The gaskets installed in the fire doors in the oil purifier room, engineer store, emergency escape trunk on platform 8600, the paint store, and the BOSN store are heavily deteriorated or dry rotted and do not maintain the fire boundary requirements.

All life - saving appliances and arrangements shall comply with the applicable requirements of the Code. the normal equipment of every lifeboat shall consist of: watertight receptacles containing a total of 3 L of fresh water for ach person the lifeboat is permitted to accommodate... The freefall lifeboat can accommodate a total of 20 persons and should carry a total of 60 L of fresh water, but is only provided with 24L of fresh water.

Efficient guard rails or bulwarks shall be fitted to all exposed parts of the freeboard and superstructure decks. Guard rail and stanchion located port side at cargo hold #3 are severely damaged.

... On ships carrying at least two search and rescue dry locating devices and equipped with free-fall lifeboats one of the search and rescue locating devices shall be stowed in a free-fall lifeboat and the other located in the immediate vicinity of the navigation bridge... The ship is equipped with a free-fall lifeboat but both SARTS were stowed on the navigation bridge.

Rescue boats shall be stowed: in a stat of continuous readiness for launching n not more than 5 min... The crew took longer than minutes to start the rescue boat engine.

Fire-fighting systems and appliances shall be kept in good working order and ready for immediate use. 04 fire hoses were observed by PSCO to not be in proper working order with leaks identified.

Before the ship leaves port at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO's observed failure of the rescue boat engine to start within the allotted timeframe of five minutes on three separate occasions.

A safety management certificate shall be issued to every ship by the Administration or an Organization recognized by the Administration. Ships ISM-SMC was expired. Ship has surveys scheduled for next port. 74 SOLAS 2020 cons CH IX Reg 4.3

The condition of the ship and its equipment shall be maintained to conform with provisions of the present convention to ensure that the ship in all respects will remain fit to proceed to sea without presenting an unreasonable threat of harm to the marine environment. Ship could not demonstrate proper operation of the oily water separator. MARPOL 73/78 2011 Cons Annex I Reg 6.4.1

The fire damper shall operate automatically but shall also be capable of being closed manually from both sides of the bulkhead or deck. The fire dampers located in the purifier room were incapable of closing manually - 74 SOLAS (2001) II-2/48 - 10C

Where the emergency source of electrical power is a generator, unless a second independent means of starting the emergency generator is provided the single source of stored energy shall be protected to preclude its complete depletion by the automatic starting system. PSCO identified cross over valve open from the emergency air supply tank to the main. 74 SOLAS (2004) II-1/43.3.1.2 - 10c

After any survey of the ship under regulations 7, 8, 9, or 10 has been completed no change shall be made in the structural arrangements, machinery, equipment without the sanction of the administration. Thermal oil cabinet located in the engine room that control quick closing valves for this system have been disabled by the crew. 74 SOLAS(2020) I/11(b) 50ac

All external high-pressure fuel delivery lines between the high-pressure fuel pumps and fuel injectors shall be protected with a jacketed piping system capable of containing fuel from a high-pressure line failure. A jacketed pipe incorporates an outer pipe into which the high-pressure fuel pipe is placed, forming a permanent assembly. The jacketed piping system shall include a means for collection of leakages and arrangements shall be provided with an alarm in case of a fuel line failure. The Port Main Diesel Engine leakage alarm was found inoperative and in alarm since 21May2021 - 74 SOLAS (2001 Cons) Ch II-2/15.2.9 - 30ac

In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. No fuel oil leaks shall be situated where spillage or leakage therefore can constitute a hazard by falling on heated surfaces. The port main diesel engine secondary duplex fuel filters were found excessively leaking fuel. The port main diesel engine crankcase breather discharge was found routed into a five gallon bucket with approximately 2.5 gallons of fuel in it located outboard side of the engine. A fuel oil leak was discovered on the governors for both main engines, which then drained into two five gallon buckets. The main engines’ fuel supply lines had a temporary repair that was leaking and being collected in a five gallon bucket. The fuel module system in the purifier room is leaking from the relief valve and being captured into drum then transferred to the number 40 overflow tank via portable pump. Cardboard was found to be placed around the fuel oil leaks on the main engines’ governors; the pieces of cardboard were saturated with fuel oil creating an extreme fire hazard. Portable buckets and drums were found throughout the engine room nearly completely full from the collection of leaking fuel oil.- 74 SOLAS (2001 Cons) Ch II-2/15.2 - 30ac

All electrical apparatuses shall be so constructed and so installed as not to cause injury when handled or touched in a normal manner. PSCO identified exposed wires forward of anchoring arrangements on the bow of the main deck - 74 SOLAS (2004 Cons) Ch II-1/45.1.3 - 16ac

The Oil Record Book Part I shall be completed on each occasion, on a tank-to-tank basis if appropriate, whenever any of the following machinery space operations takes place in the ship: collection and disposal of oil residues (sludges). The oil residue/ sludge that was found to be leaking from the main engines was not recorded in the Oil Record Book - MARPOL 2017 ANNEX I/17.2.3 - 17ac

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements, which may be established by the company. In meeting these requirements, the company should ensure that: the inspections are held at appropriate intervals; any non-conformity is reported, with its possible cause, if known; appropriate corrective action is taken. Due to the objective evidence in the above deficiencies, the vessel is not in substantial compliance with relevant conventions, calling into question the adequacy and implementation of the vessel’s SMS under the ISM code. PSCOs collected objective evidence that the vessel was not identifying non-conformities and following the inspection and reporting requirements of its SMS. An external audit conducted by the Flag is required within 30 days to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the completed audit report to USCG prior to departure from port - 74 SOLAS (2014 Cons) Ch IX/Reg 3.1 ISM Code Part A Reg 6.4 10.2.2, 5.1.5, and 9.1 - 30ac

Fire extinguishers shall be situated ready for use. Portable extinguishers which have been discharged shall immediately be recharged or replaced with an equivalent unit. Fire extinguisher #23 in the engine room is low on pressure and therefore not in compliance. - 74 SOLAS (2020 Cons) Ch II-2/10.3.2.4 - 17ac

No person may operate any vessel equipped with installed toilet facilities unless it is equipped with an operable Type II device. Vessels marine sanitation device type II chemical treatment system is inoperable - 33 CFR 159.7(a)(1) - 10c

Piping systems shall be of construction adequate for the service for which they are intended and should be installed and protected to reduce danger to persons on board due to hot surfaces and other hazards. The fuel service line in the purifyer room is kincked and multiple lagging pieces are missing. 74 Solas (04) II-1/26.1

Machinery installations shall be protected to reduce any danger to persons on board with due regard to hazards. 05 oil buckets are in the engine room, shaft seal is full of oil in the bilge, forward bilge on star/port full of oily water. Bilge alarm not functioning. 74 Solas (04) II-1/26.1

FSD bewteen changing room and engine room has a closing mechanism which prevents the door from being secured and will not stay closed.

The machinery, boilers and other pressure vessls, associated piping systems and fitings shall be of a design and construction adequate for the service for which they are intended. Lube oil piping directly above #2 main engine purifier is leaking. SOLAS74(04 Cons) Ch. II-1/Reg 26.1 16a

1499 - Steering Gear System - The design pressure for steering gear piping shall be at least 1.5 times the max working pressure expected. Actuator return hydraulic lines are different than originally installed and are not marked with working pressure. Class to attest to proper component installation and working pressure. 74 SOLAS 09 con. II-1/29

Conducted rescue boat drill with unsatisfactory results. Crew members assigned to rescue boat were unable to release the hook promptly upon boat reaching the water. Crew also experienced difficulty making the hook ready for retreival of rescue boat. Issued requirement to prove crew profenciency prior to departure to the satisfaction of the attending marine inspector.

Vessel has not submitted a Vessel Response Plan according to the E-VRP website. In accordance with ALCOAST 398/05 a new regulation is going to be issued for non-tank vessels over 400 gross tons to submit a Vessel response plan. Currently the NTVRP has been submitted and approved for the following COTP zones: Jacksonville, FL, Miami, FL, and Tampa, FL.

Cargo hold 3, fuel tank 18 punctured with 7" hole due to container being dropped on tank top. Provide report from flag state or direct representative of flag approving temporary repairs.

The two fire hydrants on the main deck in front of the accommodations are a different type than the other hydrants on the vessel. They have threaded connections vice twist on quick connects. Provide hydrants and hoses that are interchangeable through out vessel. Prior from departure [name withheld] Beach. Until all hydrants and hoses are compatible the 2 different hoses and hydrants must remain connect

Dog on water tight hatch for pipe tunnel in engine room is stripped and unable to be tightened. Replace dog to the satisfaction of class prior to 28May05.
