A submersible electric bilge pump may be used as a power bilge pump and shall be permanently mounted. Forward engine room submersible electrical bilge pump was found not to be secured to the hull structure bottom. 46 CFR 180.520(e), Code 16C.

Each life jacket carried carried on board must be marked with vessel name. Life jackets had an easily peel-off vessel name sticker and not permanent to the jacket. 46 CFR 180.71(e) 10C.

A vessel of more than 26 feet in length, but not more than 65 feet, must carry a minimum of one life buoy of not less than 24 inches in diameter. Lifering color was found faded. 46 CFR 180.70 Code 16C

A vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible, extracts for the areas to be transited may be provided instead of the complete publication. Nautical chart was not up-to-date and dated YR1998. 46 CFR 184.420

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure and personal injury. The following shall be corrected: a. steering water intrusion at the rudder post assembly; b. remove oily absorbent materials found in bilges; c. correct raw cooling water leak at the water pump and fire pump; d. correct excessive diesel engine oil leaks as indicated and identified during the exam. 46 CFR 182.100, Code 16C

A hatch exposed to the weather must be watertight and have securing devices. Steering compartment hatch is not watertight and needs to be secured to the vessel. 46 CFR 179.360, COde 16C.

Provision must be made for closing all air supply for a space protected by a fixed gas extinguishing system. All three engine compartment hatch covers were found not to be airtight and with holes, allowing air to enter the space if Fire-Boy fire suppression system is activated allowing CO2 to escape the protected area, in an enclosed passenger accommodation area. 46 CFR 182.465 (h) Code 16C.

Electrical installation including, but not limited to: (i) Elementary one-line diagram of the power system; (ii) Cable lists; (iii) Bills of materials; (vi) Power, lighting, and interior communication panel boards with number of circuits and rating of energy consuming devices; (vii) Type of capacity of storage batteries; (ix) Electrical plant load analysis. Vessel does not have a current one line diagram and over the years has had much work done without verifying gauge of wiring installed or removing unused or extra wiring. Operator shall identify all repairs and alterations work conducted without USCG approval and identifying the items listed. Submit an electrical One-Line Diagram to the USCG Marine Safety Center no later than 31JULY2024. Once approved, then conduct all necessary repairs according with such plan and 46 CFR 111 and 183. 46 CFR 177.202 COde 705C due in 31JULY2024.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel operator reported high engine temperature and heat exchanger not operating as intended. Vessel operator required to submit engine diagnostic report and repair proposal prior to beginning work. Vessel must conduct sea trial to the satisfaction of the attending marine inspector prior to operating commercially. 46 CFR 182.200(a)

No marine employer shall engage or employ any individual to serve as a crew member unless the individual passes a chemical test for dangerous drugs for that employer. Captain could not present valid evidence of completing preemployment drug testing.

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Water light battery was not of the approved type and did not operate as designed.

Each ring life buoy on a vessel must:(1) Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Ring life buoy grab line is deteriorated and requires renewal.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel experienced excessive water intrusion through rudder stock and shaft penetration when in operation triggering electronic bilge pump aft of main engine during entirety of underway drills. Operator to assess cause of excessive intrusion and prepare vessel for sea trial prior to operating.

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). A second power pump is an acceptable alternative to a hand pump if it is supplied by a source of power independent of the first power bilge pump. Vessel has no hand pump.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Drills not regularly logged or conducted.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: (ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Inspect portable extinguishers monthly and log in log book.

A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Installed filter had particulate matter in bottom and was black in color. Conduct proper maintenance and replace parts as needed. Not operating as intended.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. Fuel tank did not have flame screen.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Placard posted does not have information specific to the vessel for master to utilize in event of emergency.

Vessel reporting vibrations in the shaft and damaged bearings.

A vessel must have a reliable means for shutting down a propulsion engine at the main pilothouse control station. The engine stop button at the operating station was not capable of stopping the engine due to a worn spring within the governor.

At each inspection, the bilges and other spaces may be examined to see there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping system, or block escapes. Escessive oil was observed in the bilge and must be removed.

Main engine was found leaking cooling water, further inspection is necessary to identify cause. Repair and conduct sea trial to the satisfaction of the attending marine inspector.

While testing the fire pump, the fire hose began leaking water between the coupling and outer jacket. Replace fire hose.

The master shall conduct sufficient drills and give sufficient instructions to make sure all crew members are familiar with their duties during emergencies. No drill has been logged since May 21, 2021. Sufficient drills and training for emergencies shall be logged and available for review by the Coast Guard.

Vessel must be equipped with an adult size life jacket for each person authorized. Vessel must also be equipped with child size life jackets equal to at least 10% of the maximum number of passengers permitted. All Life jackets found to be worn. Replace all required life jackets (28) adult and (03)child

The fire pump outlet must be fitted with a pressure gauge. Fire pump outlet gauge was found to be cracked and inoperable. Requires replacement.

All cable and wire must be not be installed with sharp bends or supported with plastic tie wraps. repair/replace wiring located in aft steering compartment installed with sharp bends and supported by plastic tie wraps.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. 05 life jackets determined not to be in serviceable condition were replaced on the spot.

Replace the wasted exhaust bracket in the engine room.

Address minor wood rot in various areas of engine room.

Add securing bolts in two support columns.

Replace 2" through hull ball valve for the engine cooling water supply as the current one is hard to operate.

Replace the lettering for the fire hose valve as letters are falling off the bulkhead.

Relocate the terminal block near the Fireboy system that is currently low in the bilge to a higher, drier, location.

Replace the highly corroded electrical component located near the batteries.

Determine if the grounding strip aft of the engine is necesary, if it is, repair or replace it.

Prove proper operation of pre-engineered fire extinguishing system emergency overide.

Replace valve opening/closing lever on raw water intake.

replace oil filter with suitable metal marine type strainer and filter
