On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. The pump outlet must be fitted with a pressure gauge. MI observed maximum fire pump pressure gauge reading of 38 psi. Repair gauge or provide evidence that the pump is capable of producing the required throughput of 50 gallons per minute. 46 CFR 181.300(b)

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, each hose must:(1) Be lined commercial fire hose that conforms to UL 19 (incorporated by reference, see 46 CFR 175.600) or hose that is listed and labeled by an independent laboratory recognized by the Commandant as being equivalent in performance;(2) Be 15.25 meters (50 feet) in length and 40 millimeters (1.5 inches) in diameter; and(3) Have fittings of brass or other suitable corrosion-resistant material that comply with NFPA 1963. MI observed pinhole on towards hydrant end of fire hose. Replace fire hose to the satisfaction of MI. 46 CFR 181.320(b)

System design and installation must be in accordance with the Marine Design, Installation, Operation, and Maintenance Manual approved for the system. Bulkhead penetration between engine room anf fuel void is not sealed. 46CFR181.410(a)(3)

Chart aboard vessel was found not up to date. 46 CFR 185.420 As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible;

Currency of alcohol testing devices were out of date. 46 CFR 4.06-15(a)(1) The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI.

EPIRB registration was expired. O/O provided proof of new registration through e-mail (see attached).

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel did not complete credit drydock inspection which was due by 29 FEB 2024. Vessel will need to complete drydock inspection prior to carrying passengers.

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. Several lights on child's lifejackets were inoperable. Replace as necessary. 46 CFR § 180.75 16c

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a dry dock examination and an internal structural examination at least once every two year. The last dry dock and internal structure exam was conduct February 10, 2020. A dry dock and internal structure exam must be conducted prior to passenger.

Provide proof of SOLAS grade retro tape currently installed on lifebuoys and lifefloats, replace as necessary.

Provide repair plan for hull plate replacement in lazzarette near bilge suction prior to credit drydock.

Clean and provide a gauging report for hull platting along centerline under deck platting in #2 space prior to credit drydock

Tested CO2 system engine shut downs and alarms, pressure switches failed to shut down all engines and activate audible alarm. Vessel must prove proper operation of alarm and shut down devices. 46CFR181.410 (b)(10)

Vsl is broadcasting incorrect Name, Vsl is broadcasting incomplete dimension information (AIS Antenna Position "X" equals 0) and Vsl is not broadcasting static draft information

MMSI number needs to be programed into VHF Radio

Bilge suction in steering gear space/laz is not working. Vessel limited to carrying no more than 49 pax until fixed.

COI needs to be posted in area accessible to passengers.

Vessel did not meet the ground detection requirements of 46CFR183.324(c). Vessel must istall ground detection.

Lash paddles to lifefloats in accordance with 46 CFR 180.175(f)(2).

Repair fractured weld in way of transverse deck frame #1, center line, fore peak, as indicated by Marine Inspector.

Complete weld around spool piece in way of galley sink drain overboard discharge.

NY01: Submit repair proposal to the OCMI regarding the crack and puncture of the vessel's hull rec'd on 19NOV. Proposal must be approved to the satisfaction of USCG Marine Inspector.

NY02: Proceed to shipyard to complete satisfactory repairs to the vessel's hull.

NY03: Submit in writing Form 2692, Report of marine casualty, to this office, ACTNY.

Provide portable hand bilge pump w/ appropriate length of suction hose (10 gpm).

Permanently repair bilge system manifold in way of #1 compartment branch line.
