PSCO observed wing-nut release for escape trunk from engine room was seized, preventing means of escape. 74 SOLAS 2001 II-2/2.2.6 17c

Fire doors in main vertical zones bulkheads, galley boundaries and stairway enclosures, other than power-operated watertight doors and those which are normally locked, shall satisfy the following requirements: the doors shall be self-closing and be capable of closure against an angle of inclination of up to 3.5 degrees opposing closure. PSCO observed A-0 fire door in main stairway inncapable of closing, on its own. 74 SOLAS 2001 II-2/30.4.1

A food ration as described in paragraph 4.1.5.1.18 totalling not less than 10,000 kj for each person the lifeboat is permitted to accomodate these rations shall be kept in airtight packaging and be stowed in a watertight container. Four of the fourty-six required lifeboat food rations lost their vacuum seal. SOLAS 2014 CH III Sec IV Reg 34 ISA CH IV 4.4.8.12

Every oil fuel pipe, which, if damaged would allow oil to escape from a storage tank shall be fitted with a cock or valve directly on the tank capable of being closed from a safe postion outside the space in the event of a fire. There were wood blocks observed that would prevent the valve from operating. 74 SOLAS II-2/Reg. 15.2.5
