The emergency eqipment provided shall be maintained in good condition at all times. Reported to our office that the rescue boat's turbo suffered failure and the rescue boat is inoperable. Provide to our office risk mitigation proposal for the mean time while rescue boat is out of service and provide third party tech report of rescue boat being restored to satisfactory condition as originally designed.

If a fire detector is in a space, it must provide effective detection of fires most likely to occur in the space. The fire detection system in TLQs 17, 77, and 78 are not functional. Therefore, TLQs 17, 77, and 78 do not meet the requirements to be manned.

A fire main system must have enough hydrants so that each accessible space may be sprayed with at least two spray patterns of water. Eight fire hydrant control valves were frozen in the closed position. Repair or replace valves to ensure all accessible spaces are able to be reached by two spay patterns of water.

All installations shall be of marine type engines suitable for the intended service, designed, and constructed in conformance with the requirements of this sub chapter. The emergency generator turbo charger has an exhaust leak at the turbine housing.

All staging, platform,s and other working surfaces and all ramps, stairways, and other walkways must be kept clear of portable tools, materials, and equipment not in use and be promptly cleared of substances which create a tripping or slipping hazard. The main hydraulic oil tanks in both cranes are leaking, causing a slip hazard

Failure of power from the normal source. If there is a reduction of potential of the normal source by 15 to 40 percent, the diesel engine or gas turbine driving the final emergency power source must start automatically with no load on the emergency power source. After initial testing of alarms and shutdown, the emergency generator was unable to be restarted. Make repairs to the satisfaction of the CG Marine Inspector. 46 CFR 112.25-5

Each lifeboat, rigid life-raft, and rescue boat, unless otherwise stated, must carry the equipment specified for it in table § 108.575(b) of this section. Multiple flares in lifeboat B were missing or expired. Replace or renew necessary flares as required in Table 46 CFR 108.575(b). 46 CFR 108.575(b)

Each marine shipboard cable must meet all the requirements for construction and identification of either IEEE 1580, UL 1309, IEC 60092-353, or NPFC MIL-C-24640A or NPFC MIL-C-24643A (all five standards incorporated by reference; see 46 CFR 110.10-1. Cables PDS-1500a on the hull deck and CBA-8050 on the production deck had damaged sheathing which exposed the internals of the cable. Make repairs IAW the standard used during installation. 46 CFR 111.60(a)

The inspections and tests shall insure that the equipment and associated structure are in satisfactory operating conditions and fit for the service for which they are intended. #8 Chain winch has a hydraulic leak from the directional control valve which leaks into a drip tray directly above the water. Make sufficient repairs to ensure no further leaks are present. 46 CFR 61.01-1(b)

Persons responsible for actual operations, including owners, operators, contractors, and subcontractors, shall ensure that those operations subject to their control are conducted in compliance with workplace safety and health regulations of this part and, in addition, free from recognized hazards. The lower rung of the hull deck ladder is wasted causing the step to excessively flex under weight. The handrail between the cellar deck and production deck has a 3"X3" section of wastage that is sharp and nearly wasted through. Make repairs to the satisfaction of the attending marine inspector. 33 CFR 142.4(b)

Survival craft recovered by power means must be provided with safety devices that prevent damage upon recovery. Aft limit switch on lifeboat B is inoperable.

Each lifeboat must be fitted with equipment IAW 46 CFR 108.575(b). Handheld flares and smoke signals found expired. Flares were on order.

All Auxiliary engines installations shall be suitable for intended service. Emergency generator found with a substantial exhaust leak at the turbo.

Gas turbine installation shall meet 46CFR58.10-15. Could not test generator due to lube oil leak on generator B combustion compressor. Make effective repairs and provide Third Party report providing proper operation of all alarms and shutdowns of MTGG. 46CFR58.10-15 50 c

The emergency equipment provided shall be maintained in good condition at all times. As found lifeboats have not been launched as per EEP in 24+ months. All elements of the EEP shall be conducted annually. Launch each boat within 30 days and provide report with photographic evidence. 33CFR146.140 50 c

Each grounded system must have only one point of connection to ground regardless of the number of power sources. Observed multiple grounding wires on Hull Deck disconnected/wasted. Provide photograph evidence for repairs. 46CFR111.05-13 16 c

Emergency equipment provided shall be maintained in good condition at all times. Observed maintenance reports indicating work completed yet had not been conducted as per OEM. Conduct MOC to evaluate OEM maintenance required and provide plan to USCG. (Winch, davit, releasing gear, lifeboats). 33CFR146.15 705 c (90 days)

Emergency equipment provided shall be maintained in good condition at all times. Observed fast rescue boat had no personnel trained on board. Provide training certificates. 33CFR146.15 705 c (90 days)

ABS/USCG approved ISIP Manual, 2017 (rev 4) has not been implemented. onboard documentation shows (rev 2) onboard being followed. Provide gap analysis and ensure 3rd party is using (rev 4).

Marine Inspectors observed no dedicated davit for USCG approveed rescue boat. OIM stated rescue boat is launched/recovered by either crane on board facility. However, during man overboard drill, crew was in "stand by" status near the crane, the total time to launch/recover was more than 15 minutes (17 min). In addition, during the drill, marine inspectors observed an unsafe condition while crew tried recovering rescue boat. Crew member nearly thrown overboard while trying to connect rescue boat to crane in minimal sea state. Shortest possible time requirements cannot be met after normal working hours due to crane inactivity. Provide plan to USCG to meet the minimum time requirements for launching/recovering man overboard.

46 CFR 108.135 - Each boundary bulkhead of the galley must be an A Class. Galley door is out of alignment and in poor condition resulting in not properly closing. Replace/repair to original condition.

46 CFR 111.75-15 - Sufficient illumination must be provided by the E-lighting that provides for safe egress from space. Both emergency lights in temp quarters found missing and unop. Repair/replace to original condition.

46 CFR 107.305(v) - Each boundary bulkhead must be an A Class rating as per approved FCP. Several TLQ's Rox Blocks are installed incorrectly to include dead end wires, oversized cables, and misaligned blocks.

No BII extinguisher located next to internal combustion engine (Haliburton Generator) on pipe rack. Provide at least one BII extinguisher.

Provide enough fire hydrants so that Zone 14 on Kodiak MCC module can be sprayed with 2 streams of water.

Replace 2 jumper hoses on rig with fire main piping or suitable alternative.

Provide proof of generator shut down requirements of rig generators.
