Subject to the provisions of 3.16.1.2 and 3.16.1.3, the fire safety requirements of bulkheads, decks, doors, and stairways should, as far as practicable and reasonable in the opinion of the administration, comply with the applicable requirements of the 1974 SOLAS convention for such bulkheads, doors, and stairways. PSCO observed that, excluding the metal doors to the engineer room , all doors fitted through the accommodation spaces and leading up to the bridge are wooden, not self-closing, and not designed to prevent the passage of flame or smoke. Structural members and all blocks, falls, pad eyes, links, fastenings and all other fittings used in connection with launching equipment shall be designed with a factor of safety on the basis of the maximum working load assigned and the ultimate strengths of the materials used for construction. A minimum factor of safety of 4.5 shall be applied to all structural members and a minimum factor of safety of 6 shall be applied to falls, suspension chains, links, and blocks. PSCO observed no maximum working load assigned with consideration to factors of safety and no safe working load test conducted to verify these conditions are met. Each lifeboat shall be fitted with a permanently affixed approval plate endorsed by the administration or its representative, containing at least the following items: manufacturer’s name and address; lifeboat model and serial number; month and year of manufacture; number of persons the lifeboat is approved to carry ; and the approval information required under paragraph 1.2.2.9 and each production lifeboat shall be provided with a certificate of declaration of conformity. The power supply of daylight signaling lamps should be marked externally with the identification of the manufacturer; equipment type or model identification under which it was type tested; and serial number of the unit. In every ship, spaces containing main or auxiliary oil-fired boilers or fuel oil units, should be provided with the following fixed fire-extinguishing systems in accordance with the provisions of section 3.5. Means shall be provided for automatically giving audible warning of the release of fire-extinguishing medium into any ro-ro spaces and other spaces in which personnel normally work or to which they have access. The pre-discharge alarm shall be automatically activiated (e.g. by opening of the release cabinet door). The alarm shall operate for the length of time needed to evacuate the space, but in no case less than 20 s before the medium is released. Conventional cargo spaces and small spaces (such as compressor rooms, paint lockers, etc.) with only a local release need not be provided with such an alarm. PSCOs observed that the carbon dioxide system's controls are not located in a release box fitted with a pre-discharge alarm that is automatically activated by opening the release box door.
