Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Lifeline and floating waterlight lanyard on ring life buoy was dry rotted. Replace lines. 46 CFR 176.808(b)

Abandon ship, man overboard, and fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Log of drills was missing. Perform and log drills. 46 CFR 185.520(f) 46 CFR 185.524(d)

All observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Insulation was hanging down from the ceiling and onto the 01 deck batteries, causing fire hazard. Secure insulation. 46 CFR 176.830(a)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. Galley light switch cover was cracked and outdoor outlet on 01 deck was missing cover. 46 CFR 183.200(b)

Fuel lines must be accessible, protected from mechanical injury, and effectively secured. Minor chafing was observed on fuel return line on port engine against the engine mount. Secure line to prevent chafing. 46 CFR 182.455(b)(3)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. The Safety Radiotelephony Certificate was expired. Vessel restricted to 1,000 feet from shore. 47 CFR 80.903

Stations in the maritime service must be licensed by the FCC. Station License was expired. Vessel restricted to 1,000 feet from shore. 47 CFR 80.13

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. During COI inspection, vessel was unable to conduct man overboard drill due to lack of crew. Within 7 days of issuance of this CG-835V, coordinate a man overboard drill while underway using vessel's equipment and procedures with minimum crew per COI to the satisfaction of the attending marine inspector. 46 CFR 176.808

A vessel not more than 19.8 meters (65 feet) in length is not required to carry a rescue boat unless: (1) The vessel carries passengers on an open or partially enclosed deck; and (2) The cognizant OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. The forward embarkation ladder, which is used during man overboard drills to allow for rescue, has a damaged and frayed cable for the winch. 46 CFR 180.210(b)

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction. The starboard side handrail closest to the bow of the vessel on the embarkation ladder was found to be cracked at the bottom connection point. 46 CFR 177.900(b)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Ventilation was unable to be fully secured when tested; requiring ventilation closes fully before operation.

Each cable entering a box or fitting must be protected from abrasion, and must meet the following requirements: Each opening through which conductors enter must be adequately closed... In damp or wet locations, each cable entrance must be watertight... Each junction box, connection box, and outlet box for use in a damp or wet location must be of watertight construction. Loose wiring found in Engine Room and Bar Compartment. Recommended securing in junction box.

Operator was unable to prove operation of bilge system. Make repairs and prove operation of bilge suction in each watertight compartment to the satisfaction of the attending Marine Inspector. 46 CFR 182.500(a)

Vessel permanently installed two 5000W heaters on the upper deck without plan approval or installation verification. Provide elementary one-line diagram of heater installation, cable list, rating of circuit breakers and switches, and electrical load analysis or remove heaters. Heaters are not to be operated prior to plan approval and installation verification. 10/27/2023: Vessel rep has disconnected heaters and tagged out breaker while Hargrove completes elementary one-line diagram of heater installation, cable list, rating of circuit breakers and switches, and electrical load analysis. Heaters are not to be reconnected until plans are approved and installation is completed to the satisfaction of the Coast Guard. 10/12/23: Vessel rep requested 14 day extension for def no. 2. Extension granted and due to expire 10/30/2023

Second deck fire hose has a hole.

Temporary electric cords were found throughout the vessel being used as permanent electrical installations on second deck band area, bar, operationg station, along railings on the vessel, and 1st deck bar area.

Two of the liferings on the vessel were found to be deterorated and need to be replaced.

Vessel did not have max passenger capacity signs going to 2nd deck and up to the operating station as required by stability letter.

Fire extinguisher on the second deck bar area did not have a bracket.

Charts not up to date. Update charts.

Retro reflective tape on all life jackets needed replacement. Replace retro on all life jackets.

Public address system inoperable. Repair PA system.

Emergency procedures were not posted at the operating station. Post procedures at the operating station.

Emergency fuel shut off handle was inaccessible. Provide access to emergency fuel shut off handle.

Steel jacketed flex hose attached to the intake side of the fire pump was found to be leaking. Replace in kind as per original design.

Mast head light inoperable. Repair or replace the mast head light.

First aid kit not approved. Acquire necessary items.

Stability letter not posted at the operating station. Post stability letter at the operating station.

No passenger limit decal IAW the vessels stability letter posted on the second deck. Post new decal leading up to the second deck.

Dead end wires were found exposed on the starboard side immediately outside the bridge door. Remove or reconnect exposed wires.

Portable emergency lights missing at the operating station and in the engine room. Install working lights at both the operating station and in the engine room.

Ammeter missing from battery charger. Install ammeter on the battery charger.

Identified evidence of a slight hydraulic fluid leak coming from the piston seals on the hydraulic steering ram. Repair or replace hydraulic steering ram.

Public address system inoperable.

Dead end wires in second deck concession area. Remaove or place in proper junction box.

Vessel does not have a max passenger count placard for the second deck. Max count as per stability letter on second deck is 130.

Fire pump does not reach proper pressure (35 PSI vice 60 PSI), show proper operation.

#2 generator is inoperable, make operable.

Engine room ventilation closures do not fully close, make closures vapor tight.

EPIRB self test/ignite switch does not return to the close position, have EPIRB serviced to show proper operation.

Fire station on the 02 deck is inoperable, make operable.

Electronic compass is showing deviation, verify proper operation.

Secure galley/concession stand appliances.

Forward bow void watertight hatch is not moused, mouse hatch to vessel.

Forward engine room bulkhead has broken receptacle, repair/replace.

Stern has loose wires on 01 deck, either remove the wires or place wires in watertight enclosure.

Remove check valve from the fire main/bilge system or provide portable hand pump for each space.

High water alarm becomes disabled after acknowledging any single compartment, make operable.

Port and starboard-side engine room ventilation duct closure inop. Ensure ventilation to engine room may be secured.

Spaces toring (and protected by) carbon dioxide not properly marked. Ensure spaces are properly marked.

Dishwasher, soda machine, and alcohol are unsecured. Ensure gear adrift is secured for rough weather.

Provide current radio operator permits for all acting captains.

Drug consortium roster is not up-to-date. Provide updated drug consortium roster.

EAP supervisor training certification not completed. Provide certification for all acting captains.

Grey water valve not labeled on tank on starboard side. Ensure all valves are properly labeled.

Galley windows are cracked and broken. Repair or replace windows.

Ground tackle (anchor) does not have mousing to secure shackle. Ensure mousing is provided for ground tackle.
