Following installation, the master, owner, operator, agent, or person in charge of the vessel must properly maintain the BWMS IAW all manufacturer specifications. Vessels BWMS is inoperable. Vessel shall not discharge ballast into U.S Waters.

All other types of cargo spaces shall be fitted with either a fixed fire detection and fire alarm system or a sample extraction smoke detection system complying with the requirements of the fire safety systems code. FOLLOWING INSTALLATION, THE PERSON IN CHARGE OF THE VESSEL SUBJECT TO THEIS SUBPART MUST PROPERLY MAINTAIN THE BWMS IN ACCORDANCE WITH ALL MANUFACTURER SPECIFICATIONS. THE CG APPROVED BWMS MUST MEET THE APPLICABLE BALLAST WATER DISCHARGE STANDARD FOUND IN 151.2030 OF THIS SUBPART AT ALL TIMES OF DISCHARGE INTO THE WATERS OF THE U.S. THE VESSEL'S BWMS WAS REPORTED TO BE INOPERABLE

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed 16 emergency lighting fixtures and 14 non-emergency lighting fixtures that were inoperable.

The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a dark ground. The load line was found to be scraped away.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. In meeting these requirements, the company should ensure that the appropriate corrective action is taken. PSCO observed vessel's monthly safety inspection log showed lighting was inspected satisfactorily as recent as three days prior to exam indicating substandard inspection and documentation procedures. External audit recommended.

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. Vessel us storing 03 cylinders of refrigerant in the engine room with a temporary metal strap.

EVERY STEAM PIPE AND EVERY FITTING CONNECTED THERETO THROUGH WHICH STEAM MAY PASS SHALL BE SO DESIGNED, CONSTRUCTED AND INSTALLED AS TO WITHSTAND THE MAXIMUM WORKING STRESSES TO WHICH I MAY BE SUBJECTED. STEAM PIPE IN THE ENGINE ROOM DIRECTLY ABOVE THE BALLAST WATER TREATMENT SYSTEM WAS LEAKING EXCESSIVELY. THE CREW REPLACED THE FLANG GASKET AND THE LEAKING STOPPED. RECTIFIED ON THE SPOT.

Rescue boats shall be stowed in a s tate of continuous readiness for launching in not more than 5 minutes. Rescue boat engine was unable to start within 5 minutes. 74 SOLAS (97) III/14.1

0615- Rescue Boats- At all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. The outboard engine on the rescue boat was found by the crew to be inoperable on 2TMAY18 during weekly checks. 74 SOLAS(Cons. 97)III/19.2- 40ac

2050: Each emergency generating set to be automatically started shall be equipped with starting devices approved by the administration with a stored energy capability of at least three consecutive starts. A second source of energy shall be provided for an additional 3 starts within 20min unless manual starting can be demonstrated to be effective. Using second source of energy, generator shut down unexpectedly after 45 seconds. Crew was not able to restart the emergency generator for approximately 1.5 hours. 30C. 74SOLAS(1997 cons)II-1/44.2

1410-Propulsion M/E-No oil fuel tank shall be situated where spillage or leakege therefrom can constitue a hazard by falling on heated surfaces. Precautions shall be taken to prevent any oil that may exscape under pressure from coming into contact with heated surfaces. Fuel from fuel injector line was found leaking on to engine and pooling below the engine cylinders. SOLAS 74(97 Cons)II-2/15.2.4

The self closing fire doors(class A) in the stairway enclosures on A,B,C deck will not close completely as designed causing a fire hazard.

Vessel reported a loss of the number 6 cylinder of the main engine, stating that it was a problem with a valve.

Provide/install standard discharge connections for piping from machinery spaces. Complete within 30 days (07AUG04) and prior to any discharge of oily wastes.

Replace reflective tape on free fall life boat within thirty (30) days (07AUG04)
