Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Discovered multiple dead end wires under the helm console. Remove the dead end wires, prior to credit drydock, to the satisfaction of the MI. 46 CFR 120.200 (b)

An adult life jacket must be provided for each person carried on board a vessel. In addition, a number of child-size life jackets equal to at least 10% of the number of persons permitted on board must be provided. Identified two child and two adult life jackets that were damaged beyond repair. Life jackets were replaced during the inspection. 46 CFR 117.71 (a & b)

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Operation of the fire main system and checking of the pressure at the most remote and highest outlets. Vessels fire main found weeping from a cracked weld during operational testing. 46 CFR 115.810(a)(3)

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Emergency light cage broken by port side boarding door. 46 CFR 120.410

Portable fire extinguishers must be refilled when the net content weight loss exceeds that specified for fixed systems Table 115.810(b). Portable fire extinguisher found with pressure gauge in the red. 46 CFR 115.810(b)

After loading and prior to departure, and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's stability letter. Upper decks were found marked with passengers counts in excess of those allowed by the current stability letter. 46 CFR 122.315(a)

Each buoyant apparatus must have Type II retro reflective material meeting subpart 164.018. BA missing retro reflective material. 46 CFR 160.10(p)

A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Medical certificates must be issued and will remain current for the following periods of time, unless otherwise noted on the certificate. Captain was missing MMC medical card. 46 CFR 15.401(c)

Life jackets may be continued to be used if they are maintained in good and serviceable condition. Life jacket found to be torn. 46 CFR 180.71(d)

Each survival craft must be; Stowed in a way that neither the survival craft nor its stowage arrangements will interfere with the embarkation and operation. IBA line tied incorrectly to the weak link. 46 CFR 117.130 (a)(2)

The cognizant Officer in Charge, Marine Inspection (OCMI) determines the total number of persons permitted to be carried on a vessel. In determining the total number of persons, the OCMI may consider the total weight of passengers, crew, and variable loads; stability restrictions and subdivision requirements of the vessel; the vessel's route, general arrangement, means of escape, and lifesaving equipment; minimum manning requirements; and the maximum number of passengers permitted in accordance with § 115.113 of this subpart. Vessel is limited to 50 passengers on the third (02) deck and 100 (01) passengers on the second deck.

The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Vessel has exceeded allowable limit on weight changes since the last stability evaluation. Vessel shall conduct new stability testing and obtain new stability letter.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Relocated emergency fuel shutoff valve pull cables to be tested.

At each inspection for certification of a vessel, the owner or managing operator shall be prepared to have the vessel ready for inspections of the hull structure including the following: Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Identified 3" crack in overhead of main deck with temp patch in place and no documented Correction Report. Make permanent repairs at next credit drydock.

A Coast Guard marine inspector will conduct required periodic and follow-on inspections necessary to ensure compliance with Coast Guard regulations. Certain components of company action plan lists inspection intervals at daily/monthly/quarterly (steering/lifesaving/firefighting) that are not being documented.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected the cognizant OCMI may conduct an internal structural examination in any affected space... and to effect permanent repairs. Holes and corrosion were discovered above port main engine exhaust in overhead. Provided plan proposal to MI for permanent repairs.

Locate cause and make appropriate repairs to port main diesel engine cooling system and prove proper operation to the satisfaction of the attending Marine Inspector prior to carrying passengers..

Remove and replace petcock on port keel cooler with one that has a locking cap.

Repair hole in engine room air supply duct for starboard side engine room air intake located exterior of house at aft end of duct.

Repair wasted area of engine room air supply duct for port side vent shaft at forward end of duct work in lower deck accom space.

Make vapor tight by renewing wasted metal in way of aft stairtower over engine room access.

Renew in accordance with NVIC 7-68 hull plating in way of engine room as identified by gaging located in area just aft of port generator.

Perform gaging of areas in way of engine room, under port shaft log and deteriorated area just aft of port generator. Hull repair to be completed contingent of gaging report.

Perform gaging of lazzerette forward bulkhead in way of sewage tank footprint. Bulkhead shall be determined adequate for use in accordance with wastage allowance from NVIC 7-68, or renewed.
