A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Bilge suction line in Lazarette was found to have no strainer. Add Suitable strainer to bilge suction line and make available for inspection to the attending marine inspector.

Each vessel must carry a valid certificate while subject to the Bridge-to-Bridge Act. The vessel does not have a valid Bridge-to-Bridge certificate. 47 CFR 80-1005

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely.

Every vessel must be fitted with a suitable magnetic compass designed for marine use, to be mounted at the primary operating station.Except on a vessel limited to daytime operations, the compass must be illuminated. The magnetic compass onboard is not illuminated.

Doors on a cooking appliance must be provided with hinges and locking devices to prevent accidental opening in heavy seas. Sea rails, with suitable barriers to prevent accidental movement of cooking pots, must be installed on a cooking range. Cooking equipment onboard does not have sea rails on the stove and hinge on the oven door.

The bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. Aft Engine Room bilge suction does not have adequate strainer. 46 CFR 182.25-5(b)(Old-T)

Pressure vessels described in § 54.01-5(c)(3), must be visually examined by a marine inspector prior to installation. The marine inspector also reviews the associated plans and manufacturers' data reports. Pressure vessel installed onboard has not been examined by a marine inspector.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom.

All pressure vessels other than unfired steam boilers shall be protected by pressure-relieving devices that will prevent the pressure from rising more than 10 percent above the maximum allowable working pressure, except when the excess pressure is caused by exposure to fire or other unexpected source of heat. MI discovered relief valve rated at 200 psi where MAWP is 175 psi. Replace relief valve.

Vessels described by 46 CFR 175.110(d) must submit plans to the cognizant OCMI, in accordance with 46 CFR 176.700, for the installation and use of monitoring device to ensure the wakefulness of the watchmen required in paragraph (a) of this section. Vessel has not submitted Watchmen Plans to the OCMI for review and approval. 46 CFR 185.410 (b)

A propulsion engine control system, including pilothouse control, must be designed so that a loss of power to the control system does not result in an increase in shaft speed or propeller pitch. Vessel re-powered and does not have the following: For Subchapter T vessels with electronic propulsion control systems using microprocessors or computers, Automation Testing should be limited to testing the functionality outlined in 46 CFR 184.620, which requires that: “a loss of power to the control system does not result in an increase of shaft speed or propeller pitch.” This testing shall be conducted in accordance with the Marine Safety Center (MSC) accepted and reviewed testing protocol (Qualitative Failure Analysis (QFA), a document identifying the assumed outcomes of individual component failures) and (Design Verification Test Procedure (DVTP) a detailed test procedure to verify each failure mode identified in the QFA), at new construction or when new machinery is installed or substantially upgraded. Please provide an MSC accepted and reviewed testing protocol QFA and DVTP for the new engines, to the attending USCG marine inspector. 46 CFR 184.620 (c).

Vessel must undergo an annual inspection within 3 months before or after each anniversary date of COI. Vessel exceeded 3 month interval on 13JUN21. Vessel is required to complete a satisfactory inspection prior to carrying passengers.

Where fire systems or equipment are not required, but installed, the system or equipment and its installation shall meet the requirements of this part. The vessel stbd engine has an electric heat detection device that is missing with dead end wires exposed. Vessel need to either install a functional heat detection device and prove to an attending CG marine inspector their ability to detect heat, or remove the heat detection system entirely from both port and stbd engines.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspections. The vessels stbd engine inboard side has two dead end wires exposed near top of the engine.

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other than small buoyancy compartments, under all practical conditions. The vessels bilge manifold valves were found to locked in the closed position and the master did not have the code to the combo lock preventing the vessel from opening the manifold and giving the crew the ability to dewater compartments in a timely manner.

The fixed power bilge pump shall be self-priming and may be driven off the main engine or other source of power. It shall be permanently connected to the bilge main and may also be connected to the fire main. If of sufficient capacity the power bilge pump may also serve as a fire pump. The vessel was unable to pull suction from the lazarette and prove proper function to the attending Coast Guard Marine Inspector.

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 150F must be suitably insulated where necessary to prevent injuries. The vessel has exposed exhaust piping in the engine room on the following: 1. stbd generator at the elbow 2. stbd main engine inboard at the "Y" 3. port main engine inboard forward bank at the "Y"

All wiring and cables shall be suitable for marine service and for the particular installation used. The general alarm bell in the pilot house wiring was pulled out of the crimp and exposed.

Lighting fixtures shall be constructed in accordance with the requirements of Underwriters' Laboratories, Inc. Standards for Marine Type Electric Lighting Fixtures, Subject 595, or shall bear the proper Underwriters' Laboratories, Inc. "Marine Type" label. The vessel stbd aft engine room light fixture is broken.

On a vessel of more than 65 feet in length, the public address system must be a fixed installation and be audible during normal conditions throughout the accommodation spaces and all other spaces normally manned by crew members. The vessels PA system was intermittent and could not be proven functional to attending Coast Guard Marine Inspectors.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the installation date on which the unit was installed. The vessels EPIRB hydrostatic release unit showed a three year expiration date and they could not provide paperwork to prove installation date.

Fuel filters fitted with bowls of other than steel construction must be USCG approved and be protected from mechanical damage. Portside main diesel engine was missing a flame shield on the fuel filter. 46 CFR 182.455(b)(6)

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspections. The starboard engine room electrical panel was missing blank circuit. 46 CFR 183.01-15

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspections. Inboard starboard electrical panel contained holes in the bottom. Plugs to be provided to fill holes. 46 CFR 183.01-15

Unless a splice is made by an insulted pressure wire connector listed by Underwriters Lab, Inc., it shall be thoroughly soldered, taped first with rubber and then with friction tape. Battery charger in engine room has twist on type wire nuts on splice. 46 CFR 183.05-50(b)(2)

Wiring joints and splices shall be mechanically secure and made in junction boxes. Battery charger in engine room had dead end wires. Tool room contained dead end wire from light fixture. 46 CFR 183.05-50(b)

Where liquid level indicating gages are installed, they shall be of heat resistant material. All sight glass tube gages must have documentation providing current tubes are heat resistance or required to change out with heat resistant material. 46 CFR 182.20-25(a)(5)

Each fuel tank shall be fitted with a vent pipe which shall be connected to the tank at its highest point under normal operating conditions. The discharge ends of vent pipes shall terminate on the hull exteriors as high above the waterline as practicable and remote from any hull openings. Portside generator day tank vent pipe, vented directly into tool room. Provide plan to vent day tank in accordance with 46 CFR 182.20-35. 46 CFR 182.20-35(a

Rails or equivalent protection must be installed near the periphery of all decks to a vessel accessible to passenger or crew. Found stern periphery to be missing protection. Provide railings or equivalent protection at stern.

Vessels > 40 ft and Ocean going must have a waste management plan. Found no waste management plan for vessel. Provide a plan to satisfaction of MI.

Each lighting fixture globe, lens, or diffuser must have a guard. Light bulb in E/R missing guard. Proved guard for light bulbs in engine room.

Prove proper operation/weathertightness of forward berthing emergency escape hatch.

Provide proof of company enrollment in drug and alcohol testing consortium.

Provide proof that all ship's crew are enrolled in consortium.

Provide all required first aid kit contents per 46 CFR160.041

Provide adequate construction for the seven bunks in the galley space, protection of crew not adequate, limited to 20 overnight accommodations.

Provide light guards/covers for all bunk spaces.

Grind down two (02) pinholes in fire main piping on discharge line in engine room, then provide permanent weld repairs.

Provide watertight plug or sealant to ring buoy, starboard side.

Identify oil leak on port engine, remove all oil beneath.

Renew wasted/leaking F/O piping in engine room.

Repair lagging on engine exhaust in store room.

Provide plans for approval of the MSD installation.

Crop and renew wasted decking under galley above store room.
