Every oil tanker of 150 gross tons and above and every ship other than an oil tanker of 400 gross tons and above shall carry on board a shipboard oil pollution emergency plan approved by the administration. The plan must meet the guidelines of MEPC 54(32. Initially, when asked, the OIM was unable to present an approved SOPEP. When a SOPEP was presented, Annex II of the plan (points of contact) was not current.

The operator of each manned OCS facility shall develop an Emergency Evacuation Plan (EEP) for the facility. Initially, OIM was unable to provide an approved EEP. When an approved EEP was presented, the USCG approval letter made mention that the EEP was approved when evacuation via helicopter procedures was added to the plan. The plan presented did not have helicopter evacuation procedures.

Where pressure indicating gages are used, they are chosen to be compatible with the pressure to be indicated. In no case, shall the upper range of the gage be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function. Attending MODU examiners examined 4 newly installed ASME approved air receivers. 3 out of 4 air receivers had an MAWP of 200 psi, relief valves set at 150 psi, and gages that maxed at 160 psi. Lifeboat air pressure gages were also found faulty. Replace to the satisfaction of the USCG.

Each lifeboat must be launched with its assigned operating crew aboard and manuevered in the water at least once every 3 months, during an abandonment drill. At the time of the exam, the cerw was in excess of 3 months since the last time each lifeboat was launched. Lower all lifeboats within 30 days to the satisfaction of USCG.

The units lifeboats do not installed "white listed" release hooks as per IMO MSC/CIRC 1392. USCG CG-ENG policy letter 01-14 has identified non "white listed" hooks to pose an additional hazard to vessel personnel. 33 CFR 142.4(c)(1)
