Each vessel owner or operator subject to this part must ensure that copies of the following documents are carried on board the vessel and are made available to the Coast Guard upon request:(1) The approved Vessel Security Plan (VSP) and any approved revisions or amendments thereto, and a letter of approval from the Commanding Officer, Marine Safety Center (MSC). VSP was not on board the vessel.

The requirements and procedures each COFR Operator (as defined in § 138.30(b)) must use to establish and maintain the evidence of financial responsibility required by the OPA 90 and CERCLA (both defined in § 138.30), and to obtain Certificates of Financial Responsibility (COFR). COFR was found expired.

Each hose assembly used for transferring oil or hazardous material must meet the following requirements: Each hose must be marked with one of the following:(1) The name of each product for which the hose may be used;(f) Each hose also must be marked with the following, except that the information required by paragraphs (f)(2) and (3) of this section need not be marked on the hose if it is recorded in the hose records of the vessel or facility, and the hose is marked to identify it with that information:(1) Maximum allowable working pressure;(2) Date of manufacture; and(3) Date of the latest test required by 33 CFR 156.170. 3 inch diameter hose was missing markings.

Discharge piping of all tank vessels shall be tested at least once a year. Vessel cargo hose and hard pipe was last inspected 22Feb2018.

Above-deck cargo pipe and hose was last hydrostatically tested on 01/31/2017. Perform annual hydro test prior to conducting cargo operations.

Cargo pipe pressure gauges have not been calibrated within the past year. Calibrate gauges prior to conducting cargo operations.

Void 2 port hatch cover has severe corrosion on gasket channel. Replace cover.

Provide a means of draining or removing discharged oil or hazardous material from within the coamings on the tank tops. 33 CFR 155.310, prior to cargo ops.

Prove proper operation of tank barge liquid overfill protection (overfill alarms/shutdown).

Owner/Operator could not provide a list of SOPEP materials onboard the vessel.

Owner/Operator could not provide a vessel reponse plan onboard the vessel.

Ensure side light screens are of black matte color.

Prove proper operation of high level alarms (prior to carrying cargo)`

Existing fire extinguishers are not USCG approved; provide vessel with two USCG approved marine type B-II fire extinguishers.

Provide documentation attesting to the fact that the cargo pump relief valve functions satisfactorily at the pressure at which it is designed.

Provide documentation attesting to the satisfactory hydro test of the cargo piping.

Provide documentation attesting to the satisfactroy calibration of the cargo pump pressure gage.

Three areas of bilge knuckle wasted with pinholes.

PROVE PROPER OPERATION OF OVERFILL PROTECTION, ALARMS, AND CLOSED GAUGING SYSTEM IAW 46 CFR.

PROVIDE STUDS ON VAPOR COLLECTION CONNECTION IAW 46CFR AND REMOVE CAMLOCK FITTING FROM PORT VCS CONNECTION.

TEST & MARK CARGO PIPING IAW 33CFR 156.170

MAKE VAPOR RECOVERY PIPING SYSTEM AVAILABLE FOR QUARTERLY EXAM DUE TO THE CARRIAGE OF POLYMERIZING CARGOES.

MAKE PERMENANT REPAIRS TO THE CARGO TANK STEAM LINES AT THE CARGO TANK TOPS. STEAM LINES SHALL NOT BE USED UNTILL PERMENANT REPAIRS ARE MADE.

MAKE VESSEL AVAILABLE FOR AN INTERNAL STRUCTURAL EXAMINATION BY A COAST GUARD MARINE INSPECTOR.

PROVIDE EXPLOSION PROOF JUNCTION BOXES FOR ALL ELECTRICAL CONNECTIONS IN HAZARDOUS LOCATIONS. ELECTRICAL CABLE IS NOT TO BE USED FOR POWER UNTIL THIS ITEM IS CORRECTED.

RELOCATE THE SIDE LIGHT SCREENS TO OUTSIDE OF HAZARDOUS LOCATIONS.
