Stability information (stability details indicated on stability letter), is required on certain vessels by paragraphs (b), (c), or (d) of this section. Enough stability information, including stability calculations and assumptions made to use them, must be provided to allow the master to be able to determine operating guidelines, loading restrictions, and ensure compliance with the applicable intact and damage stability regulations of this chapter. Vessel had a deployable sonar installation & doubler plate support bracket welded to side of the vessel & vessel is currently sitting with approx. 3.5-5 degree list to STBD. Conduct new Simplified Stability Test or provide documentation from Professional Engineer (PE) Or Naval Architect that vessel is still operating with in the constraints of their Stability Letter dated March 28, 2023.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available. Vessel only has 2 buckets.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Battery Expired on EPRIB.

Stowed so that when the vessel sinks the survival craft floats free. Vessel had ratchet strap over life float.

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. One Life Jacket was incorrectly labeled with wrong vessel name and missing light.

When the cognizant OCMI or the Commanding Officer, Marine Safety Center determines, in accordance with § 178.210, that a vessel must have stability details indicated on its Certificate of Inspection or a stability letter. Maximum weight of portable equipment permitted on the vessel. Vessel is restricted to no more then 3255 lbs of cargo & the VCG shall not exceed 32 inches until new stability test is conducted or sonar equipment is removed.

Starboard stern remote fuel shutoff valve did not close.

Generator did not automatically shutoff when pre engineered fixed fire extiguishing system was tested in the machinery space.

The cognizant OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. MIs have determined that the vessel does not contain the design or arrangement to effectively rescue a person from the water. 46 CFR 180.210(b)(2)

Operation of transmitters of any ship station must be performed by a person holding a commercial radio operator license or permit; Master did not have permit

A manned oceangoing vessel of 40 feet or more in length engaged in commerce must have a garbage management plan; vessel does not have plan

The marine employer must have a drug testing program in place; vessel unable to provide proof.

A vessel of more than 7.9 meters (26 feet) in length, but not more than 19.8 meters (65 feet), must carry a minimum of one life buoy of not less than 610 millimeters (24 inches) in diameter; vessel's ring buoy is deteriorated.

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant; vessel has several terminal boards under pilothouse w/ blade type fuses.

Port side engine lost lower crankshaft bearings. Repair vessel to operate condition.

Crew must conduct drills to the satisfaction of the attending Marine Inspector.

A pre-engineered fixed fire suppression system must automatically shut down engines when activated and have the capability to override and restart engines after activation; vessel’s Fireboy system did not shut down STBD Maine engine and was not capable of manual override.

Buoyant apparatus with plastic foam buoyancy must have a plastic foam body with an external protective covering. The external protective covering must be Elastomeric vinyl accepted by the Commandant. Covering found wasted and cracking and in need of repair as recommended by manufacturer.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load. Starboard shaft has an unacceptable wobble, indicating engine was possibly not properly aligned during haul-out.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Vessel held no valid Safety Radiotelephony Certificate.

Make repairs to bent/cracked hull plate extension IWO aft, port transom.

Demonstrate satisfactory operation of valves within 6" of waterline and below.

Crop and renew the hull plating identified below both shaft where pitting was observed, as discussed.

Re-attach the bonding wire to the top of the fuel tank.

Repair cracked aft hatch frame as discussed.

Repair cracked hatch framing for both engine compartment hatches, as discussed.

Remove rusted fitting observed outboard side of starboard ventilation and repair as discussed.

Remove junction box and associated wiring from port side raw water hose, or identify it's purpose and re-install.

Renew chaffed coolant water exhaust hose forward of starboard exhaust pipe.

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. Provide proof the name of the vessel is correctly marked as Gracie Jean.

Port Engine was disassembled due to overheating condition

Machinery, equipment, materials and arrangements not covered by standards in this sub-chapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. There were open boxes of paint cans in the forward compartment. Remove boxes.

The minimum number of portable fire extinguishers required on a vessel must be acceptable by the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required. The crew cabin does not have a fire extinguisher rated at 2-A. Place the proper extinguisher with bracket in the crew cabin.

A vessel on an Oceans route must be equipped with an electronic position fixing device, capable of providing accurate fixes for the area in which the vessel operates, to the satisfaction of the cognizant OCMI. The Single Sideband radio did not have the MMSI number installed. Install number into the single sideband.

A vessel not required to have a power driven fire pump must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. There was only one functional fire bucket onboard. Place two fire buckets on the vessel.

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission. The radio safety certificate was expired. Obtain valid certificate.

Each lifejacket kept in stowage must be readily available. The designated lifejacket stowage was filled with other items making the items not readily available.

Vessel must carry a first aid kit approved under 46 CFR 160.041 or equvalent. Vessel's first aid kit was expired.

The master shall conduct sufficient drills to make sure crew members are familiar with duties. USCG needs to witness crew drills.

An exposed hazard, such as rotating machinery, must be properly protected my a cover. The starboard main engine, secondary bilge suction belt/pulley has no guard.

the master and crew will be familiar and have mounted emergency instructions for FIRE/HEAVY WEATHER/MAN OVERBOARD. Emergency instructions not mounted.

A placard must be posted next to all radiotelephones with the emergency broadcast instructions and information, specific to the vessel. Vessel had no placard.

Crew training, drills, and EPIRB testing shall be logged. There is no Ship's Log on board.

Vessel must carry up to date charts, U.S. Coast Pilot, CG Light List, Tide Tables, and Current Tables. Vessel has no charts or pubs on board.

The employer shall provide an Employee Assistance Program (EAP) for all crewmembers. there is no EAP on board.

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these available to the USCG upon request. There were no records of chemical testing on board.
