A towing vessel's survival craft must meet the functional requirements of paragraphs (b)(1) through (5) of this section. Functional requirements describe the objectives of the regulation. Survival craft must be readily accessible. The vessels survival craft was not on board the vessel.

46 CFR 141.340(g)(1): Each life jacket must have a life jacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the life jacket. The vessel is required to have three life jackets IAW their COI. The vessel only had two life jackets with lights attached.

All portable fire extinguishers must be inspected or tested at least once every 12 months. The portable fire extinguishers were last inspected in September 2024.

46 CFR 140.720: Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. The vessel did not have an operational mast light.

46 CFR 67.7: Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. The vessels Certificate of Documentation expired in June 2025

All portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months. Certification or licensing by a state or local jurisdiction as a fire extinguisher servicing agency will be accepted by the Coast Guard as meeting the personnel certification requirements of NFPA 10 for annual maintenance and recharging of extinguishers. Annual inspections of the portable fire extinguishers were not completed and out of date.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Ventilation closures for engine room do not freely close.

A towing vessel may not be operated without having onboard a valid COI issued by the Coast Guard as required by 136.202. Vessel operating in the Great Lakes without a current COI from local OCMI. Vessel is in a Laid up status due to recent ownership change.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the 50% of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.
