While u/w in Whitefish Bay, ships engineers determined that the #1 cylinder head on the port main had failed. Vessel is e/r to Waiska Bay Anchorage to effect repairs. Once repaired conduct engine trials, and have Captain and Chief Engineer provide written attestations as to the successful completion to the satisfaction of the Coast Guard.

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with sub-chapter F (marine engineering). The vessel reported that the port main engine shut down as designed following an oil mist detection alarm. Vessel is required to make satisfactory repairs to the port main engine and return to operational service.

The installation of all ship service generating systems, shall be in accordance with the requirements of subchapter J (Electrical Engineering); exciter on #2 gen reportedly burned out, rendering gen inoperable - vessel is down to three SSDGs, plus shaft generator. In discussion with vessel, this meets all power generating needs for the vessel conducting any operation and still satisfies minimum subchapter J requirements. Sector NGL and Sector Detroit briefed for concurrent clearance with no extra restrictions requested. Conduct repair to satisfaction of cognizant OCMI within 14 days. 46 CFR 96.05-1(a)

Where an item, or method of construction, or testing is required to meet the standards established by the American Bureau of Shipping, the current standards in effect at the time of construction of the vessel, or otherwise as applicable, shall be used. Vessel's boom railing was damage when boom was rotated for cargo offload. Vessel shall made permanent repairs to the satisfaction of the local OCMI.

Each boiler, including superheater, reheater, economizer, auxiliary boiler, low-pressure heating boiler, and unfired steam boiler, must be available for examination by the marine inspector at intervals specified by table 1 to § 61.05-10. Starboard & port side economizers are due for a hydro test and a fireside inspection.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. Vessel self reported an irregularity on the STBD main engine clutch. Vessel is to have a qualified third party complete repairs, provide technician's report on repairs, and a statement from Chief Engineer attesting to the proper operation of the ship's propulsion. 46 CFR 58.05.01

The following discrepancies were not able to be completed during 2025 drydocking and will need to be addressed during 2026 layup season: 1)Bulkhead 88 second strake from spar decks needs to be replaced. 2)9 Port Ballast Tank Frame 227: Insert at bilge turn bottom seam is not welded to hull. 3)1 Stbd Ballast Tank Upper Longitudinal: Longitudinal frame was partially replaced but welded to bad steel on aft end at bulkhead. Frame needs to continue through bulkhead into 2 stbd ballast tank.

Vessel reports loss of SME due to failed "cam-follower". Vessel is authorized to transit from current position on Lake Superior into the Port of Duluth/Superior (on PME) to affect repairs. Make repairs to, and demonstrate proper function of, the SME, to the satisfaction of USCG MSU Duluth. Provide copy of technician's report to MSU Duluth prior to departure from port.

Vessel reported failed #2 start air valve on port main engine. Repair within OCMI zone and send attestation letter to local marine inspector.

The survey shall be such as to ensure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel shut down port main diesel engine due to starter valve issue on the port MDE. Vessel has parts on-board and effected repair. 46 CFR 91.30-1

Vessel experienced issues with starter air valves on STBD MDE and Port MDE #5 cylinder. Vessel repaired STBD MDE and will proceed to Thunder Bay anchorage IVO Alpena, MI. Vessel is having parts delivered and will effect repair. The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. The vessel noticed the start air valve on the #1 cylinder on the stbd main engine was over heating and took the engine off line to trouble shoot.

Master made notification to [name withheld], of crew shortage due to consent based discharge of AB Jake C. Smith, ref #2765623. The vessel is authorized to sail with one less AB until 28 September 2023. Three person watch system and work/rest hours must maintained in accordance with applicable laws. The vessel/company shall make reasonable efforts at each port to find a replacement.

The start air valve on the #5 cylinder on the port main diesel engine failed resulting in the shut down of the port main reducing the vessels propulsion and maneuverability. Make appropriate repairs and test to the satisfaction of ABS marine surveyors or the Coast Guard.

All installations shall be of marine type engines suitable for the intended service. Vessel reported that one of the pistons on the Port MDE was damaged and required repair. Vessel has requested 2 weeks to carry out repairs and to transit on the starboard engine only during that time. Vessel has been given the ok to transit on one engine and advised to maintain extra rounds in the engine room. Report any changes to the local [name withheld] is to use tug assist at his discretion with the exception of MSU Duluth AOR. While transiting in and out of port in the Duluth AOR the vessel is required to have a tug assist. Repairs to be made to the satisfaction of the attending Marine Inspector or to the attending Class Surveyor by 9 June 2022. 46 CFR 90.20-1 46 CFR 58.10-10(a)

Vessel reports pinhole weeper in ballast piping between the #5S ballast tank and the pump (located within conveyor tunnel space). A temporary soft patch has been applied and has been deemed effective. Vessel authorized to continue operating with temporary repair in place until permanent repairs are completed underway on next upbound trip on Lake Superior. Temporary repair must be examined daily, until permanent repairs are completed, to insure watertight integrity is maintained. Notify MSU Duluth once repairs are completed.

Port main engine brought out of service due to a failure in the #1 cylinder. Vessel shall conduct repairs and provide statement attesting to proper operation. Vessel shall keep local OCMI informed of any changes in propulsion capabilities.

Starboard main engine not operational due to water in crankcase. Vessel is authorized to operate with reduced propulsion on the port engine. Vessel shall conduct repairs and provide inspection/testing attesting to proper operation. Vessel shall keep local OCMI informed of any changes in propulsion capabilities.

[name withheld] reported 6 fractures, 3-6" in length, on the spar deck at hatch 31 and 32. Fractures are located above the #5 cargo hold, starboard side. Vessel is in a loaded condition with coal on board. Vessel to continue to St. Clair power plant and Monroe power plant to off load cargo then to proceed to Duluth, MN for permanent repairs at Fraser Shipyard. Repairs are to be made to the satisfaction of the attending Coast Guard Marine Inspector and Class.

On #2 SSDG, a plug came out of the exhaust side of the turbo charger and fell into the exhaust manifold. Properly replace plug and conduct operational testing. Provide a statement from the Chief Engineer attesting to the repair and operational testing, to the satisfaction of the [name withheld].

port boiler experienced abnormal operation with damage to the rear inspection covers. Have fire side and water side inspection completed by CG marine inspector and third party technical representitive prove proper operation prior to putting boiler back in service.

The inboard safety relief valve (SRV) for the stbd auxiliary boiler (#10840) did not satisfactorily relieve pressure at or below the system MAWP during annual testing. Repair, or replace, the SRV and demonstrate proper operation in the presence of a USCG Marine Inspector IAW 46 CFR 61.05-20.

THE EMERGENCY GENERATOR OVER SPEED SHUT DOWN IS INOPERATIVE. MAKE REPAIRS TO THE E-GEN AND PROVE PROPER OPERATION OF THE OVER SPEED SHUT DOWN. IF THE E-GEN IS OPERATING, AN ENGINEER SHALL BE ON WATCH IN THE SPACE TO MONITOR ENGINE SPEED. REPAIRS TO BE MADE PRIOR TO SEPTEMBER 08, 2018.

THE #3 SSDG IS INOPERATIVE. MAKE REPAIRS TO THE #3 SSDG FUEL INJECTION PUMP AND PROVE PROPER OPERATION OF THE #3 SSDG. REPAIRS ARE TO BE MADE PRIOR TO DEPARTING NEXT PORT OF ST. CLAIR, MI.

The starboard main engine was taken offline due to a worn cam lobe. The starboard main engine must be repaired to the satisfaction of the cognizant OCMI.

Conduct permanent repairs and provide written statement attesting to the proper operation of the starboard start air compressor.

Temporary repairs made to spider fracture at base of starboard anchor pocket. Damage caused by anchor fluke. This area to be cropped and renewed during 2018 dry dock. Also, examine other insets in both anchor pockets casued by flukes and repair if necessary. Steel at base of pockets show considerable grooving from anchor chain. This area should also be examined and renewed if necessary.
