Vessel must maintain adequate and up-to-date charts and publications to include: Coast Pilot [name withheld], Tides, and Currents. Unknown when the Coast Pilot was last updated. The Light List was from 2015. The Tides Table was for 2019. There was no currents table. Required all the above publications to be updated. 46 CFR 184.420 (a)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. The plug on the ring buoy was deteriorated from the sun and easily disintegrated upon touching it. Ring Buoy must be replaced and the line connecting the life line to the ring buoy was disintegrating and must be replaced. Master plans to splice a new line. Ring buoy must be replaced prior to departure. 46 CFR 176.808 (b)

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station:(1) A light to indicate discharge;(2) An audible alarm that sounds upon discharge; and (3) A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. The Fireboy installed in the engine/machinery space was unable to shut the engines down and did not alarm at the operating station. Required to rewire the system as per manufacturers instructions and perform operational test in presence of marine inspector prior to departure. 46 CFR 181.420

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant OCMI. The second means of escape for the below deck accommodation space does not have a sign on the main deck side of the hatch. Required to place the above markings on the hatch. 46 CFR 185.606

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening a means of escape must be obvious, rapid, and of adequate strength. Handles and securing devices must be permanently installed and not capable of being easily removed. A door, hatch or scuttle must open towards the expected direction of escape from the space served. The secondary emergency escape in the accommodation area leads up to the main deck. The hatch is fairly heavy and the jib boom is directly over the hatch opening and prevents the hatch from being fully opened. Discussed the concerns with the Master, Master plans to rig the topping lift and boom at a higher angle which will allow the hatch to open enough for persons to escape. Also required a post to be installed to hold the hatch in place when opened. Required to fix the above prior to departure. 46 CFR 177.500 (h)

Footholds, handholds, ladders, and similar means provided to aid escape, must be suitable for use in emergency conditions, of rigid construction, and permanently fixed in position, unless they can be folded, yet brought into immediate service in an emergency. Currently, it is difficult to push the secondary emergency escape hatch in the accommodation space open and climb out on to the main deck. Required to install a step for easier and universal accessibility. Required to fix the above prior to departure. 46 CFR 177.500 (m)

Provide an updated chart. Last update completed 5 Dec 2014.

Provide a first aid kit with contents listed in 46 CFR 160.041-4

Provide at least (6) distress flares.

Provide DAPI information to include employee assistance information, hotline numbers, and DER/Crew training.

Provide a working floating light for the ring buoy. Restricted to daylight operations.

Provide a fixed fire extinguishing system for the engine room.

Provide (02) adult life jackets. Total persons allowed reduced to 10.

Provide a rigging inventory and maintenance inspection program IAW NVIC 02-16.
