Key personnel must complete training in 33CFR101.650(d)(2) by January 12, 2026. Company does not have a CYSO and all key personnel have not yet completed the training as of this inspection. 33 CFR 101.650(d)(4) 705: 60 days

Electrical equipment on a vessel must be installed and maintained to:(a) Provide services necessary for safety under normal and emergency conditions;(b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock;(c) Minimize accidental personnel contact with energized parts; and(d) Prevent electrical ignition of flammable vapors. MI observed multiple locations within the lazarette with exposed wiring, exposed junction boxes, and inoperable battle lanterns (emergency lighting). Make corrections and maintenance all electrical installations withing the lazarette and make as originally designed.

All machinery and equipment must be installed and maintained. Hydraulic steering pump operated as designed, however, the pump casing and wiring was heavily corroded. Due to apparent lack of maintenance and overall poor material condition of hydraulic pump, replace the pump in kind with new.

Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by noncombustible hangers or blocks. Owner self-reported main engine exhaust leak. Temp repairs were made due to unavailability of parts. No active exhaust leaks were present. Subbase is in process of fabricating a custom section of the exhaust piping. Estimated time of completion is 60 days. Remove lagging in all suspect areas of exhaust piping in engine room and lazarette and inspect for leaks. Make all necessary repairs.

Provide a working radar screen and prove proper operation of the vessel's radar.

Provide visual indicator for engine compartment bilge high water level alarm.

Post COI and Stability Letter where observation by passengers is likely.

Provide current FCC Vessel Certificates.

Electrical equipment used in a machinery space must be drip proof. Steering control junction box in the steering gear space is wasted and holed. Replace junction box in-kind.

Cable support rigging for the tower above the pilot house is not properly tensioned.

Observed 4 inch crack in weld seam of port rudder (inboard side) at the bottom of the rudder post and 5 other corrosion points along the vertical weld seams of the same area. Make proper repairs.

Vessel's scheduled drydock inspection on was not able to be completed on schedule due to the COVID-19 Pandemic. Vessel movement is restricted to domestic voyages only, per MSIB 09-20. A drydock inspection must be completed prior to carrying passengers on an international voyage. After 27AUG21, and in accordance with 46CFR176.600(c)(1), SUNDANCE BREEZE is prohibited from carrying passengers on any route until a drydock examination is completed.

The three inflatable buoyant apparatus on board require servicing. All three were last serviced April of 2019, but although brand new, they require annual servicing for the vessel's SOLAS route per 46 CFR 160.151-57(n)(1)(ii) and cannot go 2 years between service. Vessel has an EPIRB and is restricted to a domestic route <3 miles from land until the IBAs are serviced and properly mounted back on the vessel.

Replace time delay for CO2 system and provide service report.

Provide current EPIRB hydrostatic release.

Provide proof of random drug testing.

Amend SMS daily check sheet to log monthly EPIRB testing.

Remove or properly protect bus bars on generator.

Make proper connections on relay on generator.

Degrease breaker panel on center distribution panel.

Replace holed raw water hose outboard starboard main diesel engine.

Make proper connection of wires on center main diesel engine PTO clutch.

Provide watertight seal on aft hatch, main deck.

Repair grinding tiller connection bolt on port rudder post tiller.

Properly fasten deck hatch forward of bar in main cabin.

Obtain corrected Safety Radiotelephony Certificate with correct MMSI and Call Sign.

Demonstrate that correct MMSI is entered into DSC VHF radios and AIS.

Demonstrate properly operating emergency lighting in passenger cabin.

Mount engine room bilge high level alarm float switch.

Provide globe for engine room light.

WORK LIST: Deck plating wasted IVO water fill piping on port side. Provide repair proposal and repair.

Conduct drills in accordnace with vessel's SMS.

Conduct and log EPIRB tests monthly.

Clean, diagnose and repair oil leak on service generator.

Ensure complete wrapping on port engine exhaust lagging.

Provide protective globe for light fixture on starboard side of engine room.

Replace missing blanks on switchboard breaker panel.

Replace retro-reflective tape on both aft ring buoys.

Provide 300mm diameter bell.

Label PFD storage locations on upper deck.

Generator and fire pump sea-valves unservicable. Repair or replace sea-valves and make servicable.

Cracked welds found at fuel tank attachment points. Crop and renew cracked welds.

Operator failed to make vessel available for an annual inspection within 3 months of the anniversary date of 08Oct2015. Complete renewal inspection prior to carrying passengers.

Replace fire hose nozzle due to stripped threads.

Replace non-operational fire pump gauge and prove 60psi.

Provide proof of compliance with chemical testing requirements. Documentation was not available onboard the vessel at time of inspection.
