to the satisfaction of the attending marine inspector.

Replace missing nut on starboard tiller arm.

Repair cracks in skeg on port and starboard side of the engineroom.

Conduct all hands man overboard and firefighting training and drills and provide documentation.

Install required engine RPM gauges at operating station as per 46CFR182.410(b).

Repair generator cooling water leak.

Clean bilges free of oil, excess oil on top of port engine and water/acid in battery boxes.

Prove proper operation of bilge system and alarms.
