The scantlings, material, and workmanship, the subdivision of cargo spaces, the arrangement of cofferdams, the testing of tanks and cofferdams, shall be at least equivalent to the requirements of a recognized classification society for the particular service specified in the application for the certificate of inspection and permit for the transportation of liquid flammable cargoes in bulk as of the date when the tank vessel was built or as of the date when the vessel was converted into a tank vessel. In the absence of such classification requirements, the Officer in Charge, Marine Inspection, shall satisfy himself that the vessel's structure as specified in this section is safe for the service to be specified in its certificate of inspection. Vessel suffered damage to the port bow, reported a 4'W x 3'L x 6" deep inset. Vessel is authorized one time transit in accordance with proposed voyage plan to Vopak Taft, LA for discharge, then to FMT Shipyard Greta, LA for permanent repairs to satisfaction of USCG prior to carriage of cargo.

Barge came into shipyard on a transit plan from Sector NOLA for substantial hull damage to Port and Starboard gunwale, side shell, bilge knuckle, and bottom plate. Repairs were made in the #1S, #2S, #6S Hopper Voids and Stern Voids to make the vessel seaworthy. Extensive hull damage remains throughout the barge. Prior to loading cargo, make permanent repairs to the satisfaction of the attending CG Marine Inspector.

All tank vessels shall conform to the requirements in this subpart in construction of hulls. Vsl was reported with damage received from Hurricane Ida and currently aground IVO MM 113. Vsl shall provide damage survey and salvage plan report to the satisfaction of the CG prior to movements. When major alterations or major repairs of tank vessels become necessary the work shall be done under the direction of the Officer in Charge, Marine Inspection, and shall be in accordance with the regulations in effect for new construction insofar as possible. When minor alterations or minor repairs of tank vessels become necessary such work shall be under the direction of the Officer in Charge, Marine Inspection, and shall be in accordance with the regulations in effect at the time the vessel was contracted for or built, or in accordance with the regulations in effect for new construction insofar as possible. Vsl was reported with damage received from Hurricane Ida Vsl shall provide damage survey and salvage plan report to the satisfaction of the CG prior to movements.

D8 TBSIP SELF-REPORTED DEFICIENCY. Make permanent repair to 4" x 1/8" fracture in the port stern side shell at the 5' draft mark right at the 3rd side long

Found a 24" fracture on the stbd stern corner wrapper located at the 6' draft mark.

Vessel reports 3" x 1/16" fracture in corner wrapper second corner bracket up from the bottom

1/8 " Hole on bottom 25 ' aft of # 2 wing tank stbd side 1' inboard .Temp repair is foam patch with plwood and mechanical means of compressing.

Found a 4 1/2' x 4" inset into the port side shell just below the deck knuckle located on the #3 void. Crop and renew to original scantlings.

Repair the port stern corner of barge. Two large fractures where discovered (12" deck/gunnel and 2' vertical corner). Conduct a thorough internal examination of stern to determine the extent of internal damage.

6" x 1/2" and 12" x 1/2" fractures found starboard corner wrapper stern box.

Hull Plating - Fractured

Upon inspection of satisfactory temporary patch repair to damage to headlog of bow rake, authorized barge to load cargo and to proceed on single voyage to Freeport, TX. Barge owner to notify Houston-Galveston OCMI upon arrival in zone and submit permanent repair proposal no later than 26 NOV 2003.

PROVIDE DOCUMENTATION TO TESTING OF SEVEN 1" VALVES ON PIPELINE PRV TO THE SATISFACTION OF LOCAL OCMI.

MAIN DECK INSERTS WERE SHOWING SIGNS OF WASTAGE AND CORROSION. REQUIRED TO REMOVE PAINT AND INSPECT WELD TO INSERTS.

REPAIR LADDER HAND RAIL BASES ON TANK TOP.

BENCH TEST 3 SAFETY RELIEF VALVES WHICH HAD BEEN REMOVED FROM THE VESSEL AND DELIVERED TO ST. GABRIEL VALVE SERVICE. SATISFACTORY TEST TO BE WITNESSED BY C.G. MARINE INSPECTOR.

BENCH TEST 3 SAFETY RELIEF VALVES REMAINING ON BARGE DUE LOAD OF BUTADIENE.

MAKE PROPER REPAIR TO 3 INCH FRACTURE ON MAIN DECK IN WAY OF PORT STERN VOID.

REPAIR OR RENEW ONE DOG ON BOW VOID PORT SIDE HATCH.

RENEW OR REPLACE #5 STARBOARD WING VOID AND STERN VOID HATCH COVERS AND BUILD UP KNIFE EDGES.

CONDUCT SATISFACTORY OPERATIONAL TEST OF THE AIR OPERATED EMERGENCY SHUTDOWN SYSTEM IN THE PRESENCE OF A C. G. MARINE INSPECTOR. LOADING OF CARGO PROHIBITED.

BENCH TEST SEVEN SAFETY RELIEF VALVES ON CARGO PIPING BY-PASS TO THE SATISFACTION A C. G. MARINE INSPECTOR.

TEST SECOND SET OF SAFETY VALVES IN THE PRESENCE OF C.G. INSPECTOR.

STENCIL MAX PRESSURE OF CARGO TANKS ON HOPPER COVER.

REPAINT DRAFT MARKS.

EFFECT PERMANENT REPAIRS TO APPROXIMATELY 6'X 4' HOLE AND ASSOCIATED_x001C_ DISTORTED FRAMING STBD BOW HEADLOG CORNER.

REPAIR DAMAGE TO BOW RAKE STBD SIDE IN AREA OF KNUCKLE WELD. REPAIRS MUST BE COMPLETED PRIOR TO ON LOADING ADDITIONAL CARGO.

TEST SAFETY RELIEF VALVES (ALL) IN THE PRESENCE OF A U.S. COAST GUARD MARINE INSPECTOR AND INSTALL ON SUBJECT VESSEL.

ALL REPAIRS ARE TO BE COMPLETED PRIOR TO 05FEB86 TO THE SATISFACTION OF THECOGNIZANT [name withheld] PRIOR TO RELOADING ANY CARGO.

TOWING VESSEL PERSONNEL SHALL MAINTAIN REPAIRS AND CHECK FOR ANY LEAKAGE INTO STERN VOID.

BARGE IS ALLOWED TO MAKE ONE VOYAGE TO FREEPORT, TX TO DISCHARGE PRESENT CARGO AND RETURN TO BATON ROUGE AREA FOR PERMANENT REPAIRS.

MAKE AND MAINTAIN TEMPORARY REPAIRS TO FRACTURES ON STERN VOID.
