Vessels described by 46 CFR 175.110(d) must comply with the regulations in § 177.500. Vessel's two means of escape from overnight accommodation space lead to the same compartment. Passengers cannot utilize overnight accommodations. 46 CFR 177.115(c)

A ladder leading to a deck scuttle may not be used as a means of escape except:(1) On a vessel of not more than 19.8 meters (65 feet) in length, a vertical ladder and a deck scuttle may be used as not more than one of the means of escape from passenger accommodation space. Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are readily accessible and visible to the crew and passengers. Vessel has life jacket placard for only one of the three types of life jackets on board. 46 CFR 185.516

All fittings on fire hose shall be of brass, copper, or other suitable corrosion resistant material. Nozzles on port and starboard fire hose were severely corroded. 46 CFR 181.15-10(f) Old T

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Auxiliary boilers and heating boilers and their associated piping and fittings will be given separate consideration and must meet the applicable requirements of subchapter F...heating boilers must be tested or examined every three years. There were no records of boiler or furnace testing. Hoses were not CG approved fire rated.

Electrical installations shall be to the satisfaction of the [name withheld] cord connections were used for fixed lighting and compressor.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel missed it's drydock that was due 28FEB23. Vessel must complete drydock examination prior to carrying passengers. 46 CFR 176.600

MI observed that on the Generator switchboard the ammeter and the frequency (hertz) meter were inop. Require vsl repair/replace equipment for proper operation. 46cfr183.10-5(g)

MI observed ground detection on the generator switchboard inop. Require repair/replace equipment for proper operation. 46cfr111.05-21

MI observed numerous black ground cables on the engine room batteries had cuts and crack on the insulation. Require cables be replaced with proper battery cables. 46cfr183.05-40(a)

Identified misalignment of installed portable fire extinguishers with revised regulations and policy. Vessel has one legacy B-II CO2 fire extinguisher in ER, with remaining fire extinguishers NFPA-10/UL rated or dual rated w/USCG rating. In addition, when the COI was revised to list the new performance-based rating, the required arrangement was not updated. For the current operating arrangements, layout, and configuration, the VOYAGER should have (1) 10-B:C on the upper deck/ operating station, (2) 2-A w/ (1) in the main deck salon and (1) in the below deck berthing, and (2) 40-B:C located in the concession area outside of the Engine Room with (1) for responding to Engine Room fires and the other for fires in the Concession area from risk of fire from the furnace or appliances. Vessel must add two (2) 40-B:C extinguishers in the vicinity of the Concession area/access to the Engine Room and replace the 10-B:C in the Salon with a 2-A extinguisher. Life Ring Buoy on upper deck by wheelhouse is in unserviceable condition. Vessel is required to have a USCG Approved Ring Buoy w/ Light in this location. Replace with new, approved Lighted Ring Life Buoy. CITE: 46 CFR §180.70

One older Kapok PFD unserviceable (Kapok cells leaking). Replace unserviceable PFD with new, USCG Approved PFD. CITE: 46 CFR §180.71

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Observed significant rusting and wastage of the end shell of the 15 gal combined air compressor/receiver in Compartment 2. Prove safe condition of or replace air receiver. CITE: 46 CFR §182.100

Electronic propulsion control systems must be designed so that a loss of power to the control system does not result in increased shaft speed or propeller pitch, as per 184.620(c). A propulsion control system drawing should be submitted along with a ‘Test Protocol’ to verify this. The Test Protocol should consist of a limited Qualitative Failure Analysis (QFA) and Design Verification Test Procedure (DVTP), only including analysis of the loss of power to the control system.
