Navigation Assessments not being conducted on short transit evolutions. Ensure NAV assessments conducted each time prior to getting underway.

Fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months. Full records of inspection/testing not on board vessel. Provide records.

Each liferaft must have a painter system providing a connection between the vessel and liferaft. Vessel's liferaft painter was not attached to weak link. 46 CFR 141.310 46 CFR 199.130(c)(6)

Vessel EPIRB must be stowed with controls set for automatic activation so that it will float free. EPIRB hydrostatic release unit was expired. 46 CFR 141.310 46 CFR 199.130(c)(7)

Portable fire extinguishers must be inspected in accordance with NFPA 10, and recorded by logbook or tag.Fire extinguishers aft of pilothouse was not being inspected and logged monthly. 46 CFR 142.240(a)(1) 46 CFR 142.240(a)(2)

Each lifebuoy must be marked in block capital letters with the vessel name. Port and stbd lifebuoys stenciling was faded. 46 CFR 141.360(b)(4)

Fuel systems for towing vessel main engines and generators must have a documented maintenance plan. Vessel did not have a fuel system maintenance plan. 46 CFR 143.255(a)

The master shall ensure fire-fighting drills are conducted at least once each month. Fire drills were not being conducted at the required frequency. 46 CFR 142.245(a)

Individuals, who are not crewmembers, on board a towing vessel must receive a safety orientation prior to getting underway or as soon as practicable thereafter, to include: (a) The location, operation, and use of lifesaving equipment; (b) Emergency procedures; (c) Methods to notify crewmembers in the event of an emergency; and (d) prevention of falls onboard. Provide orientation to non crew.

Each liferaft must be arranged for float-free launching. Liferaft hydrostatic release unit and weak link was installed upside down.

(a) All lifesaving equipment must be tested and maintained in accordance with the minimum requirements of § 199.190 of this chapter, as applicable, and the vessel's TSMS, if the vessel has a TSMS. (b) Inspections and tests of lifesaving equipment must be recorded in the TVR, official logbook, or in accordance with any TSMS applicable to the vessel. The following minimum information is required: (1) The dates when inspections and tests were performed, the number or other identification of each unit inspected and tested, the the results of the inspections and tests, and the name the crewmember, surveyor or auditor and any others conducting the inspections and tests; and (2) Receipts and other records documenting these inspections and tests must be retained for at least 1 year after the expiration of the COI and made available upon request. Lifesaving equipment inspection records inconsistently inspected and recorded. Ensure consistent inspections and recordkeeping maintained. 46 CFR 141.235

All fire-extinguishing systems must be inspected / tested at least once every 12 months. Last FFE service date more than 12 months. Provide FFE servicing and maintenance documentation.

Flammable / combustible products onboard must be stored in a designated storage room / cabinet with additional 40BC PFE when not in use. Propane tanks stored in metal cabinet without additional PFE and 01 propane tank stored outside of cabinet directly on deck. Store propane tanks IAW with requirements and provide additional 40 BC PFE for storage cabinet.

Means to detect smoke that alerts crew must be equipped in berthing spaces. Heat detectors found in berthing areas. Provide smoke detectors in berthing areas.

Install SOLAS B liferaft properly

Install properly serviced Immersion suits for all crewmembers.

Provide onboard proper amount of Flares for route.

Repair pin hole in super structure near/under the PORT weather deck door.

Provide onboard approved “workvest” with lights for all crewmembers.

Provide lights and whistles for all type I lifejackets.

Conduct required maintenance, monthly and annual of all lifesaving appliances to include checklist for each item in accordance with manufacture instruction.

Install proper fire extinguisher bracket for the B-II installed near locker on main deck.

Properly label fire station and fire pump controller in engine room.

Provide onboard required fuel system maintenance program in accordance with manufacture recommendation.

Repair damaged dog for STBD side air vent closure.

Remove daisy chain of extension cords powering the cooling fan for steering hydraulics.

Install proper anchor light in accordance with navigation rules 2 mile visibility.

Relocate amber towing lights in vertical line.

Provide onboard most current version of the navigation rules book.

Provide onboard all required dayshapes.

Provide copy of FCC license for Master of vessel.

Provide documentation for all crewmembers enrolled in DAPI with testing results.

Install lagging on all exposed areas of the generator exhaust piping.

First aid kit, add means to splint broken bones.

Remove any and all flammable material from machinery space.

Provide info or replace both pressure vessels installed on STBD machinery space or stencil with relief valve pressure.

Provide onboard monthly /annual drills and instruction to include Health and Safety training/ Fire and Man overboard. 14SEP21: will be working with company safety officer to ensure regulation will be followed. extended this reg to 14 days.
