In systems and applications where flexible hoses are permitted to be clamped, double hose clamping is required where practicable. Starboard main diesel engine raw water cooled exhaust had one broken hose clamp.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical panel in engine room was missing front panel cover.

Construction and arrangement of vessel must allow free and safe operation. MI found deck support frame that fell off.

At inspection for certification each vessel observed for unsafe practice. MI found combustible and gear stored on top of start batter bank for generator.

Each vessel must be equipped with at least two operable portable batter powered lights. one at operating stations and one at the access of E/R. MI found E/R portable light to be not operable.

Must be fitted with ground tackle and mooring lines necessary for vessel to safely anchor or moor. MI found anchor line splice frayed.

Portable watertight containers for distress flares shall be a bright color and clearly mark "DISTRESS SIGNALS". MI found no marking on container.

A vessel is inspected for compliance with the standards required by this subchapter. MI found terminal block in lazarette corroded and no mounted.

Twist on type connectors cannot be used for making joints in cables. MI found twist type connectors on light in bridge.

Valves for removing water or impurities drum diesel fuel traps or strainers are prohibited. MI found Fuel oil valve in E/R with no cap.

A visual and audible alarm must be provided at the operating station to indicate a high water level. No light was installed for the forepeak bilge high level alarm. Install light at operating station for forepeak high bilge alarm. 46 CFR 182.530(a)

Bunks where installed shall have a minimum length of 74" and a minimum width of 24". They may be constructed of wood or metal. Mattress is to be covered with material which has been treated to give it fire resistant properties and which will provide the mattress with a reasonably smooth surface free of sharp depressions. Arrangements shall not be more than 3 high, with a minimum distance of 24" vertical between bunks; ladders of steps shall be provided for each top bunk. Construction and arrangement shall allow free and unobstructed access to the bunks. Each bunk shall be immediately adjacent to an aisle leading to a means of escape from the lounge area. Vessel is equipped with 08 double bunks. Double bunks are not considered equipped for adequate egress as the outboard bunk is not immediately adjacent to an aisle leading to a means of escape. Vessel is restricted to 14 passengers in lower overnight accommodations. 46 CFR 177.30-7(b)

Re-fasten deck frames in forepeak (as indicated).

The fixed power bilge pump shall be self-priming and may be driven off the main engine or other source of power. Bilge pump would not prime due to wasted impeller.

Provision shall be made by means of plugs, covers, etc., to prevent the admission of air into the space protected. Found 01 inch diameter hole between engine room and accommodation space.

All electrical installations shall be to the satisfaction of the officer in charge, marine inspection. Air lines laying on electrical wire in fuel room. Electrical bundle laying in overhead tied to forward peak bilge line.

PROVIDE LOGS FOR LIFESAVING EQUIPMENT

PROVIDE MARKINGS FOR VESSEL. PLACE NAME OF VESSEL ON CLEARLY VISIBLE EXTERIOR PART OF THE PORT AND STARBOARD BOW AND STERN OF VESSEL. HAILING PORT OF VESSEL MUST BE MARKED ON SOME CLEARLY VISIBLE EXTERIOR PART OF THE STERN OF THE VESSEL.

PROVIDE UV PROTECTED OR DARK IN COLOR LINE FOR RING BUOY.

PROVIDE GUARD TO COVER PULLEY SYSTEM ON BOTH MAIN DIESEL ENGINES.

Post COI publicly under transparent medium

Post stability letter under transparent medium.

Provide hose clamps to bilge foot fittings in tank space, engine room, and lazarette.

Mark expiration date on hydrostatic release unit.

Log EPIRB testing monthly and lifesaving equipment maintenance.

Remove wasted pressure vessel in tank room.

Install check valves on bilge lines or to bunk room, tank room, and lazarette, or install stop check valves for each.

Make forward lazarette bulkhead watertight.

Provide electrical panel doors for starboard pqanel engine room and lazarette.

Install proper rail guard for stern embarkation access, with spacing no more than 09in from deck to first rail course, & 12in between courses above, able to withstand 200 point load. Iaw 46 CFR 177.35-1

Remove wasted FRP surrounding exhaust piping, port & stbd, lazarette compartment, and grind down to good metal, for further inspection of possible pinhole punctures.

Provide repair proposal for wood rot of wood support overhead of stbd quarter section of engine room.

Ensure watertight integrity of all bulkheads, concerning through-bulkhead penetrations.

Remove & renew wasted wood support surrounding stbd generator water intake through-hull valve.

Rule Bilge pump and associated wires were not properly secured.

Remove dead end wires in aft steering as indicated.

Properly secure/mount wires in AFT Steering as indicated.

Properly install shore tie cable to plug as indicated.

Prove proper operation of fixed bilge system in tank space (aft of E/R).

Provide vessel name on ring buoy.

Fuse connector under bridge was broken while doing repairs. Was being held together with alligator clips at time of inspection.

The label on the escape hatch was covered when a new floor was put in. Escape hatch needs to be labeled to prevent from being covered.

One 2.5 gallon fire bucket was not onboard during the inspection. Item was added before the inspectors departed.

Name of the vessel was worn. Item was corrected on the spot.

Mastheadl light was burnt out. Light was replaced and fixed on the spot.

Missing lights and whistle on eight PFDs. Item was corrected on the spot.

Caulking around pipe that ran through transverse bulkhead became unseated and cracked. Item repaired on the spot.

Did not have 03 2.5gal fire buckets. Had 02 5gal fire buckets.

Missing 01 red flare.

Gyro compass back back light in operable.

Mast head light lens cracked making it difficult to see illuminated bulb

No instructions for operation of engine room fixed fire extinguishing system.

Replace 8 man lifefloat IAW 46 CFR 180.200. Vessel is restricted to 43 total persons onboard.

Remove Dead end wires in wheel house.

Replace water tight globe for water light to life float IAW 46CFR 180.175 (f) (4).

Re-label remote emergency fuel valve and label direction of operation IAW182.455(b) (4)

Make speaker wire connections at operating station permanent.

Register EPIRB through NOAA.

Repair/Replace cracked bridge window.

Make distribution panel on bridge dead front.

Provide drip shield for electrical distribution panel in lazerette.

Provide lens/globe for overhead lights on port side of engine room.

Refasten weak link for lifefloats to vessel.

Provide two 2 1/2 gallon fire buckets.

Replace buoyant lifeline.

Provide new paddle for lifefloat and properly lash all paddles.

Arrange lifefloats to float free.

Provide adequate closure devices for compartment vent opennings on main deck.

Provide adequate arrangement of dive compressed air hoses routing to the maindeck through vent openning.

Replace flex hose with metallic piping in engine room from bilge manifold to bilge pump and from bilge pump to check valve through hull I.A.W. 46 CFR 180.710(c)(1)

Temporarily provide one watertight closure device for each natural air vent located on the main deck. For spaces protected by fixed extinguishing system, closure device must be readily available and mounted in the vicinity of the vent. Owner may elect for non-protected spaces to raise the height of vent opening to 12 inches instead of providing closure device.

Temporarily amend emergency instruction list to include specific procedures for installation of closure devices on all main vent openings during rough weather or when crossing hazardous bars.

Permanently reconfigure all natural air vents on main deck to a height of 12 inches w/ installed scoop, cowl, or gooseneck.

Install permanent watertight coaming that is 6 inches high on (2) weathertight doors located starboard/forward and center/aft of the main cabin.

Make temporary repairs to holes in main deck forward of pilothouse in way of former anchor windlass equipment foundation. LEGACY COMPONENT DESC: Deck Plating

Replace plastic (PVC) piping with metallic piping on fixed bilge system servicing bunkroom, fish hold, and lazarette spaces. Maintain submersible electric bilge pumps in good working order until repairs complete. LEGACY COMPONENT DESC: Piping

Install metallic wire supports on cables/wiring every 24 inches in engineroom space.

Install dripshield on switchboard in engineroom.

Replace non-metallic discharge hose to submersible electric bilge pump servicing fish hold with rigid nonmetallic material (PVC) using metallic through bulkhead/hull fittings & valves. LEGACY COMPONENT DESC: Piping

Mark on both sides of engineroom watertight door in 1 inch letters "Watertight Door - Keep Closed".

For all rigid plasitc (PVC) pipes, install at each penetration of a watertight deck/bulkhead an acceptable metallic through deck/bulkhead fitting and metallic cutoff valves as per 46CFR182.720.

Extend all weathertight hatches, vents, and openings located on the main deck to at least 12 inches above the weather deck.
