Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Three (03) wooden life float paddles were found split or broken. 46 CFR 180.175(d)

At least one ring life buoy must be fitted with a floating waterlight. Vessel's ring life buoy waterlight (port forward location) did not work. 46 CFR 180.70(d)

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. 46 CFR 185.604(b)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. (ISO 18813:2006 Standards) Vessel's first-aid kit had expired and incomplete contents. 46 CFR 184.710

A serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Inspector observed expired alcohol test strips on board. 46 CFR 185.212 ; 701C

A visual and audible alarm must be provided at the operating station to indicate a high water level. Inspector observed port and starboard aft compartments did not have visual and audible alarm. 46 CFR 182.530(a)(1) ; 701C

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio Beacon (EPIRB) installations, as set forth in 47 CFR part 80. Inspector observed expired EPIRB battery and additional EPIRB not registered with NOAA. Rectify w/in 7 days. 46 CFR 184.502 ; 705C

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Inspector observed one paddle broken, 01 light out, & retro tape degraded. 46 CFR 180.175 (d) ; 16C

The owner or managing operator of a vessel shall ensure that the vessel complies with the laws and regulations applicable to the vessel and that the vessel is otherwise satisfactory for the intended service. Inspector observed protective guard for belts on port MDE not attached. 46 CFR 176.402 ; 50C

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. Inspector observed lagging not secured on exhaust. 46 CFR 177.970 ; 50C

A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of § 111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. Inspector observed temporary cord used to power ventilation fans. Remove fans or install permanent wires for equipment. 46 CFR 183.340 (r) ; 50C

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters: "Emergency Exit, Keep Clear". MI observed the engine room escape hatch cable and stenciling (topside) need to be replaced. 46 CFR 185.30-15(a)

The float-free link must be secured to the painter at one end and secured to the vessel on the other end. MI observed two life float I-bolts must be replaced due to severe corrosion and do allow the life float to be secured to the vessel. Passenger count reduced by 24 until deficiency rectified. 46 CFR 180.20-1(1)(iii)

Each life float shall be fitted with a life line and shall be equipped with two paddles, a water light and a painter. MI observed four port and two starboard life float painters must be replaced due to deterioration. 46 CFR 180.15-1(a)

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel shall install system to the satisfaction of attending USCG Marine Inspector. 46 CFR 181.405(c)

Flammable items not otherwise covered by the regulations of this subchapter, such as rechargeable batteries, including lithium ion batteries utilized for commercial purposes, must be handled, stored, and operated in a way that mitigates the risk of hazardous conditions. Vessel shall develop plan to the satisfaction of attending USCG Marine Inspector. 46 CFR 185.364

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts, U.S. Coast Pilot, and Coast Guard Light List. Vessels charts, U.S. Coast Pilot, and Coast Guard Light List were all out of date, and not corrected. 46 CFR 184.420(a)

An adult life jacket must be provided for each person carried on board a vessel. 01 life jacket was found to be torn, and not in serviceable condition. Passenger count reduced by 1. 46 CFR 180.71(a)

All cable and wire must not be installed with sharp bends. The cables connecting the port sider generator to the battery were installed with sharp bends coming off the terminal of the battery 46 CFR 183.340(b)(5)

All cable and wire must be installed with metal support spaced not more than 24 inches apart and in such a manner as to avoid chafing and other damage. The cable going to the transducer in the starboard side #2 hull compartment was not supported in any way and was lying across the deck. 46 CFR 183.340(b)(4)

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. The remote fuel shutoff valves were not marked. 46 CFR 185.608

The Emergency check-off list shall contain not less than the applicable portions of the "recommended Emergency Checkoff List", 185.25-5. Cut off air supply to fire- close hatches, ports, doors, and ventilators, etc. Vessels vent covers for the engine room did not fit snugly over the vent openings and did not cut off the air supply. 46 CFR 185.25-1(b)(Old-T) 46 CFR 185.25-5(c)(1)(Old-T)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. The hull plating in the port side lazaret space was found to have 1/8th inch pitting on the centerline between the forward transverse frame 1 and 2. Resolve at the 2022 dry dock scheduled for the fall of 2022. 46 CFR 177.01-5 (Old-T)

Crop and renew fractured welds found on bottom plate stiffners located in compartment #2, port side, at the weld to the collision bulkhead. 46 CFR 176.700

Crop and renew pitting, measuring 1/8" in depth or more, found on side shell plate in port/stbd compartment #2. Pitting located on the inboard side of the hull. 46 CFR 176.700

Identified corroded thru hull valve (positive shut off valve) handle in the lazarette. Replace thru hull valve handle in lazarette. 46 CFR 182.520

A self propelled vessel of 65 feet or more in length engaged in commercial service shall carry a class A or class B AIS device. 33 CFR 164.46 (b)(1)and (2)

Each vessel that operates on an oceans route in warm water must be provided with life floats of an aggregate capacity that will accommodate at least 100% of the total number of persons permitted on board. Each vessel certificated to operate on a coastwise route within 3 miles of land must either be provided with life floats of an aggregate capacity that will accommodate at least 100% of the total number of persons permitted on board or meet the standard for collision bulkheads and be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board and have onboard a type 1 406 Mhz EPIRB. Each vessel certificated to operate on a limited coastwise route must be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board. Found three 12 person life floats with decayed passenger nets. Renew life floats in accordance with manufacturer specifications and Coast Guard guidance. Total persons onboard on oceans routes reduced to 63 persons. 46 CFR 180.202(b)

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at pump outlet. The pump outlet shall be fitted with a pressure gage. During inspection Marine Inspector witnessed faulty pressure gauge on fire pump. Replace pressure gauge on outlet side of fire pump. 46 CFR 181.10-1(c)

Propulsion machinery shall be capable of operating at constant marine load under such requirements without exceeding its design limitations. During inspection, Marine Inspector identified starboard generator fuel hose having deep cut/groove. Replace starboard generator fuel hose with same type hose or J-1942 or equivalent. 46 CFR 182.05-1

All escape hatches and other emergency exits shall be marked on both sides using at least 1 inch letters, "EMERGENCY EXIT, KEEP CLEAR". During inspection, Marine Inspector witnessed lack of required markings on engine room emergency exits. Mark all emergency exits with proper markings. 46 CFR 185.30-15

On a vessel on which the number of passengers permitted on the upper decks is limited by stability criteria, as indicated by the vessels stability letter, the max number of passengers allowed on the upper decks must be indicated by a durable marking of numbers and letters at least one inch in height at the entranceway to each such deck. During inspection, Marine Inspector identified lack of required markings leading to upper deck indicating max number of passengers allowed. Provide correct marking to indicate max number of passengers allowed on upper deck. 46 CFR 185.30-3(e)

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The gauge at the pump outlet read 45 PSI during the test witnessed by the Marine Inspector. Verify proper operations to the required 60 PSI. 46CFR181.10-1(c)

Penetrations and openings in watertight bulkheads must have means to make them watertight. The watertight door located in the starboard side, under deck void forward of the diesel tanks was in bad repair. The gasket surrounding the door has past its useful life and is no longer creating a watertight seal on the knifes edge. Additionally the top and bottom dogs were no longer touching the backing plate on the door opening and need to be adjusted to allow for a tight seal on the door. 46CFR171.114(a)(2)

The lifeline and pendants must be as furnished by the manufacturer with the approved life float. The pendants on two life floats located on the starboard side of the top deck of the vessel is loosing it strength and becoming brittle. Replace each pendants on these two life floats according to manufacturer requirements. 46CFR180.175(f)(1)

A vessel of more than 65 feet in length must have at least one fire axe located in or adjacent to the primary operating station. The fire axe's mounting location has been damaged and can no longer hold the axe. Replace. 46CFR181.600

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of 46CFR4.06. Alcohol test strips provided by ASAP used for testing within 2 hours of a serious marine incident are expired. Replace. 46CFR16.240

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. The first-aid kit on board did not meet requirements listed. Replace. 46CFR184.710

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot or similar publication; Coast Guard Light List; and Tide tables. The vessel had outdated publications. Replace with current. Additionally extracts from the publications for the areas to be transited my be used instead of the complete publication. 46CFR184.420(a)(2),(3), and (4)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. The number of passengers allowed on the top deck has been removed due to repairs conducted and is no longer readable. 46CFR185.602(g)
