The following emergency lighting and power loads must be arranged: Illuminated signs with the word "EXIT" in red letters throughout a passenger vessel so the direction of escape to the open deck is obvious from any portion of the vessel usually accessible to passengers and crew, except machinery spaces, and except stores and similar spaces where the crew are not normally employed. 10+ photo-luminescent "EXIT" signs throughout the vessel are marked "Replace before May 2025" on the manufacturer's label, and have lost their photo-luminescent properties. Replace all photo-luminescent signs that have passed their service/expiry date to the satisfaction of the Marine Inspector.

Auxiliary machinery vital to the main propulsion system must be provided in duplicate. Loss of #2 MDE caused #1 MDE to take full operational load do to faulty relay. Deficiency rectified.

Approved fire extinguishing systems must be installed, as required by Table 76.05-20. Vessel's spnkler system had 13 nozzles that were clogged and inoperable. 46CFR76.05-20

Vessel's must have a marine radar system for surface navigation. Vessel had inoperable Radars due to installation of new Radars. 33CFR164.35

#2 End Primary steering not responding via rudder angle indicator. The steering gear must be capable of moving the rudder from 35 degrees to 35 degrees at its deepest loadline draft and running at maximum ahead service speed, and from 35 on either side to 30 on the other in not more than 28 seconds under the same condition. 46 CFR 58.25-10(b)(2)

Operator reported an ABT breaker connected to the emergency generator which experienced a malfunction. Each bus meet the requirements of either Section 7.10 of IEEE or IEC 60092-307. Provide technician report to the USCG identifying the root cause of the failure and attesting to repairs.

Each passenger vesssel must carry the following: Launching appliances or marine evacuation systems equally distributed on each side of the vessel. Vessel self-reported that rescue boat winch had experienced a hydraulic leak and as such, vessel will be allowed to continue operations at half-passenger capacity until issue is rectified. -50 C , 46CFR199.201(b)

“All doors, except those that are normally kept closed, shall be of a type which are capable of release from the control station and from a position at the door.” 46 CFR 72.05-25(b)(9)(ii) -The vessel's fire screen door #2 would not automatically close upon release from the #1 end pilot-house. Provide proof of proper function of FSD to USCG.

All piping, valves and fittings shall meet the applicable requirements of subchapter F. During a routine fire drill, two holes were discovered in the fire main. Company made temporary repairs while a new pipe is fabricated. 46CFR76.10-15(a)

Make repairs to the emergency generator. Replace all deteriorated flexible hoses and repair the oil leak saturating both exhaust manifolds.

Repair all modifications found in way of multiple cables installed in a single cable transit penetrating the class "A" fire boundary between EOS and various adjacent spaces.

Update the fire control plan to reflect the stowage location of the fire out-fits.

Repair loose fan guard for the No.1 reduction gear standby pump.

Adjust the emergency fuel oil shut-off valve to provide full actuation of the valve and achieve positive closure.

NR2 rescue boat davit currently experiencing overload alarm during normal operation. Davit only operable through manual deployment mode. Repair and conduct satisfactory operational test of rescue boat deployment. 46 CFR 71.55-1 (a)

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. The accumulator on the #1 rescue davit is at low pressure and the automatic switch that turns on the pump to bring the pressure back up to the normal operating level is not working. Ensure pressure is maintained at the correct level until switch can be permanently repaired. Vessel is required to inform the Coast Guard upon repair of the switch. 46 CFR 199.190 (a)

General emergency alarm signal must: be located in passenger and crew areas where they can alert persons in spaces where those persons be maintaining, repairing, or operating equipment, stowing or drawing stores or equipment, or transiting such as public spaces, workshops, galleys, ect. The general is not audible in #2 reduction gear space and the #5 fire zone.

Each vessel must carry a lifejacket for each person onboard. 48 lifejackets were deteriorated and were removed the vessel.

Obtain the USCG Marine Safety Center's approval for the installed steering control system and prove proper installation to an attending Caost Guard Marine Inspector, no later than the vessel's next scheduled maintenance period (31MAR2020).

Propulsion control system would not respond in Pilot [name withheld] mode, only manual (Eng Room) controls. No alarm or indicator was given when steering controls were inoperable in PHC mode. Effect repairs to automated vital system, propulsion control system in all modes.

21MAR18, 1917: Received initial JHOC notification.
