All ships of 3,000 gross tonnage and upwards shall, in addition to meeting the requirements of paragraph 2.5, have: a 3GHz radar or, where considered appropriate by the Administration, a second 9GHz radar. Vessel's secondary S-Band, 10cm, 3GHz radar system is inoperable. Vessel unable to affect repairs while in Apra Harbor, Guam and is required to repair system by next port, Honolulu, HI. 74 SOLAS (2020 Cons.) V/19.2.7.1 - 15 c.

Vessel entered the port of Pago Pago prior to submitting a one-time waiver for a Vessel Response Plan that did not cover American Samoa. 33 CFR 155.5025(a)

Each battery must meet the requirements of this subpart. Wingnuts were found at battery terminal connections, where a normal hex nut should be used for marine applications. Replace all wingnuts with hex nuts to make a more secure connection for vessel operations. 46 CFR 188.25-1 46 CFR 111.15-1 IEEE 45-2002

Each panel board must meet section 17.1 of IEEE 45-2002 for good marine practice. Missing blanks were found throughout several panel boards exposing the bus bar. Check all grey power panels and install blanks where missing throughout vessel. 46 CFR 188.25-1 46 CFR 111.40-1 IEEE 45-2002

Table 193.50-10(a) indicates the minimum required classification for each space listed. Vessel missing the required 160-B for the incinerator room in accordance with the table cited above and the vessel's COI. Provide the approved semi-portable fire extinguishing in accordance with Table 193.50-10(a) to the satisfaction of the attending marine inspector. To be resolved by October 8, 2025. 46 CFR 193.50-10(b)

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. On systems in which the CO2 cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Each control valve to branch lines must be marked to indicate the related space served. CO2 cylinders are located outside the protected spaces with no schematic diagram of the system as described above posted in the CO2 cylinder space. Provide schematic diagram of the system as described in the regulation to the satisfaction of the attending Marine Inspector. 46 CFR 193.15-10(h)

The electrical engineering details shall be in accordance with subchapter J (Electrical Engineering) of this chapter. Hazmat Locker intake fan electrical automatic damper is not certified for use in a hazardous location or explosive atmosphere. Install an approved electrical automatic damper meeting the regulations prescribed in 46 CFR 111.105-9 to the satisfaction of the attending Marine Inspector. 46 CFR 188.25-1(a)

The electrical engineering details shall be in accordance with subchapter J (Electrical Engineering) of this chapter. The wiring for the Hazmat Locker intake fan electrical automatic damper is spliced and the connections encapsulated in electrical tape. Wire the new certified electrical automatic damper for the Hazmat Locker intake fan as per 46 CFR 111.105-9 to the satisfaction of the attending Marine Inspector. 46 CFR 188.25-1(a)

Each self-propelled vessel, except as provided in paragraph (d) of this section, must have an electric or mechanical engine order telegraph system from the navigating bridge to the engine room. Vessel reported issue with engine control: recurring delay on the starboard engine response to command. Once safely moored, vessel is restricted from movement until proper operation of the engine is demonstrated to the satisfaction of an attending marine inspector. 46 CFR 113.35-3(a)

Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must include fire detection and extinguishing, flooding safety, propulsion, maneuvering, electric power generation and distribution, and emergency internal communications. PSTPs have not been updated. Current crew not able to use the current PSTPs to achieve proper shut down. 46 CFR 61.40-6

Each survival craft must be stowed so as not to require lifting from its stowed position in order to launch, except that a davit-launched liferaft may be lifted by a manually powered winch from its stowed position to its embarkation position. The vessel's 25 person liferafts' maximum weight of 4945.5 lbs exceeds the port and starboard liferaft davits' manufacturer established safe working load of 4629 lbs. Provide documentation from the davit manufacturer that the davits' safe working load can be increased or submit a proposal to the cognizant OCMI to address this issue. The vessel's total persons allowed is reduced to 46 persons until this deficiency is cleared to satisfaction of the attending marine inspector. 46 CFR 199.130 (a) (7) (i)

All ships 3,000 gross tons an upwards shall, in addition to having a 9 GHz radar, have a 3 GHz radar. The vessel's 3 GHz radar is not operational. Repair radar. 74SOLAS(20)V/19.2.7.1

The installation of all systems of an electrical engineering shall be in accordance with the requirements of Subchapter J. Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. It was reported that the number 1 main diesel generator experienced an under-excitation event, which caused a "blackout" condition, resulting in a loss of propulsion. Demonstrate the proper operation of the number 1 main diesel generator to the satisfaction of the attending USCG Marine Inspector. 46 CFR 188.25-1 (a) 46 CFR 111.01-1 (a)(1)

The boundary bulkheads of general laboratory areas, chemical storerooms, galleys, paint and lamp lockers and emergency generator rooms shall be of “A” class construction. In the chemical lab aft of the fwd door there is a cable penetration that is not fully installed.

Tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Generator number 3 high differential pressure alarm locked out. Resolve to the satisfaction of the attending MI or remove from the KILOMANA’s PSTP.

No repairs or alterations affecting the stability or safety of the vessel with regard to the hull, machinery, and equipment shall be made without the knowledge of the Officer in Charge, Marine Inspection. Port ballast water treatment system online chlorine and TRO monitor (TRO 4) inoperable. Repair in-kind and prove proper operation to the satisfaction of the attending MI.

All vessels on an international voyage are required to have a Cargo Safety Construction Certificate. The renewal and periodical surveys shall include an inspection of the equipment referred to in paragraph (b)(i). (b)(i) says complete inspection of the fire safety systems and appliances life-saving appliances and arrangements except radio installations, the shipborne navigational equipment, means of embarkation for pilots and other equipment to which chapter II-1, II-2, III, and V apply. All ships of 150 gross tonnage and upwards and passenger ships irrespective of size, shall, in addition to the requirements of paragraph 2.1, be fitted with a spare magnetic compass, interchangeable with the magnetic compass as referred to in paragraph 2.1.1. The vessel does not have a spare magnetic compass.

The applicable standards established by the ABS Steel Vessel Rules (incorporated by reference 46 CFR 58.03-1) may be used as the standard for design, construction, and testing of main and auxiliary machinery except as modified in this subchapter. Vessel starboard MDE has been taken offline due to a grounded motor fan, and respective crew cannot repair while underway. Utilize towing vessel(s) of adequate horsepower and maneuverability to enter the Honolulu Harbor and all mooring operations. 46 CFR 58.05-1

All enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. The Motor Room #2 exhaust ventilation closer activation handle broke during operation.

Each drain and scupper is closed by the mechanical means required by 155.310. All containments did not have the mechanical plug readily available.

An inflatable liferaft or inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant for the particular brand, and in accordance with servicing procedures meeting the requirements of part 160, subpart 160.151, of this chapter no later than the month and year on its servicing sticker affixed under 46 CFR 160.151-57(n), except that servicing may be delayed until the next scheduled inspection of the vessel, provided that the delay does not exceed 5 months. Vessel's annual inflatable liferaft servicing date will occur while vessel is underway. Vessel's inflatable liferafts shall undergo their annual servicing at a USCG approved facility at earliest opportunity upon returning to port, but no later than 5 months beyond the annual servicing date.

Vessel has had three blown fuses in the field power supply of the port and stbd main propulsion motors, one on October 11th and two on October 17th, all resulting in losses of propulsion. Provide an OEM technicians repair proposal that identifies the cause of the fuse failures, a recommended repair plan, & preventative actions to prevent recurrence of these failures. While transiting to or from a harbor or within 1 nm from shore the vessel must have assistance from a towing vessel of adequate horsepower and maneuverability. Such assistance towing vessels must be under the direction and control of an individual holding a license or a Merchant Marine Credential (MMC) authorizing them to engage in assistance towing under the provisions of 46 CFR 11.482.

No repairs or alterations affecting the stability or safety of the vessel with regard to the hull, machinery, and equipment shall be made without the knowledge of the Officer in Charge, Marine Inspection. A fuse on the vessel's port propulsion motor opened causing a loss of propulsion while the vessel was performing dynamic positioning for research purposes. The fuse was replaced in kind and control of the port propulsion drive motor was regained.

No repairs or alterations affecting the stability or safety of the vessel with regard to the hull, machinery, and equipment shall be made without the knowledge of the Officer in Charge, Marine Inspection. It has been reported that the vessel's starboard propulsion motor "tripped" off line becoming not operational while the vessel was underway. Trouble shoot the cause of the starboard propulsion motor's failure and provide a repair proposal to the cognizant USCG [name withheld] the cognizant USCG OCMI has approved the repair proposal, complete the repairs to the satisfaction of the attending Marine Inspector.

No repairs or alterations affecting the stability or safety of the vessel with regard to the hull, machinery, and equipment shall be made without the knowledge of the Officer in Charge, Marine Inspection. It has been reported that the vessel's starboard propulsion motor "tripped" off line becoming not operational twice over a 1 month period while the vessel was underway. Repair the vessel's starboard propulsion motor to the satisfaction of the attending USCG Marine Inspector.

It was reported that the number 4 main diesel generator's voltage regulation capabilities failed which resulted in the vessel experiencing a dead ship casualty. Demonstrate the proper operation of the number 4 main diesel generator to the satisfaction of the attending USCG Marine Inspector.

Vessel reported stbd propulsion main drive faulted resulting in a loss of propulsion. Faulty damping resistor was replaced.

At all times, each lifesaving appliance must be in working order. The rescue boat davit's hydraulic pump does not operate as designed. Prove proper operation of vessel's rescue boat davit prior to departure from Sector San [name withheld] zone.

The starboard SCR suffered a circuit board failure. Make permanent repairs to SCR, provide tech report and prove proper operation of propulsion motor.

Each pressure vessel must be examined or tested every 5 years. The extent of the test or examination should be that necessary to determine that the pressure vessel's condition is satisfactory and that the pressure vessel is fit for the service intended. In addition, each pressure vessel listed on the Certificate of Inspection that is fitted with a manhole or other inspection opening so it can be satisfactorily examined internally, must be opened for internal examination every 5 years. Prepare air receiver on main deck for internal inspection.

The marine inspector must check the settings of the safety or relief valves on all pressure vessels twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor. Demonstrate proper operation of relief valves on air receiver on main deck.

Remove 2" Discharge Hose Connections, 2" Wafer Butterfly Valves, 2" Wafer Check Valves, and T- Pipes with flange installed for portable pump quick connections located in 1st Platform Port and Starboard Scientific Stores and Machinery Rooms as drawn in Pages 3 and 4 of Bilge System Modifications Drawing Number 12008-529-01.

Provide weathertight louvers/closure doors to exhaust/intake located in superstructure for both port and stbd engineroom and emergency generator room. Machinery space ventilators in an exposed position shall be fitted with strong covers of steel or other equivalent material permanently attached in their proper position and capable of being secured watertight.

Remove holdback hooks for A-Class space entry doors to maintain A-Class structural boundries. General construction of the vessel shall be such as to minimize fire hazards.

Code 01108: During and after any survey made by the assigning and issuing authority, those items, fitting, etc., which are found to be in an unsatisfactory condition by the surveyor shall be repaired or renewed in order to place the vessel in a satisfactory condition. No load line certificate shall be issued, endorsed, extended, or reissued or delivered to a vessel subject to this subchapter until after unsatisfactory conditions have been corrected. Found three steel ventilation hatches and steel structural fire protection bulkheads for ventilation hatch closures holed, corroded, and wasted. Ventilators pass through the superstructure and shall have substantial constructed coamings of steel or other equivalent material. The following windows were found pitted •SR 01-08, •SR 01-09, •SR 01-10, •SR 01-1246. 46 CFR 42.09-45

Code 07109: A pressure relief valve set to relieve between 2,400 and 2,800 lbs per square inch shall be installed in the distribution manifold to protect the carbon dioxide extinguishing system piping in the event that all branch line shutoff valves are closed. Pressure relief valve data plate found painted over and inspector was unable to verify set pressure. Clean data plate or replace in kind. 46 CFR 193.15-15 (d)

Code 07109: All carbon dioxide extinguishing system distribution valves and controls shall be of an approved type. CO2 pressure switch found installed in the system to protect the machinery space does not display a Coast Guard approval number. Provide Q spec Coast Guard approval for fixed extinguishing system protecting the engine space. 46 CFR 193.15-10 (g)

Update PSTP procedures and demonstrate proper operation of remaining PSTP items listed on the worklist dated 14 NOV 2017. Must be completed by next scheduled annual inspection or NLT 24 AUG 2017 (end of 90 window for next annual inspection), which ever comes first.

Provide list of material specifications for plan review of start air system piping prior to commissioning. This part of the system to remain locked out / tagged out until corrected.

Each vessel must carry life jackets approved under approval series 160.155,160.176 or 160.177, each life jacket provided on board the vessel was approved under approval series 160.055 for domestic voyages only. The vessel is restricted to a domestic route.

Prove the proper installation of the vessel’s automation upgrade in accordance with the approved plans.

Provide an approved PSTP and DVTP, NLT the vessel's COI renewal inspection or 9MAR17.

Provide adequate banding and chafing gear to ensure proper support and protection for all cable installed on the vessel.
