An inflatable liferaft or inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant for the particular brand, and in accordance with servicing procedures meeting the requirements of part 160, subpart 160.151, of this chapter—(1) No later than the month and year on its servicing sticker affixed under 46 CFR 160.151-57(n). Liferaft servicing was expired at the time of inspection. Service life rafts at an approved facility and provide evidence to attending marine inspector.

All unfired pressure vessels must be installed to the satisfaction of the cognizant OCMI. The design, construction, and original testing of such unfired pressure vessels must meet the applicable requirements of subchapter F (Marine Engineering) of this chapter. STBD UPV pressure relief valve shows corrosion and did not open at design pressure relief. Replace PRV and provide test to attending marine inspector.

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel missing stern name and hailing port. Mark name and hailing port vessel stern.

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the structure, machinery, equipment, and on a sailing vessel, rigging and sails. The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. In addition, the OCMI may require the vessel to get underway. At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. At time of inspection a crew was not provided. Prior to carrying passengers or cargo, provide properly licensed crew who are enrolled in company's drug testing program, and complete fire, man overboard, and abandon ship drills to the satisfaction of the attending USCG Marine Inspector. 46 CFR 176.404

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel did not install an interconnected fire detection system by 27DEC2022. Vessel is restricted to a LIMITED COASTWISE route until it's compliance with the interconnected fire detection requirements of 46 CFR 181.405 have been verified by a USCG marine inspector.

The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. At each initial and subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions to be specified by the inspector. At time of inspection a crew was not provided. Prior to carrying passengers or cargo, provide properly licensed and trained crew who are enrolled in company's drug testing program, and complete fire, man overboard and abandon ship drills to the satisfaction of the attending USCG Marine Inspector.

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR Part 80. One of the VHDF-DSC radios missing MMSI number.

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Records of EPIRB test were not available.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector.

Each pressure type wire connector and lug must meet UL 486A (incorporated by reference; see 46 CFR 175.600) or other standard specified by the Commandant. The use of twist-on type wire nuts is permitted under the following conditions: (1) The connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. The MSD discharge pump was found with twist type connectors and not in an enclosure.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. During inspection the following items were found requiring repair. Repairs must be made prior to credit for next scheduled dry docking. 1) Cracks were found in the engine room outboard of the starboard main engine in way of the main deck connection to the hull. 2) Cracks were found in the lazarette access hatch installation ring. 3) The access hatch to the rig water tank in the forward lower bilge was found with missing bolts. Provide bolts to ensure watertight seal.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate protection against hazards that might endanger the vessel and the persons being transported. Deficiencies 2-27 provide objective evidence that the vessel is substandard and not pro-actively managing technical and operational requirements. Vessel must complete a satisfactory COI re-inspection prior to any movement.

All vessels required to be provided with a power-driven fire pump shall also be provided with a fire main system including fire main, hydrants, hose and nozzles. Vessel's fire main was found wasted and holed at the port side riser to the main deck. The wasted and holed condition prevented fire main pressure at either of the two provided fire hydrants. Repair wasted fire main piping to the satisfaction of the attending USCG marine inspector and demonstrate proper operation of fire main system. 46 CFR 181.15-1 (existing vessel) Amended from Code 30 to Code 16 10NOV21: Vessel repaired wasted fire main and demonstrated operation, however port side riser was found leaking at the threaded connection to the deck penetration. Repair and pressure test to satisfaction of attending USCG marine inspector.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics. Vessel had no records of training or drills conducted after June of 2021. Provide crew training and drills and associated log entries prior to departure.

Vessels over 65 feet in length shall be provided with two fixed power bilge pumps. Vessel's dedicated electric power bilge pump was inoperable. Vessel could not demonstrate operation of secondary bilge pump (cross-connected fire pump) due to concerns about discharging oil in the bilge lines. Repair inoperable bilge pump and demonstrate proper operation of both power bilge pumps to the satisfaction of attending USCG marine inspector. 46 CFR 182.25-10(a) (existing vessel)

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's trim and stability book, stability letter, Certificate of Inspection, and Load Line Certificate, as the case may be. The vessel may not depart until it is in compliance with these requirements. Four feet of water was found in the forepeak space and the crew was unaware of how or why it was there. Dewater forepeak space and make available for internal inspection by attending USCG marien inspector prior to departure.

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's trim and stability book, stability letter, Certificate of Inspection, and Load Line Certificate, as the case may be. The vessel may not depart until it is in compliance with these requirements. Approximately one foot of water was found in the unused ballast tank located in the bilge below the forward passenger compartment, along with an electric sump pump and hose. The crew was unable to explain how or why the water was in the ballast tank. Dewater ballast tank and ensure no water ingress to tank prior to departure.

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel except that a float-free link is not required if the vessel operates only on waters not as deep as the length of the painter and stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. The starboard life raft painter was not attached to the weak link, which would not inflate the raft automatically. The vessel operator representative stated that the raft had been that way since installation by a servicing company in July (three months). Vessel representative attached painter to weak link during inspection.

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Public address system was not adequate in accommodation spaces and forward passenger area for crew. Make public address system adequate to attending Marine Inspector.

A vessel of more than 12 meters in length shall be provided with a whistle and a bell. Vessel's whistle was inoperable. Demonstrate proper operation prior to departure.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Dead-end wire found under operating station. Remove/make wire in compliance with regulations to satisfaction of attending Marine Inspector.

Each ring life buoy must be marked with Type II retro-reflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Retro-reflective material was missing on aft deck ring life buoy. Repair/replace retro-reflective tape to satisfaction of attending Marine Inspector.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. (b) Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in § 185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessel's emergency instructions were not vessel specific and did not accurately describe the equipment and procedures for the vessel. Amend and display emergency procedures applicable to vessel. 10NOV21: During deficiency check vessel presented revised procedures which still contained information not applicable to the vessel. Provide vessel specific procedures.

The master of the vessel shall ensure that each EPIRB, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; and that the EPIRB test shall be logged or otherwise documented, as applicable. Vessel was not conducting EPIRB test or recording testing. Begin recording and testing EPIRB to the satisfaction of attending Marine Inspector. Vessel tested EPRIB successfully during inspection.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. First aid kit contents expired. Replace required contents of first aid kit with unexpired items.

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the inspection of each portable and semi-portable fire extinguisher. The B-V semi-portable fire extinguisher was not serviced as per NFPA 10. Provide proof of servicing to attending Marine Inspector.

Fire extinguishing equipment installed on a vessel in excess of the requirements of §§ 181.400 and 181.500 must be designed, constructed, installed, and maintained in accordance with a recognized industry standard acceptable to the Commandant (CG-ENG-4). Independent modular smoke detector located in galley space was found to be inoperable. Provide operable smoke detector.

The owner, charterer, managing operator, or master of a vessel making a coastwise voyage where passengers embark or disembark from the vessel to another vessel or port other than at the port of origin must keep a correct list of the names of all persons that embark on and disembark from the vessel. Vessel does not keep a correct list of the names of all persons that embark on and disembark from the vessel. Moving forward the vessel shall see that the list is prepared prior to departing on a voyage. The list must be communicated verbally or in writing ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. The crew and passenger list shall be available to the Coast Guard upon request.

Openings through which conductors enter ahall be adequately closed. Armored cable entering the electrical outlet box on the port side of the lazarette was pulling out and not adequately closed. Provide complete closure for cable entrance. 46 CFR 183.10-21(f) (existing vessel)

Except as otherwise provided all electrical equipment shall be dripproof protected. Electrical outlet box on port side of lazarette was found with a broken outlet cover. Replace outlet box or repair cover. 46 CFR 183.10-10 (existing vessel)

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Emergency lighting throughout egress route was not operable. Repair/replace to satisfaction of attending Marine Inspector. 46 CFR 184.30-5 (existing vessel) 10NOV2021: Vessel repaired and demonstrated installed emergency lighting, but was missing emergency lighting in passageway between the watertight doors forward of the engine space. Provide additional lighting.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Vessel's engine room bilge was found with approximately five inches of oily water mixture. Remove oil and water from bilge.

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR Part 80. The Bridge-to-Bridge Act applies to every power-driven vessel of 20 meters or over in length while navigating in the navigable waters of the United States. Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Vessel does not have a certificate with valid endorsement. Vessel must obtain valid certificate to come into compliance with regulatory requirement and provide objective evidence to attending Marine Inspector. 10NOV2021: Vessel awaiting attendance by FCC licensed inspector for certificate issuance.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The potable water pressure tank was found not permanently installed and secured only with manila line. Adequately secure potable water pressure tank.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Records were not available showing monthly inspection of portable adn semi-portable fire extinguishers.

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's trim and stability book, stability letter, Certificate of Inspection, and Load Line Certificate, as the case may be. The vessel may not depart until it is in compliance with these requirements. Vessels stability letter requires that any openings that could allow water to enter the hull or deckhouse should be kept closed when rough weather or sea conditions exist or are anticipated. Non-return ball worn on port side lube oil tank vent. Replace ball or provide other closure device.

An inflatable liferaft must be serviced in accordance with servicing procedures meeting the requirements of 46 CFR 160 no later than the month and year on its servicing sticker affixed under 46 CFR 160.151-57( n ). The affixed inspection sticker information required by 46 CFR 160.151-57 ( m ) ( 3 ) ( ii ), and the date of servicing and repacking on the servicing facilities certificate issued in accordance with 46 CFR 160.151-57 ( p ) ( 3 ), did not match. Ensure liferaft is serviced in accordance with servicing procedures meeting the requirements of 46 CFR 160 and provide objective evidence showing this servicing to the attending Marine Inspector.
