KON TIKI II

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: (a) Operation of the main propulsion machinery both ahead and astern. Vessel operator reported the KON TIKI II suffered a starbard main engine exhaust manifold and turbocharger failure rendering the starboard main engine inoperable. 46 CFR 115.804

Key personnel must complete training in 33CFR101.650(d)(2) by January 12, 2026. Company does not have a CYSO and all key personnel have not yet completed the training as of this inspection. 33 CFR 101.650(d)(4)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the tank compartment, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the compartment and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Electric solenoid valves must not be used, unless used in addition to the manual valve. The stbd engine fuel shutoff valves did not effectively close the valves (code 17). The shutoff valves were adjusted and functioned appropriately during inspection.

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level in each of the following normally unmanned spaces: 1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette;(2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and(3) A space with a non-watertight closure , such as a space with a non-watertight hatch on the main deck. The port engine room high level alarm was inoperable (code 17). The high level alarm was replaced and functioned appropriately.

Notwithstanding any other provision of this subpart, except as provided in paragraph (b) of this section, a Certificate of Documentation but no trade endorsement thereon, issued to a vessel which is the subject of an outstanding mortgage filed or recorded in accordance with subpart Q of this part or any predecessor regulations, remains valid for purposes of:(1) 46 U.S.C. chapter 125;(2) 46 U.S.C. chapter 313 for an instrument filed or recorded before the date of invalidation, and an assignment or a notice of claim of lien filed after that date;(3) 46 U.S.C. 56101, 56102 and 57109; and(4) 46 U.S.C. 56301. The vessel possess a valid COD, but it was not onboard the vessel. The COD was printed and placed onboard.

Each vessel owner or operator subject to this part must ensure that copies of the following documents are carried on board the vessel and are made available to the Coast Guard upon request:(1) The approved Vessel Security Plan (VSP) and any approved revisions or amendments thereto, and a letter of approval from the Commanding Officer, Marine Safety Center (MSC). The vessel possess a valid VSP letter of approval, but it was not onboard the vessel.

Each garbage management plan under paragraph (a) of this section must be in writing and—(1) Provide for the discharge of garbage by means that meet Annex V of MARPOL, the Act, and §§ 151.51 through 151.77;(2) Describe procedures for minimizing, collecting, processing, storing, and discharging garbage; and(3) Designate the person who is in charge of carrying out the plan. The vessel possess a waste management plan template, but it was not filled out.

If the marine employer conducts random drug testing through a consortium, the number of crewmembers to be tested may be calculated for each individual marine employer or may be based on the total number of covered crewmembers covered by the consortium who are subject to random drug testing at the same minimum annual percentage rate under this part or any DOT drug testing rule. Documentation showing which employees are a part of the consortium was not present on the vessel.

Each life jacket carried on board the vessel must be marked in accordance with § 122.604 of this chapter. Numerous life jackets have inoperable retro reflective tape. Inspect all life jackets and replace retro reflective tape as needed.

Electrical equipment on a vessel must be installed and maintained to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock;(c) Minimize accidental personnel contact with energized parts. Unconnected and unprotected wires were present at the helm and hot water heater. There is an opening in the main circuit breaker panel. There are opening in the storage room electrical outlet and at the galley stbd electrical outlet. These issued were corrected.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Crewmember punctured port engine room hull plating during maintenance corrosion removal creating a quarter-sized hole in the bottom plating and water intrusion. Temporary repair was made and haulted water intrusion. Provide hull gauging report for the port engine room bottom plating and submit with repair proposal to USCG for review and plan approval prior to making permanent repairs. 46 CFR 176.700

Vessel suffered a water pump failure on the starboard main engine. Repair or replace in-kind and conduct satisfactory sea trials in the presence of USCG marine inspectors. 46 CFR 176.804

The installation of fuel lines, including pipe, tube, and hose, must comply with the requirements of this paragraph. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Stbd fuel tanks remote shutoff valve did not operate from outside the space, make necessary corrections. 46 CFR 119.455(b)(3)

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operations. Fuel shutoff stations markings were not as required, make necessary corrections. 46 CFR 122.608

The installation of fuel lines, including pipe, tube, and hose, must comply with the requirements of this paragraph Fuel lines must be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration. Stbd fuel line between tank and bulkhead had small fuel leak. Make necessary corrections. 46 CFR 119.455(b)(2)

The Certificate of Inspection and Stability Letter must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. COI and Stability Letter were not posted as required. Post both as required. 46 CFR 115.302 46 CFR 115.306

(b) A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Label for bilge alarm was missing at operating station for stbd aft compartment. Label alarm as required. 46 CFR 119.530(a),(b)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Marine Navigation Lights,” or other standard specified by the Commandant. Nav light missing bulb; Replace bulb as required. 46 CFR 120.420

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. MI's observed a AGM battery supplying the rum punch machine behind the bar not affixed in a suitable container. 46CFR 120.220 (a)

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. AGM battery supplying rum punch machine was found to be utilizing pressure clamp type connections. 46CFR 120.350 (d)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. MI's observed excessive corrosion at fire hose connection to the fire main on the main deck. Vessel must disconnect hoses from fire main and lubricate to allow for quick disconnection for hose transfer in the event of an emergency. 46CFR 115.830(a)

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. MI's observed a 120v electrical outlet not properly affixed and without a cover in the bar storage area on the starboard side. Vessel must properly install the outlet. 46CFR 120.220(a)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI's observed a fluorescent light fixture in the bar area without a cover. 46CFR 120.200(b)

Electrical equipment used in the following locations must be dripproof; A location normally exposed to splashing, water washdown, or other wet conditions within a galley, a laundry, or a public washroom or toilet room that has a bath or shower. MI's observed non-dripproof connections in the electrical locker that has open communication to sea from below the locker. 46CFR 120.210(a)(2)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI's observed an upper deck drainage pipe that runs through the main deck electrical closet. As a best practice this configuration shall be changed to re-route the plumbing in a manner that would protect the electrical components from failure in the event of a leak. 46CFR 120.200(b)

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the cognizant [name withheld] fire extinguisher behind bar was not permanently mounted. Vessel must suitably mount extinguisher in the appropriate location. 46CFR 118.520

Electrical equipment used in the following locations must be dripproof; A location normally exposed to splashing, water washdown, or other wet conditions within a galley, a laundry, or a public washroom or toilet room that has a bath or shower. Electrical closet in galley had loose wiring penetrating a non-dripproof electrical box. Vessel must make connections dripproof 46CFR 120.210(a)(2)

During the test of the port engine room fire boy suppression system the engine shut down as required but failed to provide an audible alarm at the operating station.

At the time of the inspection attending MI's noted that the bilge pump screens at each pick up point were removed. Restore to original configurations.

Attending MI's observed waste oil being retained in a cooler in the after most void on the port side. Properly dispose of oil and establish future plans to retain and dispose of generated oil on board the vessel.

A pair of vice grips was noted holding a sea strainer together in the starboard engine room. Restore to manufacture design for function.

At the time of the inspection no freshwater was available to test the bilge pump. Prior to operations with passenger prove satisfactory operations of the installed bilge pumping arrangements.

Attending MI's noted that excessive debris was observed in all void. Old parts, tarps and operational supplies should be secured proper as to not affect the emergency bilge pumping arrangements.

Vessel is restricted to the carriage of not more than one hundred and fifty (150) passengers for hire until a satisfactory MTSA Verification exam is done. Company must also complete satisfactory security drills and a company security exercise with vessel crews as well as show proof of compliance with security training and proficiency for the CSO and VSO.

Anchor light is missing and port nav light is inop.

Provide EPIRB or survival craft as required. Each vessel certificated to operate on a limited coastwise route within three miles of land in warm water, and is equipped with an FCC type accepted Category 1 406 MHz EPIRB installed to float free and automatically activate, is not required to carry survival craft.

Properly mark fire fighting system manual activation pulls and provide operating instructions. Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored.

Fix valve on stbd side raycor. There are vice grips as a handle.

Crew did not know that there is a manual pull for the preengineered firefighting system and how to operate it.

Make proper repairs to port side expansion tank. Currently repaired with JB Weld.

Provide hose clamps on Port & Stbd oil vapor canisters.

Both stern tubes to be gauged/closely inspected at the next drydock for wastage / possible replacement.

Prove proper operation of port and starboard bilge pumping system.

Re-position and prove proper operation of lazarette high bilge alarms.

Provide means of shutting down generators automatically when fixed fire systems are activated.

Remove flammable stores or meet requirements of 46CFR 116.600, 118.400 & 120.530.

Replace starboard fire-boy cylinder or provide documentation from service technician attending to the satisfactory condition of corrosion and pitting on cylinder.

Provide manufactures documentation to indicate proper installation of dripless shaft seals.

Crop and renew holed bottom plate in way of starboard bow rake.

Post COI, Stability Letter and Emergency Instructions.

Provide updated navigational charts.

Remove sharp edges on upper deck in the forward starboard rail.

Properly mount battery chargers and air compressor.

Properly mount and wire engine room fans.

Properly label emergency fuel shut-off valves.

Provide reserve power supply for vessel VHF radios.

Provide marine radar.

Prove proper operation of emergency fuel shut-offs.

Provide MARPOL V placard.
