A lifeline attached to a lifebuoy must be, if synthetic, a dark color or certified by the manufacturer to be resistant to deterioration from ultraviolet light. All of the lifelines on respective lifebuoys are synthetic and not dark in color (yellow and orange combo) nor is there evidence that the lifelines are resistant to deterioration from ultraviolet light. Either replace with a dark in color synthetic line, natural fiber line or provide proof of UV resistance to that satisfaction of attending CG marine inspector.

Each lifebuoy stowage position must be marked with either the words “LIFEBUOY” or “LIFE BUOY”, or with the appropriate symbol. The stowage location of several of the lifebuoys onboard are marked with the wrong symbol. Either mark with the appropriate symbol or mark with the word lifebuoy or life buoy to the satisfaction of attending CG marine inspector.

The self-igniting light may not be attached to the lifebuoys required by this section to be fitted with lifelines. Several lifebuoys onboard have both a waterlight and lifeline attached. Ensure all lifebuoys have the appropriate attachments.

Each liferaft must be arranged to permit it to drop into the water from the deck on which it is stowed. A liferaft stowage arrangements meets this requirement if it — (i) Is outboard of the rail or bulwark; (ii) Is on stanchions or on a platform adjacent to the rail or bulwark; or (iii) Has a gate or other suitable opening large enough to allow the liferaft to be pushed directly overboard and, if the liferaft is intended to be available for use on either side of the vessel, such gate or opening is provided on each side of the vessel. The current stowage arrangement of the existing liferaft does not appear that it can be manually launched and not strike the main deck or gunwale before landing in the water. Either prove that the existing arrangement meets the standard above or install a launching arrangement that does to the satisfaction of attending CG marine inspector. ** 46 CFR Subchapter W does not offer an exemption or alternative to this requirement for vessels on an Oceans route **

The lifejackets stowage positions must be marked with the words “LIFEJACKETS” as appropriate, or with the appropriate symbol from IMO Resolution A.760(18). The lifejacket stowage location for those stowed in the bunk rooms are not marked. Make appropriate markings for those lifejackets contained in the bunk rooms.

All crew spaces are to be constructed and arranged in a manner suitable to the purpose for which they are intended and so that they can be kept in a clean, workable, and sanitary condition. Berthing and accommodation spaces were used for storage, and toilet and washroom facilities are not in service due to lack of water to the facilities. Either ensure galley/mess and berthing/toilet spaces have appropriate water supply and are clean and free of debris to the satisfaction of a CG marine inspector, or request the to have overnight accommodations onboard removed from the COI.

The following items which were verified as required during MARSEC level 1 in the security plan, and were not being implemented: No ID checks took place when inspectors arrived, the deck was not monitored by PIC, there was no record of security exercises for the company or record of drills for the current crew, the VSA stated that AIS is utilized when vessel is moored or anchored to ensure visibility of incoming vessels however the vessel does not have an AIS, no training records were on board for any crew members as stated is required for all crew even if they do not have security related duties. 33 CFR 104.215(e) (2)

The requirements of 58.10-5(a), (c), and (d) shall apply to diesel engine installations. All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Both the #1 port generator and the #2 starboard generator were found to be excessively leaking jacket water and lube oil. Rectify deficiency prior to the vessel's departure. 46CFR96.03(a) 46CFR58.10(a)

The requirements of 58.10-5(a), (c), and (d) shall apply to diesel engine installations. All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Both generator high pressure fuel lines were missing supports or current supports were missing hardware enable the lines to chafe on the engine blocks. Rectify deficiency prior to the vessel's departure. 46CFR96.03(a) 46CFR58.10(a)

The requirements of 58.10-5(a), (c), and (d) shall apply to diesel engine installations. All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. A lube oil leak was found on the starboard forward prime mover for the cargo pump. Rectify deficiency prior to vessel's departure. 46CFR96.03(a) 46CFR58.10(a)

Valves installed on the outside of the oil tanks must be arranged with a means of manual control locally at the valve and remotely from a readily accessible and safe location outside of the compartment in which the vales are located. The high suction remote fuel shut off valve for the forward fuel tank on the starboard side was not accessible from outside of the space containing the fuel oil tank. 46CFR96.03(a) 46CFR56.50-60(d)(1)

All other openings to a protected space must be capable of being closed. Both starboard aft engine room ventilation closures were unable to close properly. The most aft engine room ventilation closure was missing hardware for an even closure seat and the closure forward of the aft contained wasted gasket material. 46 CFR 95.16-30 (C)

This subpart contains requirements for fluid power transmission and control systems and appurtenances. Except as otherwise provided for in this section, these requirements are applicable to the following fluid power and control systems: Starting systems for internal combustion engines used for main propulsion, main or auxiliary power, as the prime mover for any required emergency apparatus, or as the source of propulsion power in ship maneuvering thruster systems. The fuel filters for both generators were missing containment underneath where they were installed. 46CFR96.03(a) 46 CFR 58.30-1(a)(5)

Electrical installations on vessel's must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. A missing breaker on the port switchboard was uncovered enabling the possibility of electrical shock to a crew member. 46CFR96.05-1(a) 46CFR111.01-1(a)(2)

Each operation described in paragraphs (d), (e), and (f) of this section shall be fully recorded without delay in the Oil Record Book so that all entries in the book appropriate to that operation are completed. The following items were missing or not correctly logged in the oil record book: Signature missing of person having charge of the ship at the bottom of each page; the entry on 28JUL2023 was missing quantity of bunkers delivered and time transfer was conducted; weekly sounding were not being taken of tanks listed on 3.1 of the IOPP Form A. 33 CFR 151.25(h)

For each ship subject to regulation 5 and 6 of this Annex, details of fuel oil for combustion purposes delivered to and used on board shall be recorded by means of a bunker delivery note that shall contain at least the information specified in appendix V to this Annex. The following bunker delivery notes were missing from the last year: 28JUL2023; 04NOV2022; 31OCT2022; 29OCT2022; and 28OCT2022.

The bunker delivery note shall be accompanied by a representative sample of the fuel oil delivered taking into account the guidelines developed by the organization. The sample is to be sealed and signed by the supplier’s representative and the master or officer in charge of the bunker operation on completion of bunkering operations. There was not bunker samples on board of all bunkers taken from: 28JUL2023; 04NOV2022; 31OCT2022; 29OCT2022; and 28OCT2022. MARPOL VI/18.8.1

Manned barges shall carry at least two fire axes. The port side fire axe was found in bad repair. 46 CFR 95.60-5(b)

At each inspection for certification, periodic inspection and at other times necessary, the inspector will determine that all fire-extinguishing equipment is in suitable condition. During operation test of the fire pump the MI noticed excessive water leakage from the pump. Crew was able to rectify the deficiency on the spot. 46CFR91.25-20(a)

Each life raft or group of life rafts must be arranged for float-free launching. The life raft was not installed in accordance with the manufacturer's instructions. The crew was able to rectify the deficiency on the spot. 46CFR96.06(a) 46CFR199.130(c)(7)

Illustrations and instructions in English, and any other appropriate language as determined must be conspicuously displayed at each muster station. The box containing the life jackets at the muster station was missing a donning placard. The crew was able to provide a placard on the spot. 46CFR96.06(a) 46CFR199.80(c)(5)

All vessels shall have efficient guard rails or bulwarks on decks and bridges. Upper rear guard rail on the corner of the second deck broken and requires repair. 46 CFR 92.25-5(a)

A complete recharge shall be carried for each self-contained breathing apparatus. Vessel requires another spare self-contained breathing apparatus charge. 46 CFR 96.35-20

Each fireman's outfit must consist of one self-contained breathing apparatus, one lifeline with a belt or a suitable harness, one flashlight, one flame safety lamp, one rigid helmet, boots and gloves, protective clothing, and one fire ax. One fireman’s outfit missing gloves. 46CFR96.35-10

46 CFR 97.15-35 Onboard training, musters, and drills must be in accordance with subchapter W. Vessel shall complete and document required fire, abandon ship, man overboard, and security drills. To the satisfaction of attending USCG Marine Inspector during next annual exam.

Barges with sleeping accommodations for more than six persons shall have permanently exhibited for the guidance of the officer in charge of the vessel the following plans: (a) General arrangement plans showing for each deck the fire control stations, the various sections enclosed by fire-resisting bulkheads, together with particulars of the fire alarms, detecting systems, the sprinkler installation (if any), the fire extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating systems including particulars of the master fan controls, the positions of dampers, the location of the remote means of stopping fans, and identification numbers of the ventilating fans serving each section. For vessels constructed on or after September 30, 1997 or for existing vessels which have their plans redrawn, the symbols used to identify the aforementioned details shall be in accordance with IMO Assembly resolution A.654(16). Vessel is required to provide an up to date and accurate Fire Control Plan to the satisfaction of marine inspector.

At each inspection, the inspector shall exmaine bilges in the machinery spaces to see that there is no accumulation of oil, excess amounts of oil and water observed in the machinery space bilge. 46CFR91.25-45(a)

It shall be the duty of the PIC to see that the vessel's fire fighting equipment is at all times ready for use. During test of main fire pump, multiple continous leaks were observed.

No oil record book entries could be found since 24SEP2017. Provide oil record book.

Replace oil soaked non-metallic fuel hoses, non-approved fuel hoses, and make permanent piping for non-metallic fuel hoses exceeding 30 inches. All fuel hoses in the engine room must be approved ASME J1942 hoses with factory-assembled fittings.

Relocate inflatable life raft and bracket to STBD side, port side does not have room to board raft due to dredge pipe and does not have an embarkation station.

Provide a corrective action plan to ensure all drills to include firefighting, abandon ship, and security are being completed and logged. The vessel could only provide records of two drills being conducted in 2015.

Demonstrate proper operation of the general alarm system. Ensure alarms are heard/ visually noticed all throughout the engine compartment, including the ECR.

Provide fuel filter bowl, located forward of genset #1.

Properly mount fuel oil pressure gauges, located forward of genset #1 and near genset #5.
