It shall be the responsibility of the master to assure himself that all exposed cargo hatches and other openings in the hull of his vessel are closed , made properly watertight by the use of tarpaulins, gaskets or similar devices, and in all respects properly secured for sea before leaving protected waters. The lid for the aft rake void was deformed and would not properly seat. Repair/replace void lid.

Tank Barge AXL 244 (CG 51398) unloaded caustic barge was a part of the grounded tow package. Prior to loading cargo the barge MUST undergo an internal structural exam.

A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this sub-chapter. The following deficiencies were identified: 1. Eight inch deep longitudinal inset in hull found along port beam approximately 27' in length in way of #1 void space. 2. Cracks in in hull in various locations of inset. 3. Deck gunwale creased and bent upward in various locations of inset. 4. Water found inside #1 void space. Complete the following to allow for CG Marine Inspectors to expand exam to assess structural damages. 1. Pump out all water from #1 void space 2. Make all spaces adjacent to hull damages gas free by a certified marine chemist Make the following repairs to the satisfaction of the attending marine inspector: 1. Crop and renew all side shell plating and deck gunwale associated with inset IAW NVIC 07-68. 2. Crop and renew all #1 port void hull side transverse trusses, vertical and diagonal stanchions, and side frames from the #2 transverse truss aft to void bulkhead

Draft Marks need to be rewelded and painted a contrasting color.

Longitudinal Frame was rolled and not supporting by chord in Void #3 bay 2. Needs to be replaced. Had old bracket as repair that was not in compliance with CG policy.

Bow rake had six inch split in bilge knuckle @ 5th port side cord.
