A vessel at anchor shall exhibit where it can best be seen: an all-round white light. MI observed the all-round anchor light was inoperable. Until rectified, vessel is restricted to daylight operations only and limited to minimum visibility of 2 Nautical Miles. 33CFR83.30 705c

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers the minimum capacity of the fire pump must be 50 gallons per minute at a pressure of not less than 60 psi at the pump outlet. The pump outlet must be fitted with a pressure gauge. MI observed that the pressure gauge fitted to the fire pump was reading inaccurately. Replace and calibrate. 46CFR181.300 (b) 16c

Gears, couplings, flywheels, and all rotating machinery capable of injuring personnel must be provided with adequate covers or guards. MI observed that protective guards were missing around the fire pump. 46CFR182.100 46CFR58.01-20 16c

STBD fuel emergency shut off valve did not fully close when pulled. This was adjusted and working properly before the inspection was complete. 46 CFR 182.455

Life Jackets were found with floatation foam deterioration. These were destroyed in MIs presence. New life jackets were purchased and labeled prior to inspection completion. 46 CFR 180.71

Damaged extension cord was found powering a drink dispensing machine. Remove extension cord and install an outlet to power the dispenser. 46 CFR 183.340

At each inspection the vessel shall be ready for machinery inspections to include operation of the main propulsion machinery both ahead and astern. The propulsion running gear was damaged when vessel grounded. Assess damage and make repair proposal to the [name withheld] must be verified and an operational test completed to the Coast Guards satisfaction, prior to carrying passengers. 46 CFR 176.804

A vessel with more than 1 deck above the bulkhead deck must have hull markings IAW 46 CFR 185.30-3(b). The vessel is missing the required hull markings matching the dimensions listed on the stability letter. 46 CFR 185.30-3(b)

Penetrations and openings in watertight bulkheads must have means to make them watertight. MI observed 3 holed cable penetrations at the top of the watertight bulkhead at frame 22. Penetration is not watertight. 46 CFR 171.114(a)(2)

Where a flexible fill pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat. and vibration, may be used. Such hose shall overlap metallic pipe ends at least 1 and 1/2 times the pipe diameter and shall be secured at each end by two clamps of corrosion resistant metal. The flexible section sha11 be accessible and as near the upper end of the filling pipe as practicable. When the flexible section is a nonconductor of electricity, the metallic sections of the filling pipe separated thereby shall be joined by a conductor for protection against static. The conductor is missing in the flexible section between the metallic sections of the filling pipe. 46 CFR 182.20-30 (d)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] construction plans for the vessel require a secondary overhead means of escape from the spaces below the main deck. The emergency escape hatch is located between frame 12 and frames 13, forward of the engine room on the starboard side. The emergency escape hatch was found blocked above by the carpet and table installation on the main deck and is not properly labeled as an emergency exit. 46 CFR 176.700(a)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Vessel was involved in a collision. Must undergo appropriate inspection by USCG marine inspector. 46 CFR 177.100

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the [name withheld] weld found at starboard longitudinal frame #1 in compartment number one. Make satisfactory repairs and match weld profile to existing welds on frames in this space. 46 CFR 176.700

All electrical installations shall be to the satisfaction of the OCMI. Identified a 110V electrical outlet installed in the air duct on #2 deck without electrical plan approval and temporarily mounted. Multi-outlet adapters (power strips) may not be connected to other adapters (“daisy-chained”), or otherwise used in a manner that could overload the capacity of a receptacle. Identified 15 surge protective devices daisy chained to an electrical outlet on deck 2 in the vicinity of the "DJ" station. The operator removed the surge protector daisy chain and the unapproved 110V electrical outlet in the air duct. 46 CFR 183.01-15

All electrical equipment and circuits must be clearly marked and identified. 46CFR183.220(d)

Cap dead end wires in engine room and have wires terminate in a junction box. 46CFR183.200(b)

The cognizant OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. Repair or replace rescue platform prior to carrying passengers. IAW: 46CFR180.210 (b) (2)

The material specification shall show that the rigid nonmetallic material possesses characteristics adequate for it intended service and environment and shall be approved for use by the Officer in Charge, Marine Inspection. Replace starboard engine lube oil hose. IAW: 46cfr 182.40-1

All wiring and cables shall be suitable for marine service and for the particular installation use. MI found unsafe splices throughout vessel, specifically in galley and 03 deck. Repair/replace were necessary. IAW: 46 CFR 183.10-20 (a)

Auxiliary machinery shall be suitable in type and design intended. MI observed excessive crankcase exhaust in engine room due to improper installation. Provide repair plan for generator crankcase exhaust. IAW: 46 CFR 182.10-1

Each collision bulkhead required by §179.210, must be constructed in accordance with §179.320, except that a collision bulkhead, must not be fitted with any type of penetration or opening except penetrations may be made if they are located as high and as far inboard as practicable and they have a means to make them watertight. MI observed wire running along overhead in forward peak compartment through collision bulkhead. Remove wire if not in use and repair penetration in bulkhead. IAW: 46CFR179.310 (a) (2)

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. MI observed oil spray accumulation in electrical outlets and unsatisfactory wire run supports in ER. Clean outlets & replace plastic wire supports w/ metallic. IAW: 46CFR183.01-15 (a)

The portable hand bilge pump shall be provided with suitable hoses for pumping bilges on the suction and discharge end. MI observed that hand pump located in engine room did not have adequate hose length to de-water all vessel compartments. IAW: 46CFR182.25-10 (e)

Each water light must be of an approved automatic electric type constructed in accordance with subpart 161.010 of this chapter. MI observed ring life buoy water light to be inadequate in operation, replace. IAW: 46CFR180.30-1 (b) IAW:

Remove flex hose connection on bilge piping servicing forward compartment and replace as original. 46 CFR 182.40-5 (Old T)

Provide repair proposal addressing use of non-metallic piping materials penetrating MTWB at frames 10, 16, and 22. 46 CFR 182.40-1

Complete all drills with crewmembers IAW 46 CFR 185.25-10

Pitting and holes located in suction end of fire main piping. Must repair prior to carrying passengers IAW 46 CFR 181.10-1.

Replace gasket in fwd watertight door IAW 46 CFR 170.270(b).

Properly support electrical cable every 24" with metal supports as per 46 CFR 183.340 (b)(4).
