Each towing vessel must have either a self-priming, power-driven, fixed fire pump; or a portable pump, and hoses and nozzles. Vessel did not have the required fire hose on board and took approximately 10 minutes to start the portable fire pump.

Each towing vessel must carry at least one appropriately-sized lifejacket for each person on board. The vessel is certificated to carry four persons and only two lifejackets were found to be on board.

A towing vessel of at least 26 feet, but less than 79 feet, in length must carry a minimum of two lifebuoys (at least 24 inches in diameter) located on opposite sides of the vessel where personnel are normally present. At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light. The vessel was found to be missing one lifebuoy with water light.

Each towing vessel must be maintained and operated so as to minimize fire hazards. Vessel has excessive accumulation of grease in the ventilation fan above the galley space.

Each cable and wire run must be installed in a manner to prevent contact with personnel, mechanical hazards, and leaking fluids. Vessel had exposed wiring on deck and in the steering compartment. Additionally, the lighting fixture in the steering compartment was not properly mounted and supported.

Electrical conductors passing through watertight bulkheads must be installed so that the bulkhead remains watertight. The bulkhead between the engine space and lazarette was found to have four cable penetrations not insulated to maintain watertight integrity.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Vessel was unable to produce a valid enrollment letter for an approved chemical testing company or provide evidence that all of the vessels crew was enrolled in an approved chemical testing program.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating condition. The vessel’s first aid kit was found to have multiple expired items.

Global Marine Transportation/Maxum Marine has failed to meet the requirement set forth in 46 CFR 136.202(a)(2) that states "All owners or managing operators of more than one existing towing vessel required to have a COI by this subchapter must ensure that each existing towing vessel under their ownership or control is issued a valid COI according to the following schedule: By July 20, 2020, at least 50 percent of the towing vessels must have valid COI's on board". Company operates three Subchapter M applicable towing vessels. As of July 20, 2020 only one of the vessels had received a COI. Therefore, company compliance is at 33.33 percent which is below the 50 percent requirement. Sector LA-LB has imposed a CG835V, Code 705c prohibiting the towing vessel ELSA (O/N: 269483) from engaging in commercial operations until a Certificate of Inspection is obtained. 46 CFR 136.202

Eliminating all extension cords and non-approved cable, using marine grade approved boat cable, permanently wire all permanently installed equipment IAW 46 CFR Part 183. For example: the air compressor.

Provide metal wire hangars on cable runs throughout the vessel IAW 46 CFR Part 183. Many runs in engineroom are lacking.

Provide overcurrent protection, at engineroom batteries for continuously operated "house" circuits IAW 46 CFR Part 183.

Provide readily accessible access to lifefloat IAW 46 CFR 180.130. The present installation, on top of the pilot house, is too difficult to climb up to, even at the pier. A possible repair is to install a ladder for climbing up.

Conduct hull exam prior to carrying passengers or cargo for hire.

Provide a MF radio and updated FCC Safety Radio Certificate. Vessel is restricted to 20 nm when carrying passengers.

Provide a medium frequency radio and updated FCC safety radio certificate.

Repair or replace deteriorated 10 man lifefloat.

Renew expired hydrostatic release for EPIRB.
